“Graft is OK” এর অর্থ: কানের পর্দার graft examination-এ ভালো/অক্ষত আছে। সাধারণত myringoplasty/tympanoplasty-এর postoperative follow-up-এ এভাবে লেখা হয়। তবে কোন operation হয়েছিল সেটি এই sheet-এ লেখা নেই।
PTA কেন: hearing threshold এবং conductive/sensorineural component, বিশেষ করে air–bone gap assess করার জন্য। Tympanoplasty assessment-এ pure-tone audiometry গুরুত্বপূর্ণ audiological test।

Prescription dose: 0 + 0 + 1 — ছবিতে ২৫ দিন মনে হচ্ছে।
MedEx Therapeutic Class: Non-sedating antihistamines / Anti-histamine.
Pharmacological class: Second-generation selective peripheral H1-receptor antagonist.
Use/Purpose in this patient: সবচেয়ে সম্ভাব্য কারণ হলো allergic rhinitis/nasal allergy control—হাঁচি, নাক দিয়ে পানি, nasal itching/congestion কমানো এবং nasal/Eustachian-tube mucosal inflammation কমাতে সহায়তা করা। Bilastine allergic rhinoconjunctivitis ও urticaria-তে indicated.
Other diseases/conditions used: seasonal/perennial allergic rhinoconjunctivitis, hay fever symptoms, urticaria.
MedEx price: Unit ৳15, 15-tablet strip ৳225; 2×15 pack ৳450. MedEx একই generic-এর 10 mg DT এবং 12.5 mg/5 mL solution-ও তালিকাভুক্ত করেছে।
Bangladesh alternatives: Bilista, Bilan, Bilahist, Biltin, Bilxen, Bilosen, Bilgate ইত্যাদি—সবই bilastine-containing brands.
***2) Neotison Nasal Spray***
Generic name: Fluticasone Furoate 27.5 mcg/spray
Brand: Neotison — Incepta Pharmaceuticals Ltd.
Prescription আমি যেভাবে পড়ছি:
“২ চাপ, ২ বেলা, ২ নাকে — ১ মাস”
অর্থাৎ likely 2 sprays in each nostril twice daily for 1 month। Confidence 0.92।
MedEx-এ ≥12 years-এর জন্য usual dose 2 sprays/nostril once daily; selected cases-এ 2 sprays/nostril twice daily ব্যবহার করা যায়।
MedEx Therapeutic Class: Nasal Steroid Preparations.
Pharmacological class: Intranasal synthetic glucocorticoid/corticosteroid.
Use/Purpose: nasal mucosal inflammation, congestion, sneezing, rhinorrhea ও allergic rhinitis control করা। এই prescription-এ Billi + Neotison একসাথে থাকায় allergic rhinitis being treated হওয়ার সম্ভাবনা বেশ শক্তিশালী। Neotison nasal spray seasonal ও perennial allergic rhinitis-এর জন্য indicated.
Price: 120 metered sprays = ৳275.
Bangladesh alternatives: Avaspray, Avamist, Avaflut, Fixonase, Flutica, Nasogurd, Rhinomist, Snofas ইত্যাদি।
Other composition/form: Neotison 100 mcg ও 200 mcg inhalation capsules আছে, কিন্তু সেগুলো asthma maintenance-এর জন্য, nasal allergy-এর nasal-spray substitute নয়।
***3) Candistin E/D — likely***
Handwriting অনুযায়ী নামটি Candistin E/D (Ear Drop) বলেই সবচেয়ে বেশি মনে হচ্ছে। Confidence 0.88।
Generic combination:
Ofloxacin 0.3% + Clotrimazole 1% + Beclomethasone Dipropionate 0.025% + Lidocaine 2%
MedEx Therapeutic Class: 4-Quinolone preparations; Anti-fungal or anti-bacterial ear drops.
Pharmacological classes: Ofloxacin = fluoroquinolone antibacterial; Clotrimazole = imidazole antifungal; Beclomethasone = corticosteroid/anti-inflammatory; Lidocaine = amide local anaesthetic.
Purpose: bacterial + fungal ear infection cover করা, canal inflammation/itching কমানো এবং pain/burning কমানো। MedEx এটি external auditory canal এবং susceptible middle-ear bacterial/fungal infection-এর জন্য নির্দেশ করে।
Dose: handwriting-টি পুরোপুরি clear নয়। এটি সম্ভবত 2–5 drops × 3 times/day in affected ear ধরনের instruction। MedEx licensed dosage হলো 2–5 drops 3–4 times daily for 7–14 days। তাই exact handwritten duration আমি অনুমান করে definitive বলছি না।

