বাংলায় Working Dx লিখতে পারেন:
রোগ নির্ণয় (Dx):
সম্ভাব্য বহিঃকর্ণের সংক্রমণ/প্রদাহ (Otitis Externa), সম্ভবত ফাঙ্গাল সংক্রমণ বা Otomycosis সহ, এবং Allergic Rhinitis।
আরও সংক্ষেপে:
Dx: বহিঃকর্ণের ফাঙ্গাল/মিশ্র সংক্রমণ (Otomycosis/Otitis Externa) + অ্যালার্জিক রাইনাইটিস।
আগের hearing test অনুযায়ী দুই কানের শ্রবণক্ষমতা স্বাভাবিক এবং মধ্যকর্ণের কার্যকারিতা স্বাভাবিক।
রিপোর্ট দুইটি মিলিয়ে বড় কোনো hearing problem ধরা পড়ছে না। Audiometry ও tympanometry—দুই পরীক্ষাই মোটামুটি reassuring।
1) Pure Tone Audiometry
গ্রাফে দুই কানের hearing threshold প্রায় 15–25 dB HL এর মধ্যে আছে এবং রিপোর্টেও লেখা আছে:
- Right ear: Hearing within normal limit
- Left ear: Hearing within normal limit
Adult clinical audiology-তে প্রায় −10 থেকে 20–25 dB HL পর্যন্ত অনেক ক্ষেত্রে normal range হিসেবে ধরা হয়।
তবে বর্তমান WHO grading-এ better-ear PTA <20 dB HL কে normal এবং 20 থেকে <35 dB HL-কে mild hearing loss category বলা হয়; এটি মূলত population/epidemiological classification। আপনার ছবির graph থেকে exact PTA বের করা যাচ্ছে না, তবে clinically significant hearing loss দেখা যাচ্ছে না।
Interpretation:
Right ear কিছু frequency-তে প্রায় 20–25 dB-এর কাছাকাছি, অর্থাৎ borderline-normal/slight range হতে পারে; left ear একটু ভালো। কিন্তু significant conductive বা sensorineural hearing loss-এর pattern নেই।
2) Tympanometry
দুই কানেই:
Type A tympanogram
- Peak pressure প্রায় −17 daPa এবং −14 daPa
- Peak compliance প্রায় 1.38 এবং 1.19
- Acoustic reflex/SRT-ART: Present
Type A tympanogram সাধারণত বোঝায় middle-ear pressure ও tympanic membrane/ossicular mobility স্বাভাবিক, এবং middle-ear effusion বা significant Eustachian tube dysfunction-এর evidence নেই।
Final Dx / Audiological Impression
আপনি clinical note-এ লিখতে পারেন:
Bilateral hearing sensitivity within normal/near-normal limits with bilateral Type-A tympanograms and present acoustic reflexes. No significant conductive or sensorineural hearing loss detected.
Confidence: 0.90
এখানে কোনো clear otitis media with effusion, middle-ear dysfunction, significant conductive hearing loss বা significant SNHL দেখা যাচ্ছে না।
Actionable Plan
যদি রোগীর কোনো hearing complaint না থাকে, তাহলে specific treatment বা hearing aid প্রয়োজন হওয়ার evidence এই রিপোর্টে নেই।
তবে যদি রোগীর কানে শোঁ শোঁ শব্দ, কান বন্ধ লাগা, কথা বুঝতে সমস্যা, এক কানে কম শোনা, dizziness/vertigo, ear pain বা discharge থাকে, তখন normal audiogram থাকলেও ENT assessment দরকার হতে পারে। তখন otoscopy + speech audiometry (SRT/WRS) এবং প্রয়োজন অনুযায়ী OAE/ABR বিবেচনা করা যায়, কারণ pure-tone audiogram একা সব auditory disorder exclude করতে পারে না।
Overview / Summary
মূল কথা: দুই কানের middle-ear function স্বাভাবিক এবং hearing clinically normal/near-normal। Right ear-এর কিছু threshold borderline 20–25 dB-এর দিকে, কিন্তু report অনুযায়ী significant hearing loss নেই।

আমি prescription-টি hearing test + tympanometry-এর সাথে মিলিয়ে দেখেছি। সবচেয়ে গুরুত্বপূর্ণ বিষয় হলো—prescription-এর visible অংশে C/C বা Dx সরাসরি লেখা নেই। তাই নিচের Dx হলো working clinical assessment, medicine pattern + আগের audiology report থেকে inference; নিশ্চিত diagnosis-এর জন্য otoscopy finding দরকার।
C/C — Chief Complaint কী হতে পারে?
