Shock
একটা Trauma এর পেশেন্ট active bleeding নিয়ে আসলো। আপনি হুরমুড়িয়ে একটা hartmanns solution এবং blood নিয়ে রোগীর কাছে দৌড় দিলেন। রোগীর ফ্লুইড রিসাসিটেশন করে দেখলেন যে রোগীর ব্লিডিং আরো বেড়ে গেছে। এখন আপনি কি করবেন?
কারো যদি active bleeding এর কারণে shock ডেভেলপ করে তাহলে প্রথমেই active bleeding stop করতে হবে। bleeding stop করার পর ফ্লুইড রিসোসিয়েশন করতে হবে। একে বলা হয় damage control resuscitation.
কোনো ongoing hemorrhage এর পেশেন্টকে যদি repeated volume resuscitation করা হয় তাহলে পেশেন্ট এর physiological exhaution ( profound coagulopathy, acidosis, hypothermia) develop করবে এবং subsequently death হবে।
কারণ একটা trauma এর পেশেন্ট যার active bleeding হচ্ছে তার এমনিতেই acute traumatic coagulopathy develop করে। তখন যদি fluid and blood transfusion করা হয় তাহলে coagulation factor dilution হয়ে pre-existing coagulopathy কে আরো worsen করতে পারে। তাই আগে fluid resuscitation না করে bleeding stop করতে হবে।
এখানে bleeding stop করা যেতে পারে FFP( fresh frozen plasma) দিয়ে।
