এখানে দুটি নির্দিষ্ট সমস্যা শনাক্ত হয়েছে।
Diagnosis 1: Superficial Gastric Fundal Ulcer
পাকস্থলীর Fundus অংশে একটি superficial ulcer।
Diagnosis 2: Post-bulbar Duodenal Ulcer
ডুওডেনামের বাল্বের পরবর্তী অংশে একটি ছোট আলসার।

অর্থাৎ পাকস্থলীর ক্যানসার হয়েছে—এমন প্রমাণ নেই। কিন্তু বায়োপসি ছাড়া ক্যানসার সম্পূর্ণভাবে বাদ দেওয়াও সম্ভব নয়।
British Society of Gastroenterology-এর ২০২৫ সালের guidance অনুযায়ী, gastric ulcer প্রথম endoscopy-তে biopsy না করা হলে সাধারণত দুই সপ্তাহের মধ্যে repeat endoscopy করার পরামর্শ রয়েছে।
এই রিপোর্টের তারিখ ১৮ সেপ্টেম্বর ২০২৬। বর্তমানে পাঁচ সপ্তাহের বেশি সময় অতিক্রান্ত হয়েছে। সুতরাং Gastroenterologist-এর মাধ্যমে দ্রুত repeat OGD with biopsy ব্যবস্থা করা প্রয়োজন।
৪. Etiology — কী কারণে এই রোগ হতে পারে?
Peptic ulcer হলো পাকস্থলী অথবা ডুওডেনামের mucosal barrier ক্ষতিগ্রস্ত হয়ে সৃষ্টি হওয়া আলসার।

কোন কারণটি সবচেয়ে বেশি সম্ভাব্য?
H. pylori infection এবং NSAID exposure—এ দুটি প্রথমে মূল্যায়ন করা উচিত।
Harrison’s Principles of Internal Medicine অনুযায়ী এগুলো peptic ulcer disease-এর প্রধান প্রতিষ্ঠিত কারণ।
তবে এই রিপোর্ট দেখে নির্দিষ্টভাবে বলা যাবে না যে রোগীর আলসার অবশ্যই H. pylori-এর কারণে হয়েছে।

৫. Clinical Features — রোগীর কী কী উপসর্গ থাকতে পারে?
এই রিপোর্টে রোগীর উপসর্গ লেখা নেই। তবে PUD-তে সাধারণত হতে পারে:
- পেটের ওপরের অংশে ব্যথা বা জ্বালাপোড়া।
- খাবারের সঙ্গে সম্পর্কিত পেটব্যথা।
- বমিভাব, পেট ফাঁপা বা দ্রুত পেট ভরে যাওয়ার অনুভূতি।
- ক্ষুধামান্দ্য।
- আলসার থেকে রক্তক্ষরণ হলে কালো পায়খানা বা রক্তবমি।
- কখনো কোনো উপসর্গ ছাড়াও আলসার থাকতে পারে।
Emergency warning: রক্তবমি, কালো আলকাতরার মতো পায়খানা, অজ্ঞান হয়ে যাওয়া, রক্তচাপ কমে যাওয়া অথবা হঠাৎ প্রচণ্ড পেটব্যথা হলে জরুরি বিভাগে পাঠাতে হবে।

সবচেয়ে জরুরি পরীক্ষা
Repeat OGD + targeted gastric ulcer biopsy + histopathology.
বায়োপসি নেওয়ার সময় gastroenterologist আলসারের edge এবং প্রয়োজন অনুযায়ী base থেকে নমুনা নেবেন।
H. pylori testing-এর জন্য প্রয়োজনীয় পৃথক gastric sampling-ও করা যেতে পারে।
PPI গ্রহণ করলে H. pylori stool antigen বা breath test false-negative হতে পারে। সাধারণভাবে পরীক্ষার আগে দুই সপ্তাহ PPI বন্ধ রাখতে হয়, তবে সক্রিয় আলসারে চিকিৎসকের সিদ্ধান্ত ছাড়া PPI বন্ধ করা উচিত নয়।
Gastric ulcer-এর healing নিশ্চিত করতে সাধারণত চিকিৎসা শুরুর ৬–৮ সপ্তাহের মধ্যে endoscopic reassessment করা হয়। কিন্তু এই রোগীর পূর্বের biopsy বাদ পড়েছে, তাই biopsy করার জন্য আরও অপেক্ষা করা উচিত নয়।
৭. 💊 Management / Treatment Plan
A. Proton Pump Inhibitor (PPI)
Uncomplicated gastric ulcer-এর চিকিৎসায় PPI প্রধান acid-suppressing ওষুধ।
প্রচলিত adult regimens:

