দুটি রিপোর্টের গ্রাফ, সংখ্যাগত ফলাফল, মন্তব্য এবং WHO-এর hearing-loss classification মিলিয়ে বিশ্লেষণ করেছি।

সবচেয়ে গুরুত্বপূর্ণ Clinical Finding:

• ১ নম্বর রিপোর্ট: দুই কানেই Type A Tympanogram—মধ্যকর্ণের কার্যকারিতা স্বাভাবিকের সঙ্গে সামঞ্জস্যপূর্ণ।
• ২ নম্বর রিপোর্ট: দুই কানে Hearing Threshold প্রায় 20–25 dB HL। রিপোর্টে Normal লেখা থাকলেও WHO 2021 classification অনুযায়ী এটি mild hearing-loss range-এ পড়তে পারে।
• বিশেষ পর্যবেক্ষণ: বাম কানের Ear Canal Volume 0.555 cc, যা কিছু প্রাপ্তবয়স্ক reference range অনুযায়ী সামান্য কম।
• Otitis Externa: এই দুটি পরীক্ষার মাধ্যমে রোগটি নিশ্চিত বা বাদ দেওয়া যায় না। এজন্য Otoscopic Examination প্রয়োজন।

Clinical confidence: মধ্যকর্ণের কার্যকারিতা স্বাভাবিক হওয়ার সম্ভাবনা উচ্চ (≈0.95)। কিন্তু নির্দিষ্ট কানের রোগের diagnosis দেওয়ার মতো clinical examination বা symptoms এখানে নেই।

B. কোথায় অস্বাভাবিকতা আছে?

🔴 Finding 1: Left Ear Canal Volume — 0.555 cc

প্রাপ্তবয়স্কদের জন্য Audiology Australia-এর একটি reference range হলো 0.6–1.5 mL। সেই অনুযায়ী 0.555 cc সামান্য কম।

তবে Queensland Health-এর অন্য একটি clinical reference range হলো 0.3–1.6 cc। সে অনুযায়ী মানটি স্বাভাবিক।

অতএব, 0.555 cc-কে নিশ্চিত pathological abnormality বলা যাবে না।

সম্ভাব্য কারণ:

• স্বাভাবিকভাবেই ছোট External Auditory Canal
• Cerumen বা কানের ময়লা
• Probe-এর অবস্থান বা canal wall-এর সঙ্গে probe-এর সংস্পর্শ
• বয়স ও কানের গঠনগত পার্থক্য

শুধু এই সংখ্যার ভিত্তিতে Cerumen Impaction বা Otitis Externa diagnosis করা যাবে না।

🔴 Finding 2: Peak Compliance-এর Unit

রিপোর্টে Peak Compliance-এর পাশে daPa লেখা হয়েছে।

এখানে একটি reporting issue আছে।

• daPa: Pressure-এর unit।
• mL/cc: Equivalent compliance-এর প্রচলিত unit।

Compliance-এর সংখ্যা 1.377 এবং 1.191 স্বাভাবিক adult range-এর মধ্যে আছে। তাই unit-এর এই অসংগতি রোগ নির্দেশ করে না।

🟢 Finding 3: Bilateral Type A Tympanogram

এটি এই রিপোর্টের সবচেয়ে গুরুত্বপূর্ণ ফলাফল।

Type A Tympanogram নির্দেশ করে যে পরীক্ষার সময় মধ্যকর্ণের চাপ এবং Tympanic Membrane-এর গতিশীলতা স্বাভাবিকের সঙ্গে সামঞ্জস্যপূর্ণ ছিল।

এই ফলাফল গুরুত্বপূর্ণ Middle Ear Effusion, উল্লেখযোগ্য negative middle-ear pressure এবং Tympanic Membrane-এর বড় ধরনের mobility abnormality-এর বিরুদ্ধে যায়।

তবে Type A Tympanogram থাকলেও External Auditory Canal-এর প্রদাহ বা Otitis Externa থাকতে পারে।

✅ Report 1 — Clinical Impression

Bilateral Type A Tympanograms, consistent with normal middle-ear function.