কেন এই assessment?
Post-operative ear status — Confidence 0.90: “Graft is OK” strongly implies previous tympanic membrane graft surgery.
Allergic rhinitis — Confidence 0.82: Bilastine + intranasal fluticasone একটি খুব typical allergic-rhinitis regimen। Current ARIA guidance intranasal corticosteroids-কে allergic rhinitis-এর গুরুত্বপূর্ণ treatment হিসেবে সমর্থন করে; 2024–2025 revision-এ fluticasone furoateও supported INCS option।
Otitis externa/otomycosis — Confidence 0.60: Candistin bacterial + fungal otic infection-এর drug; কিন্তু prescription-এ ear pain, discharge, itching, canal edema বা fungal debris লিখে রাখা হয়নি। তাই medication দেখে possible বলা যায়, confirmed diagnosis নয়। Acute otitis externa guideline-এ topical therapy first-line এবং non-intact TM suspected হলে non-ototoxic preparation গুরুত্বপূর্ণ।
4. Doctor’s Dx vs আমার Dx
এখানে সবচেয়ে গুরুত্বপূর্ণ বিষয়:
Doctor কোনো Dx লিখেননি। তাই “Doctor’s Dx এবং আমার Dx same” বলা medically correct হবে না।
আমি prescription pattern থেকে working diagnosis infer করেছি। Doctor-এর documented finding শুধু:
Graft is OK.
সুতরাং prescription-এ Dx লিখতে হলে সবচেয়ে conservative wording হবে:
Dx: Post-tympanoplasty status — graft intact; Allergic rhinitis; ?Otitis externa/otomycosis.
এখানে ? ব্যবহার করাই ভালো, কারণ external-ear infection-এর examination finding এই sheet-এ নেই।
5. Differential Diagnosis
Clinical examination/history পাওয়া গেলে প্রধান differential হবে: allergic rhinitis with possible Eustachian-tube dysfunction; postoperative tympanoplasty/myringoplasty state; otitis externa/otomycosis; এবং recurrent/chronic middle-ear infection. তবে CSOM এই prescription দেখে সরাসরি diagnose করা উচিত নয়—perforation বা active otorrhoea লেখা নেই।
6. Bangladesh practice-এ এই ধরনের condition-এ ব্যবহৃত drug groups
Allergic rhinitis: intranasal corticosteroids—fluticasone furoate/propionate, mometasone, budesonide; second-generation antihistamines—bilastine, fexofenadine, cetirizine/levocetirizine; uncontrolled cases-এ intranasal antihistamine ± INCS combination বিবেচিত হয়। Persistent AR-এ INCS preferred therapy; montelukast সাধারণত alternative therapies inadequate/intolerable হলে বেশি উপযোগী।
Otitis externa/otic infection: appropriate topical antimicrobial; fungal features থাকলে clotrimazole-containing preparation; inflammation থাকলে selected antibiotic-steroid combinations। Suspected perforation/non-intact tympanic membrane থাকলে non-ototoxic topical agent বেছে নেওয়া উচিত। Uncomplicated otitis externa-তে routine systemic antibiotic first-line নয়।
Overview / Summary
Prescription-এর core meaning: এটি সম্ভবত একটি ENT postoperative follow-up prescription। Examination-এ graft ভালো আছে, hearing assessment-এর জন্য PTA দেওয়া হয়েছে। Billi + Neotison strongly support treatment of allergic rhinitis, আর Candistin একটি bacterial/fungal ear-drop combination। কিন্তু Doctor কোনো explicit Dx বা C/C লিখেননি, তাই ear infection-কে confirmed diagnosis বলা যাবে না।

