Myasthenia Gravis (MG) একটা chronic autoimmune neuromuscular disorder, যেটাতে skeletal muscles-এর weakness হয়—বিশেষ করে eyes, face, throat ও limb muscles-এ।
চলো ধাপে ধাপে এর pathophysiology বোঝার চেষ্টা করি:
1. Normal physiology (সংক্ষিপ্তভাবে):
• আমাদের voluntary muscle contraction হয় যখন motor neuron থেকে acetylcholine (ACh) release হয়ে neuromuscular junction-এর ওপরের nicotinic ACh receptor-এর সঙ্গে bind করে।
• এতে muscle fiber depolarize হয়ে contraction হয়।
2. Myasthenia Gravis-এ কী হয়?
Autoimmune attack হয় acetylcholine receptor-এর বিরুদ্ধে।
Step-by-step breakdown:
Autoantibody production:
শরীরের immune system ভুল করে anti-ACh receptor antibody (IgG type) তৈরি করে।
Receptor blockage & destruction:
এই antibody গুলো এসে ACh receptor-এ bind করে, ফলে:
• ACh আর receptor-এ bind করতে পারে না → muscle depolarization হয় না → weakness হয়।
• Complement system activate হয়ে receptor-গুলোও destroy হয়।
3. Decreased receptor number:
Receptor গুলোর সংখ্যা কমে যায় এবং junction এর structure ভেঙে যায় → transmission efficiency কমে যায়।
4. Result:
Muscle গুলো contract করতে পারে না, বা খুব তাড়াতাড়ি fatigued হয়ে পড়ে → fluctuating weakness হয় (rest দিলে কিছুটা recover করে)।
3. কিছু special variant:
• Anti-MuSK antibody positive MG:
যারা AChR antibody negative, তাদের অনেকের শরীরে anti-MuSK (muscle-specific kinase) antibody পাওয়া যায়, যেটা neuromuscular junction-এর structure এবং receptor clustering disturb করে।
4. Summary কী হবে তাহলে:
• MG-তে autoantibody তৈরি হয় ACh receptor বা MuSK-এর বিরুদ্ধে।
• এতে ACh আর muscle-এর receptor-এ bind করতে পারে না, ফলে muscle contraction হয় না বা দুর্বল হয়।
• এটা fluctuating, অর্থাৎ দিনে দিনে বা কাজের পর weakness বাড়ে, আর rest দিলে কিছুটা কমে।
5. Treatment hint (সংক্ষেপে):
• Acetylcholinesterase inhibitor (e.g., pyridostigmine) → synapse-এ ACh এর পরিমাণ বাড়ায়।
• Immunosuppressants (e.g., steroids) → antibody production কমায়।
• Plasmapheresis/IVIG → antibody remove করতে সাহায্য করে।
এখন এড্রোফোনিয়াম কী করে?
Mechanism of Action:
Edrophonium → acetylcholinesterase enzyme কে block করে।
ফলে synapse-এ acetylcholine-এর পরিমাণ বেড়ে যায়।
ACh receptor যেগুলো এখনও unblock আছে, সেগুলোর সাথে অধিক ACh bind করে → muscle contraction একটু ভাল হয়।