উভয় কানের টিম্পানোগ্রাম Type A (স্বাভাবিক) পাওয়া গেছে, যা নির্দেশ করে যে দুই কানের মধ্যকর্ণের কার্যকারিতা স্বাভাবিক রয়েছে।

• Bilateral: উভয় কান (ডান ও বাম)।
• Type A Tympanogram: কানের পর্দা ও মধ্যকর্ণের চাপ এবং নড়াচড়া স্বাভাবিক থাকার নির্দেশক।
• Normal Middle-Ear Function: মধ্যকর্ণের চাপ ও শব্দ পরিবহন ব্যবস্থার যান্ত্রিক কার্যকারিতা স্বাভাবিক থাকার সঙ্গে সামঞ্জস্যপূর্ণ।

✅ Clinical Interpretation:

Diagnosis/Report Finding: Bilateral Type A Tympanogram (Normal Middle-Ear Function).

বাংলা: উভয় কানের মধ্যকর্ণের কার্যকারিতা স্বাভাবিক।

গুরুত্বপূর্ণ: Type A Tympanogram পাওয়া গেলেও শ্রবণশক্তি সম্পূর্ণ স্বাভাবিক—এটি নিশ্চিত করা যায় না। শ্রবণশক্তি মূল্যায়নের জন্য Pure Tone Audiometry (PTA) প্রয়োজন হতে পারে।

🔴 সবচেয়ে গুরুত্বপূর্ণ Finding: WHO Classification-এর পার্থক্য

রিপোর্টে লেখা:

−10 to 25 dB = Hearing Within Normal Limit

এটি প্রচলিত clinical audiology classification-এর সঙ্গে সামঞ্জস্যপূর্ণ।

তবে WHO-এর World Report on Hearing (2021) অনুযায়ী প্রাপ্তবয়স্কদের better-ear four-frequency PTA-এর শ্রেণিবিভাগ:

এই রোগীর গ্রাফ থেকে 500, 1000, 2000 এবং 4000 Hz-এর আনুমানিক গড়:

Right Ear PTA ≈ 24 dB HL

Left Ear PTA ≈ 21 dB HL

ফলে WHO 2021-এর epidemiological grading ব্যবহার করলে better-ear average mild hearing-loss range-এ পড়তে পারে।

কিন্তু এই classification একাই রোগ, শ্রবণপ্রতিবন্ধিতা বা চিকিৎসার প্রয়োজন নির্ধারণ করে না। Speech understanding, বয়স, উপসর্গ এবং test reliability গুরুত্বপূর্ণ।

C. Air–Bone Gap

গ্রাফে কিছু frequency-তে আনুমানিক 5–10 dB Air–Bone Gap দেখা যাচ্ছে।

এত ছোট পার্থক্য পরীক্ষার স্বাভাবিক variability-এর মধ্যেও থাকতে পারে। আবার সূক্ষ্ম conductive component থাকলেও এমন হতে পারে।

এখানে একে নিশ্চিত Conductive Hearing Loss বলা যাচ্ছে না। প্রয়োজনে masked bone-conduction thresholds ও পুনরায় audiometry করতে হবে।

✅ Report 2 — Clinical Impression

Bilateral near-normal/borderline pure-tone hearing thresholds, approximately 20–25 dB HL.

✅ বাংলা অর্থ:

উভয় কানের শ্রবণক্ষমতা প্রায় স্বাভাবিক অথবা স্বাভাবিকের সীমান্তবর্তী (Borderline) পর্যায়ে রয়েছে। উভয় কানে শব্দ শোনার সর্বনিম্ন মাত্রা প্রায় ২০–২৫ ডেসিবেল হিয়ারিং লেভেল (dB HL)।

🩺 Clinical Meaning (ক্লিনিক্যাল ব্যাখ্যা):