2. Audiogram report—কোথায় abnormal?
Audiogram date: 27-07-2026 — PRE-OPERATIVE
Right ear
PTA = 35 dB HL
Graph-এ air conduction প্রায় 30–45 dB range, কিন্তু bone conduction প্রায় 10–15 dB-এর কাছাকাছি। অর্থাৎ বেশিরভাগ frequency-তে significant air–bone gap (~20–30 dB) আছে।
Interpretation
Right-sided conductive hearing loss (CHL).
বাংলা:
ডান কানে শব্দ inner ear পর্যন্ত যাওয়ার পথে conduction problem-এর কারণে hearing loss।
এটি sensorineural pattern নয়, কারণ bone-conduction comparatively preserved।
Confidence: 0.98
35 dB PTA-কে conventional audiological classification অনুযায়ী mild conductive hearing loss বলা যায়।
Left ear
PTA = 21.6 dB HL
Air conduction প্রায় 20–25 dB, BC প্রায় 10–15 dB।
এটি conventional ≤25 dB criterion-এ essentially normal/borderline hearing। WHO-এর stricter grading অনুসারে 20 dB-এর একটু বেশি হওয়ায় slight/mild range-এর border-এ পড়তে পারে; তবে এখানে clinically significant left-sided hearing loss audiogram থেকে established নয়।

তাই audiogram-এর main abnormality

বাংলায়:
ডান কানের দীর্ঘস্থায়ী মধ্যকর্ণের রোগের জন্য endoscopic পদ্ধতিতে কানের পর্দা পুনর্গঠন করা হয়েছে এবং tragus-এর perichondrium graft হিসেবে ব্যবহার করা হয়েছে।
এটি pre-op diagnosis-এর সঙ্গে পুরোপুরি consistent। Tympanoplasty chronic/recurrent otitis media, tympanic membrane pathology এবং conductive hearing loss-এর established treatment option। AAO-HNS postoperative follow-up-এ এক মাসের পরে TM intact কি না এবং hearing audiogram দিয়ে document করার কথা উল্লেখ করে।
4. Pre-operative investigations
ছবিতে এগুলো test order, result নয়। তাই এগুলোর কোনোটি abnormal বলার সুযোগ নেই যতক্ষণ actual result না দেখি।

