Malignant Hypertension
রহিমা বেগম, বয়স ৬৫ বছর। তিনি দীর্ঘদিন যাবত bizoran 5/40 খান। এতে তার বিপি কন্ট্রোলেই ছিলো। কিন্তু হঠাৎ করে তার বিপি ১৯০/১১০ mmhg নিয়ে আসলো। আপনি কিভাবে ম্যানেজ করবেন?
প্রথমেই তার কোনো Target organ damage আছে কিনা সেটা দেখতে হবে।
Target organ damage আছে নাকি কেমনে বুঝবো?
1. Hypertensive encephalopathy আছে কিনা দেখতে হবে। তার consciousness, activity, GCS score ইত্যাদি দেখতে হবে।
2. Retinopathy আছে কিনা দেখতে হবে। তাকে জিজ্ঞেস করতে হবে হঠাৎ করে চোখে ঝাপসা দেখছেন কিনা।
3. Renal dysfunction আছে কিনা চেক করতে হবে।
4. Stroke এর কোন ফিচার আছে কিনা দেখতে হবে। যদি থাকে তাহলে কোনটা? ischemic or hemorrhagic।
যদি এগুলো থেকে থাকে তাহলে এটাকে malignant hypertension বলে।
That means, malignant or accelerated HTN is Bp more than 180/110 mmhg + target organ damage. একে অনেক সময় Hypertensive emergency ও বলা হয়।
Management :
কোনো ভাবেই বিপি হঠাত করে একদম কমায় ফেলা যাবে না। এতে করে tissue perfusion কমে গিয়ে cerebral damage যেমন occipital blindness এবং heart failure, renal insufficiency কে আরো Precipitate করতে পারে।
কারো যদি Heart failure or hypertensive encephalopathy থাকে তবুও বিপি একেবারে না কমিয়ে ideally ২৪ – ৪৮ ঘন্টার মধ্যে বিপি কমিয়ে 150/90 mmHg মধ্যে রাখতে হবে।
Treatment :
IV /IM labetalol
IM hydralazine
IV GTN
IV Na nitroprusside