Prescription থেকে সম্ভাব্য C/C:
- কানে ব্যথা/অস্বস্তি ± itching/discharge — moderate confidence 0.75
- নাকের allergy/congestion/sneezing — confidence 0.85
- সম্ভবত external-ear infection/inflammation; fungal component সন্দেহ করা হয়েছিল—বিশেষত handwritten ২ নম্বরটি যদি সত্যিই Vori 50 হয়।
তবে এগুলো prescription-এ লিখিত C/C নয়। রোগীর actual symptoms না জানলে এর চেয়ে definitive হওয়া ঠিক হবে না।
Final Working Dx
সব রিপোর্ট + prescription মিলিয়ে আমার working diagnosis:
1. Probable acute otitis externa / inflammatory external-ear infection, possibly with fungal component (otomycosis ± secondary bacterial infection). — Confidence 0.70
Aprodex ear drop + cefuroxime এবং সম্ভবত Vori দেখে এই inference। Non-invasive otomycosis হলে সাধারণত ear-canal cleaning/debridement + topical antifungal ব্যবহৃত হয়; systemic voriconazole সাধারণ uncomplicated otomycosis-এর routine treatment নয়।
2. Associated allergic rhinitis — Confidence 0.90
Neotison nasal spray + সম্ভবত Alcet 5 mg এই diagnosis-এর সাথে খুব ভালোভাবে মেলে। বর্তমান ARIA-EAACI guideline-ও allergic rhinitis-এ intranasal corticosteroid strongly supports করে।
3. Bilateral hearing within normal/near-normal limits with normal middle-ear function — Confidence 0.95
আগের PTA-তে significant hearing loss নেই এবং bilateral Type-A tympanogram। তাই OME/significant Eustachian-tube dysfunction বা conductive hearing loss-এর evidence নেই। অর্থাৎ এই মুহূর্তে Dx হিসেবে “hearing loss” লিখব না।
4. Possible Vitamin-D deficiency/insufficiency under treatment — Confidence 0.45
X-D3 prescription দেখে সন্দেহ করা যায়, কিন্তু 25-OH Vitamin D report ছাড়া confirmed Vitamin-D deficiency বলা যাবে না।
Prescription-এর জন্য concise Dx লিখতে হলে
Dx: Probable Otitis Externa / Otomycosis with Allergic Rhinitis; Hearing within normal limits bilaterally.
যদি otoscopy-তে fungal debris/spores/hyphae দেখা গিয়ে থাকে, তখন আরও specific করে:
Dx: Otomycosis (Otitis Externa) with Allergic Rhinitis; normal bilateral hearing.
Doctor-এর দেওয়া medicines
হাতের লেখার কারণে ১, ৪, ৫ নম্বর বেশ পরিষ্কার; ২ ও ৩ নম্বরে কিছু uncertainty আছে। আমি যেটুকু safely identify করা যায় সেটাই দিচ্ছি।
1. Tab. Fixcef 500 mg — identification confidence 0.95
Generic name: Cefuroxime Axetil 500 mg
Bangladesh Brand: Fixcef 500 — Navana Pharmaceuticals Ltd.
MedEx Therapeutic Class: Second generation cephalosporins
Pharmacological class: β-lactam, 2nd-generation cephalosporin antibiotic.
কেন দেওয়া হয়েছে: ENT bacterial infection cover করার জন্য। Cefuroxime susceptible bacteria-র cell-wall synthesis বন্ধ করে bactericidal effect দেয়। MedEx-এ acute bacterial otitis media, sinusitis, pharyngitis/tonsillitisসহ বিভিন্ন bacterial infection indication হিসেবে আছে।
Price: MedEx listing-এ Fixcef 500 প্রায় ৳45/tablet দেখা যাচ্ছে; retail price update হতে পারে।
Other strengths/compositions: 250 mg tablet; 125 mg/5 mL suspension. Fixcef Plus-এ cefuroxime + clavulanic acid formulation-ও আছে—সেটি একই medicine নয়।
Clinical note: যদি diagnosis শুধু uncomplicated otitis externa হয়, oral/systemic antibiotic routine first-line নয়; topical therapy preferred। Oral antibiotic justified হতে পারে যদি infection canal-এর বাইরে ছড়ায়, cellulitis/high-risk host থাকে অথবা concomitant bacterial middle-ear/sinus infection থাকে।
2. সম্ভবত Tab. Vori 50 mg — identification confidence 0.70
হাতের লেখাটি “Vori 50”-এর মতো দেখাচ্ছে।
Generic name: Voriconazole 50 mg
Bangladesh Brand: Vori 50 — Square Pharmaceuticals PLC
MedEx Therapeutic Class: Other Antifungal preparations
Pharmacological class: Triazole antifungal.