বাংলায়:
ডান কানের দীর্ঘস্থায়ী active mucosal middle-ear disease, সাথে fungal ear infection এবং conductive hearing loss; যার জন্য ডান কানে endoscopic tympanoplasty করা হয়েছে।
এটাই report/document দ্বারা সবচেয়ে শক্তভাবে প্রতিষ্ঠিত diagnosis।
6. কিন্তু এখন “পানি পড়ে + চুলকায়”—এর অর্থ কী?
এখনকার symptom নতুন করে:
- Right ear watery discharge/otorrhoea
- Pruritus/itching
- Previous otomycosis history
- Recent tympanoplasty
এই combination-এ আমার current working diagnosis:
Post-tympanoplasty right ear with recurrent otorrhoea and pruritus — probable recurrent otomycosis / otitis externa; middle-ear source or graft complication must be excluded.
বাংলায়:
ডান কানের tympanoplasty-এর পর আবার পানি পড়া ও চুলকানি—পুনরায় fungal/external-ear infection হওয়ার সম্ভাবনা বেশি; তবে graft-এর সমস্যা বা middle-ear infection থেকে discharge হচ্ছে কি না পরীক্ষা করে বাদ দিতে হবে।
Confidence for recurrent otomycosis/otitis externa: 0.75–0.80.
আমি এটাকে 100% otomycosis বলব না, কারণ বর্তমান otoscopy/oto-microscopy image নেই।
যেটা এখন লিখব না
“Recurrent COM” বা “graft failure” এখনই definite লিখব না।
কারণ perforation/graft defect/middle-ear pus এখনও documented হয়নি।
Confidence <0.40 → current evidence insufficient.
7. Doctor এখন কী Action Plan নেবে?
1. Oto-microscopy / oto-endoscopy — সবচেয়ে গুরুত্বপূর্ণ
বর্তমান right ear দেখে ascertain করতে হবে:
- graft intact কি না
- graft perforation আছে কি না
- external auditory canal swollen/red কি না
- fungal debris/spores আছে কি না
- granulation tissue আছে কি না
- discharge canal থেকে, নাকি middle ear থেকে আসছে
- postoperative infection আছে কি না
Confidence: 0.98
আগের follow-up-এ graft “OK” ছিল—কিন্তু বর্তমানে নতুন discharge হয়েছে বলে graft আবার দেখতে হবে।
2. Aural toilet / microsuction
Fungal debris বা discharge থাকলে ENT-এর under-vision microsuction/aural toilet করতে হবে। শুধু drop দিয়ে fungal debris রেখে দিলে treatment failure/recurrence হতে পারে।
3. Ear strictly dry
- Water entry বন্ধ
- Swimming বন্ধ
- Cotton bud/earbud ব্যবহার নয়
- আঙুল/কাঠি দিয়ে চুলকানো নয়
- Oil/home remedy নয়
- Ear syringing/irrigation করবেন না
4. Topical therapy examination অনুযায়ী
Otomycosis confirmed হলে: cleaning + appropriate topical antifungal commonly used হয়; clotrimazole 1% সবচেয়ে বেশি studied agents-এর একটি। 2026 meta-analysis-এ topical therapies-এর overall eradication >70% পাওয়া গেছে, তবে agent-specific efficacy variable।
কিন্তু এই রোগীর recent tympanoplasty আছে। তাই current graft integrity না দেখে নিজে থেকে antifungal/combination ear drop restart করা ঠিক হবে না।
5. যদি TM/graft non-intact হয়
Guideline অনুযায়ী suspected perforation/open middle ear থাকলে non-ototoxic topical preparation বেছে নিতে হবে।
এ কারণে aminoglycoside-containing otic drops empirically ব্যবহার না করাই safer unless ENT specifically directs it।
6. Oral antibiotic
Simple otitis externa-তে routine systemic antibiotic indicated নয়; spreading infection, cellulitis, systemic illness বা deeper infection থাকলে indication অনুযায়ী দেওয়া হয়।
8. এখন আরও কী Investigation করব?
আমার priority হবে:
- Oto-microscopy / oto-endoscopy — Right ear
- Ear discharge swab for bacterial C/S যদি discharge persistent/recurrent/purulent হয়
- Fungal microscopy/KOH ± fungal culture যদি fungal debris বা recurrent otomycosis সন্দেহ হয়
- Repeat PTA – air & bone conduction ear পরিষ্কার ও infection controlled হওয়ার পর
- SRT ± speech discrimination
- Tympanometry graft sufficiently healed হলে ENT judgement অনুযায়ী
- Left-ear microscopy + tympanometry, কারণ left TM “grossly retracted”
- Diagnostic nasal endoscopy যদি recurrent cold, nasal obstruction, snoring বা voice complaint থাকে
CT temporal bone এখন routine প্রয়োজন নেই। Persistent/recurrent otorrhoea, suspected cholesteatoma, unexplained graft failure, mastoid disease বা complication থাকলে HRCT temporal bone consider করা যায়।
9. খুব গুরুত্বপূর্ণ—নতুন postoperative PTA দরকার
বর্তমানে যে audiogram দিয়েছেন সেটি:
27-07-2026
Operation হয়েছে:
29-08-2026
অর্থাৎ এটি operation-এর আগের hearing status।
তাই surgery-এর পরে hearing improve হয়েছে কি না এই report দিয়ে বলা যাবে না।
AAO-HNS follow-up guidance অনুযায়ী one month-এর পরে tympanic membrane integrity এবং hearing audiogram দিয়ে reassess করা হয়।
Tympanoplasty COM-related conductive hearing loss-এর air–bone gap improve করতে পারে; systematic review-এ long-term follow-up-এ প্রায় 70% patients-এর postoperative ABG <20 dB পাওয়া গেছে।
10. কোন Doctor-এর কাছে Refer করবেন?