এগুলো guideline-based উদাহরণ, এই রোগীর জন্য চূড়ান্ত prescription নয়। রোগীর বর্তমান ওষুধ, সহরোগ, উপসর্গ এবং চিকিৎসার ইতিহাস অনুযায়ী ডোজ ও সময়কাল নির্ধারণ করতে হবে।
৬৬ বছর বয়সী এই রোগীর ক্ষেত্রে যদি আগে থেকেই PPI চলতে থাকে, তাহলে কতদিন ও কী ডোজে খাচ্ছেন তা প্রথমে যাচাই করতে হবে।
NICE CG184 peptic ulcer management-এ প্রয়োজন অনুযায়ী ৪–৮ সপ্তাহের acid-suppressive treatment এবং NSAID-associated ulcer-এ আট সপ্তাহের চিকিৎসার পরামর্শ রয়েছে।
B. H. pylori Positive হলে
H. pylori নিশ্চিত হলে eradication therapy প্রয়োজন।
ACG 2024 guideline অনুযায়ী antibiotic susceptibility জানা না থাকলে treatment-naïve রোগীর জন্য 14-day optimized bismuth quadruple therapy একটি recommended regimen।

Bismuth subsalicylate একটি বিকল্প হলেও রোগীর aspirin allergy, anticoagulant use এবং অন্যান্য ঝুঁকি বিবেচনা করতে হবে।
এই চার-ওষুধের regimen নিজে থেকে শুরু করা ঠিক হবে না। রোগীর renal function, অন্যান্য ওষুধ, পূর্বের antibiotic exposure এবং স্থানীয় প্রাপ্যতা যাচাই করে gastroenterologist regimen নির্বাচন করবেন।
H. pylori positive প্রমাণ ছাড়া শুধু endoscopy দেখে eradication antibiotics শুরু করার প্রয়োজন নেই।
চিকিৎসা শেষে সংক্রমণ নির্মূল হয়েছে কি না পরীক্ষা করতে হবে—antibiotic শেষ হওয়ার অন্তত চার সপ্তাহ পরে এবং PPI বন্ধ রাখার অন্তত দুই সপ্তাহ পরে, যখন তা নিরাপদ।
C. অন্যান্য চিকিৎসাগত ব্যবস্থা
NSAID জাতীয় painkiller এড়িয়ে চলতে হবে, যদি সম্ভব হয়। রোগী aspirin, clopidogrel বা anticoagulant গ্রহণ করলে চিকিৎসকের পরামর্শ ছাড়া বন্ধ করা যাবে না।
Smoking থাকলে বন্ধ করতে হবে। যে খাবারে ব্যথা বাড়ে সেগুলো এড়ানো যেতে পারে, কিন্তু শুধু খাদ্য পরিবর্তন দিয়ে আলসারের চিকিৎসা সম্পূর্ণ হয় না।
৮. Differential Diagnosis
বর্তমান endoscopy-তে আলসার পাওয়া গেছে; তাই PUD-ই প্রধান clinical diagnosis।
তবে বিশেষ করে gastric ulcer-এর ক্ষেত্রে নিচের বিষয়গুলো বিবেচনায় রাখতে হবে।

Post-bulbar ulcer দেখেই Zollinger–Ellison syndrome নিশ্চিত করা যায় না। এটি বিরল কারণ এবং নির্দিষ্ট clinical indication ছাড়া routine gastrin testing প্রয়োজন নেই।

O/E: BP 120/80 mmHg on available visits, 48.5 kg in Rx1; anaemia clinically noted but Hb unavailable.
Dx: Endoscopically confirmed PUD — superficial fundal gastric ulcer and small post-bulbar duodenal ulcer. Oral ulceration — aetiology undetermined. H. pylori status unknown. Gastric malignancy not yet excluded.
Investigations: Urgent repeat OGD+biopsy/HPE, H. pylori tests at appropriate washout, CBC+iron studies, B12, renal function as clinically appropriate.
Rx review: Reconcile all previous antibiotics/antifungals, one appropriate PPI, patient allergies/interactions; no blind medication continuation.
Referral note: 66/F with OGD on 18-09-2026 showing gastric fundal ulcer and post-bulbar duodenal ulcer, without biopsy. Kindly arrange urgent upper-GI endoscopy with targeted gastric ulcer biopsy/histopathology, assess H. pylori and previous antibiotic use, optimize therapy and document healing.