• Bilateral: উভয় কান (ডান ও বাম)।
• Near-normal: শ্রবণক্ষমতা প্রায় স্বাভাবিক।
• Borderline: স্বাভাবিক শ্রবণক্ষমতার সীমান্তবর্তী অবস্থা।
• Pure-tone Hearing Threshold: বিভিন্ন ফ্রিকোয়েন্সির শব্দ রোগী যে সর্বনিম্ন তীব্রতায় শুনতে পান।
• 20–25 dB HL: প্রাপ্তবয়স্কদের ক্ষেত্রে স্বাভাবিক শ্রবণক্ষমতার ঊর্ধ্বসীমার কাছাকাছি; ব্যবহৃত শ্রেণিবিন্যাস অনুযায়ী সামান্য শ্রবণক্ষমতার হ্রাসও নির্দেশ করতে পারে।

✅ Clinical Interpretation:

Audiological Finding: Bilateral near-normal/borderline hearing thresholds (20–25 dB HL).

বাংলা: উভয় কানের শ্রবণক্ষমতা প্রায় স্বাভাবিক, তবে স্বাভাবিকের নিম্নসীমায় রয়েছে।

গুরুত্বপূর্ণ: শুধু ২০–২৫ dB HL ফলাফল দেখে নির্দিষ্ট ধরনের Hearing Loss নিশ্চিত করা যায় না। সম্পূর্ণ Pure Tone Audiometry (PTA) রিপোর্টে Air Conduction, Bone Conduction এবং Air–Bone Gap মূল্যায়ন করতে হবে।

WHO 2021 grading অনুযায়ী estimated PTA mild hearing-loss range-এ পড়তে পারে।

Confidence: 0.90 — Threshold range সম্পর্কে।
Confidence: 0.65 — ছবির ভিত্তিতে নির্ণীত WHO severity category সম্পর্কে; numerical audiometry দিয়ে যাচাই প্রয়োজন।

৩. Differential Diagnosis — যদি রোগীর কানে ব্যথা, চুলকানি বা অস্বস্তি থাকে

রিপোর্ট দুটি নিজে কোনো নির্দিষ্ট রোগ শনাক্ত করছে না। তবে রোগীর উপসর্গ থাকলে নিচের রোগগুলো বিবেচনায় আসবে।

এই রোগগুলোর কোনো একটিকেও বর্তমান দুটি রিপোর্ট দিয়ে নিশ্চিত করা যাচ্ছে না।

Acute Otitis Externa নির্ণয়ের প্রধান ভিত্তি হলো উপসর্গ এবং ear-canal examination—বিশেষ করে tragus বা pinna নড়ালে ব্যথা এবং canal erythema বা edema।

৪. Management / Treatment Plan

A. Doctor এখন কী করবেন?

প্রথম কাজ: Otoscopic Examination.

চিকিৎসককে রোগীর presenting symptoms, onset, affected ear, ear discharge, itching, tragal tenderness, fever, swimming/water exposure, ear instrumentation এবং পূর্ববর্তী চিকিৎসার ইতিহাস নিতে হবে।

এরপর উভয় কানের External Auditory Canal ও Tympanic Membrane দেখতে হবে।

B. যদি Otitis Externa নিশ্চিত হয়

Uncomplicated Acute Otitis Externa-এর ক্ষেত্রে guideline অনুযায়ী topical therapy হলো প্রথম সারির চিকিৎসা।

একটি guideline-supported option:

Ciprofloxacin 0.3% + Dexamethasone 0.1% otic suspension

• বয়স ৬ মাস বা বেশি হলে: আক্রান্ত কানে 4 drops, দিনে ২ বার, ৭ দিন।
• এটি কেবল clinically confirmed acute otitis externa-এর জন্য একটি treatment option; এই রিপোর্ট দেখে সরাসরি prescription নয়।
• Drug allergy, formulation, otoscopic findings এবং patient factors যাচাই করতে হবে।

অন্য non-ototoxic option হলো Ofloxacin 0.3% otic solution; বয়স ১৩ বছর বা বেশি হলে labelled otitis-externa regimen হলো 10 drops once daily for 7 days।

ব্যথা থাকলে বয়স ও contraindication অনুযায়ী Paracetamol বা উপযুক্ত analgesic ব্যবহার করা যায়।