Kindly assess graft integrity by oto-microscopy and exclude recurrent otomycosis/otitis externa, middle-ear infection or graft complication; aural toilet ± culture and repeat PTA after the ear becomes dry/healed.

“ডান কানের chronic active mucosal otitis media ও conductive hearing loss-এর জন্য tympanoplasty-পরবর্তী অবস্থা; বর্তমানে আবার কান দিয়ে পানি পড়া ও চুলকানি—পুনরায় otomycosis/otitis externa সন্দেহ।”
আমি এই wording-টাই ব্যবহার করব। কারণ এতে আগের confirmed disease এবং বর্তমান suspected disease আলাদা করে পরিষ্কার থাকে।
Overview / Summary
আগের confirmed disease: Right COM—Active Mucosal disease + Otomycosis + Right CHL (PTA 35 dB). এর জন্য right endoscopic tympanoplasty হয়েছে। Audiogram preoperative এবং right conductive hearing loss clearly দেখাচ্ছে। Left TM grossly retracted; septal spur to left-ও documented abnormality।
বর্তমানে watery discharge + itching হওয়ায় recurrent otomycosis/otitis externa সবচেয়ে সম্ভাব্য, কিন্তু recent tympanoplasty থাকায় graft integrity এবং middle-ear source অবশ্যই exclude করতে হবে।

Safety point: Flamex = ibuprofen এবং Voltalin = diclofenac—দুটিই systemic NSAID। তাই নিজে থেকে একই সময়ে/একসাথে routine use করা উচিত নয়, কারণ GI bleeding, ulcer ও renal adverse-effect risk বাড়ে। Doctor যেভাবে rescue use লিখেছেন, সেই নির্দেশই follow করা উচিত। MedEx-ও ibuprofen-এর সাথে other NSAID co-administration avoid করতে বলে।

Handwritten advice আমি পড়ছি:
1. “Bandage ভেজাবেন না।”
2. “7 দিন পর dressing দেখাবেন/করাবেন।”

11. Bangladesh-এ এই condition-এ কোন ধরনের drug commonly ব্যবহার হয়?
সবগুলো একসাথে নয়; clinical finding অনুযায়ী নির্বাচন হয়।
- Topical quinolone antibiotic: ofloxacin/ciprofloxacin-type otic preparation—bacterial otorrhoea থাকলে এবং TM status অনুযায়ী।
- Topical antifungal: confirmed/suspected otomycosis-এ clotrimazole commonly ব্যবহৃত; তবে ear cleaning/aural toilet গুরুত্বপূর্ণ। Recent meta-analysis-এ topical treatment-এর overall eradication >70%, এবং clotrimazole commonly studied therapy।
- Antibiotic + antifungal + steroid combination: Candistin/Audicare/Otoxin-type combination selected mixed external-ear infection-এ ব্যবহৃত হয়।
- Systemic antibiotic: active bacterial infection, peri-operative context বা surgeon-assessed indication থাকলে cefuroxime-clavulanate/co-amoxiclav type drugs ব্যবহার হতে পারে।
- Antihistamine + intranasal steroid: associated allergic rhinitis/nasal inflammation থাকলে rupatadine/bilastine + fluticasone-type therapy।
- Short-course nasal decongestant: xylometazoline—significant nasal congestion থাকলে, সাধারণত ≤7 days।
- Analgesia: ibuprofen/diclofenac/paracetamol—postoperative pain অনুযায়ী।
Otomycosis-এর ক্ষেত্রে শুধুমাত্র medicine নয়; debris removal/aural toilet এবং ear dry রাখা clinical management-এর গুরুত্বপূর্ণ অংশ।
