এই পরিবর্তনগুলো cervical intervertebral disc degeneration-এর সঙ্গে সামঞ্জস্যপূর্ণ হতে পারে।
Cervical Spine — Decreased Disc Space at C5–C6 & C6–C7
এর অর্থ: ঘাড়ের মেরুদণ্ডের C5–C6 এবং C6–C7 কশেরুকার মাঝখানের Intervertebral Disc-এর জায়গা স্বাভাবিকের তুলনায় কমে গেছে।
সবচেয়ে সাধারণ কারণ হলো Cervical Disc Degeneration (Degenerative Disc Disease), যা Cervical Spondylosis-এর অংশ হতে পারে।

- Muscle weakness — স্নায়ু আক্রান্ত হলে হাতের পেশির শক্তি কমে যাওয়া।৩. Clinical Diagnosis কী হবে?
- শুধু X-ray-তে এই finding থাকলে সম্ভাব্য রিপোর্টভিত্তিক Impression:
- Cervical Degenerative Disc Disease at C5–C6 and C6–C7.
- তবে নিশ্চিত রোগনির্ণয়ের জন্য রোগীর বয়স, উপসর্গ, Neurological Examination এবং পুরো X-ray রিপোর্ট বিবেচনা করতে হবে।
- Confidence: 0.95 — বয়সজনিত ডিস্ক ডিজেনারেশন একটি সাধারণ ব্যাখ্যা। তবে নির্দিষ্ট রোগীর ক্ষেত্রে এটিই কারণ কি না, শুধু এই finding থেকে নিশ্চিত হওয়া যায় না।
- ৪. Investigation & Management
- Investigation: হাতে ব্যথা, অবশভাব বা দুর্বলতা থাকলে এবং বিশেষ করে দীর্ঘস্থায়ী radiculopathy-এর ক্ষেত্রে MRI Cervical Spine without Contrast উপযুক্ত হতে পারে।
- Treatment:
- Physiotherapy ও cervical strengthening exercises।
- Posture correction।
- ব্যথা থাকলে উপযুক্ত analgesic/NSAID, contraindication বিবেচনা করে।
- Progressive neurological weakness বা spinal cord compression থাকলে specialist assessment।

Used in diseases:

Clinical assessment: Indomethacin এবং Diclofenac একই NSAID শ্রেণির। তাই একই সময়ে ব্যবহারে adverse effects, বিশেষ করে gastrointestinal bleeding ও renal injury-এর ঝুঁকি বাড়তে পারে। Suppository হওয়ার কারণে systemic NSAID toxicity-এর ঝুঁকি দূর হয় না।

গরম সেঁক, উপযুক্ত posture, exercise ও দীর্ঘক্ষণ একই অবস্থানে না থাকা উপকারী হতে পারে।
তবে uncomplicated neck pain-এ routine দীর্ঘমেয়াদি cervical collar ব্যবহার সাধারণত সুপারিশ করা হয় না। কারণ এতে ঘাড়ের স্বাভাবিক নড়াচড়া কমে যেতে পারে।
2025 সালের Clinical Practice Guideline: Nonspecific Neck Pain রোগীকে সক্রিয় রাখা, exercise এবং self-management-এর ওপর গুরুত্ব দিয়েছে।
PART 5: My Working Clinical Diagnosis
1. Definition
Cervical Spondylosis হলো cervical spine-এর degenerative condition, যেখানে intervertebral disc, vertebral endplate ও facet joints-এ বিভিন্ন পরিবর্তন দেখা দিতে পারে।
2. Etiology / Pathophysiology
সাধারণ সম্ভাব্য কারণ:
বয়সের সঙ্গে disc degeneration, পূর্ববর্তী আঘাত, দীর্ঘ সময় awkward neck posture এবং degenerative joint changes।
রোগীর X-ray report-এ C5–C6 ও C6–C7 disc space কমে যাওয়ার কথা উল্লেখ রয়েছে।
কিন্তু শুধু এই রিপোর্ট থেকে degeneration-এর সুনির্দিষ্ট কারণ বা severity নির্ধারণ করা যায় না।
3. Clinical Features
এই রোগে সাধারণত দেখা যেতে পারে:
ঘাড়ে ব্যথা, stiffness, ঘাড় ঘোরাতে অস্বস্তি এবং কখনো কাঁধের আশপাশে ব্যথা।
এই রোগীর ক্ষেত্রে কেবল neck pain without radiculopathy নথিভুক্ত।

Final Opinion: চিকিৎসকের Cervical Spondylosis diagnosis যুক্তিসংগত। কিন্তু প্রেসক্রিপশনে documented neurological examination, pain duration এবং গুরুত্বপূর্ণ patient history নেই। ফলে রোগনির্ণয়ের চেয়ে ওষুধগুলোর প্রয়োজনীয়তা ও নিরাপত্তা যাচাইয়ের জায়গায় বেশি প্রশ্ন রয়েছে।

Step 1 — Drug reconciliation: প্রথমে Neucos-B-এর 1+1+1 ডোজ ও দুই মাসের duration নিশ্চিত করুন। এটি আমার কাছে সবচেয়ে গুরুত্বপূর্ণ prescribing-safety concern।
Step 2 — Review NSAID combination: Indomet চলাকালে Voltalin suppository ব্যবহারের বিষয়ে চিকিৎসকের নির্দেশ স্পষ্ট করুন। দুই NSAID একসঙ্গে নেওয়ার ঝুঁকি রয়েছে।