Uncomplicated Otitis Externa-তে routine oral antibiotics প্রয়োজন হয় না। Tympanic membrane perforation সন্দেহ হলে non-ototoxic preparation নির্বাচন করতে হবে।

কিন্তু রোগীর canal examination স্বাভাবিক হলে, শুধুমাত্র এই দুই রিপোর্টের ভিত্তিতে antibiotic বা steroid ear drops দেওয়া উচিত নয়।

C. Treatment Follow-up

Otitis Externa নিশ্চিত হয়ে চিকিৎসা শুরু করলে 48–72 ঘণ্টায় উন্নতি না হলে diagnosis ও treatment পুনর্মূল্যায়ন করতে হবে।

Diabetes, immunosuppression, severe nocturnal otalgia, granulation tissue, cranial-nerve abnormalities বা infection ছড়িয়ে পড়ার লক্ষণ থাকলে urgent ENT assessment প্রয়োজন।

৬. Prescription-এ কী Diagnosis লিখবেন?

✅ Report-Based Clinical Impression

এই মুহূর্তে সবচেয়ে নির্ভরযোগ্য documentation: Prescription — Audiological Impression

Audiological Impression:

Bilateral Type A tympanograms, consistent with normal middle-ear function.

Bilateral near-normal/borderline pure-tone hearing thresholds (approximately 20–25 dB HL).

Left ear canal volume: 0.555 cc (borderline low by some adult reference ranges).

Etiological diagnosis: Not established.

Advice: ENT consultation for clinical correlation and otoscopic examination.

এটিকে রোগের নিশ্চিত নাম না লিখে Audiological Impression হিসেবে ব্যবহার করুন।

যদি clinical examination-এ নির্দিষ্টভাবে Otitis Externa-এর লক্ষণ পাওয়া যায়, তখন আলাদাভাবে লিখতে পারবেন:

Dx: Acute Diffuse Otitis Externa — [Right / Left / Bilateral], clinically confirmed.

কিন্তু এই রিপোর্ট দুটি দেখে সরাসরি Definite Otitis Externa লেখা বৈজ্ঞানিকভাবে সমর্থিত নয়।

Consultant ENT Specialist / Otolaryngologist

বিশেষ করে Otology বিষয়ে অভিজ্ঞ ENT specialist হলে উপযুক্ত।

5. Acoustic reflexes documented as present, without detailed threshold data.

Provisional Impression:

Bilateral near-normal/borderline hearing thresholds with normal middle-ear immittance findings. Etiology of any presenting otologic symptoms remains undetermined.

Reason for Referral:

Kindly evaluate clinically with otoscopy/otoendoscopy to assess the external auditory canals and tympanic membranes, confirm or exclude otitis externa or other external-ear pathology, and advise further management.

Additional audiological evaluation: Repeat PTA with SRT/WRS if clinically indicated.

Final Assessment

বাংলায়: দুই কানের মধ্যকর্ণের কার্যকারিতা স্বাভাবিকের সঙ্গে সামঞ্জস্যপূর্ণ। শ্রবণসীমা প্রচলিত Normal Range-এর ওপরের দিকে রয়েছে; WHO 2021 classification অনুযায়ী সামান্য hearing-threshold elevation থাকতে পারে। বাম কানের Ear Canal Volume কিছু প্রাপ্তবয়স্ক reference range অনুযায়ী সামান্য কম। তবে কোনো নির্দিষ্ট কানের রোগের নিশ্চিত প্রমাণ এই রিপোর্টে পাওয়া যাচ্ছে না।

Actionable Plan

প্রথমে bilateral otoscopy → রোগীর উপসর্গ অনুযায়ী clinical diagnosis → প্রয়োজন হলে targeted investigation → diagnosis অনুযায়ী treatment।

এখানে সবচেয়ে জরুরি অনুপস্থিত তথ্য হচ্ছে রোগীর বয়স, কানে ব্যথা/চুলকানি/স্রাব আছে কি না এবং otoscopic findings।