কাজ: fungal cell membrane-এর ergosterol synthesis inhibit করে fungus-এর growth/viability নষ্ট করে।
Price: ৳35/tablet; 10-tablet strip = ৳350.
Other strengths: Vori 200 mg tablet; 200 mg/vial IV.
এই রোগীতে কেন দেওয়া হতে পারে: যদি doctor otoscopy-তে fungal otitis externa/otomycosis সন্দেহ করে থাকেন।
কিন্তু এখানে একটি গুরুত্বপূর্ণ clinical concern আছে: uncomplicated non-invasive otomycosis-এর standard approach হলো ear cleaning/debridement + topical antifungal। IDSA systemic voriconazole মূলত invasive Aspergillus ear disease-এর মতো গুরুতর অবস্থায় ব্যবহার করার কথা বলে। তাই এটি সত্যিই Vori 50 কিনা এবং doctor কী finding দেখে দিয়েছেন—এটা confirm করা গুরুত্বপূর্ণ।
3. সম্ভবত Anadol 100 mg — identification confidence 0.65
হাতের লেখা “Anadol 100”-এর মতো, কিন্তু formulation পুরোপুরি clear নয়।
Generic: Tramadol Hydrochloride
MedEx Therapeutic Class: Opioid analgesics
Pharmacological class: Centrally acting opioid analgesic.
কেন দেওয়া হয়ে থাকতে পারে: severe ear pain/otalgia কমাতে। Tramadol μ-opioid effect এবং serotonin/noradrenaline reuptake inhibition-এর মাধ্যমে analgesia দেয়।
MedEx-এ:
- Anadol 50 mg capsule = ৳8.07/capsule, strip ৳80.70
- Anadol SR 100 mg = প্রায় ৳17/unit
- Anadol 100 mg regular formulation MedEx-এ suppository হিসেবেও রয়েছে = প্রায় ৳20/unit।
তাই prescription-এর formulation confirm না করে আমি exact price/dose বলব না।
Clinical point: সাধারণ otalgia-তে আগে paracetamol/NSAID যথেষ্ট কি না দেখা হয়; opioid severe selected pain-এ short-term consideration। Alcohol/sedative-এর সাথে tramadol ঝুঁকিপূর্ণ।
4. সম্ভবত Tab. Dextor 0.5 mg — confidence 0.85
Generic name: Dexamethasone 0.5 mg
Brand: Dextor — Eskayef Pharmaceuticals Ltd.
MedEx class: Glucocorticoids / Corticosteroid hormones
Pharmacological class: Long-acting synthetic glucocorticoid.
কেন দেওয়া হয়েছে: ear/nasal inflammatory-allergic response দ্রুত কমাতে। Dexamethasone leukocyte migration ও inflammatory response suppress করে।
Price: ৳1/tablet; 10-tablet strip ≈ ৳10. MedEx বর্তমানে Dextor-কে unavailable হিসেবেও দেখাচ্ছে।
Alternatives: Decason, Dexonex, Dextason, Steron, Odeson ইত্যাদি dexamethasone brands।
Clinical review: uncomplicated allergic rhinitis-এ routine systemic dexamethasone first-line নয়; intranasal corticosteroid preferred because systemic adverse-effect burden কম।
5. Tab. Alcet 5 mg — confidence 0.90
Generic name: Levocetirizine Dihydrochloride 5 mg
Brand: Alcet — Healthcare Pharmaceuticals Ltd.
MedEx Therapeutic Class: Non-sedating antihistamines / Anti-histamine
Pharmacological class: Second-generation H1-receptor antagonist.
কেন দেওয়া হয়েছে: allergic rhinitis-এর sneezing, runny nose, nasal/ear itching, watery symptoms কমাতে। Histamine-H1 receptor blockade করে।
Price: ৳4.50/tablet; 10 tablets = ৳45.
Other composition/form: 2.5 mg/5 mL oral solution available.
6. Aprodex Ear Drop
Generic: Ciprofloxacin 0.3% + Dexamethasone 0.1%
Company: Aristopharma Ltd.
MedEx Therapeutic Class: Aural steroid & antibiotic combined preparations
Pharmacological class: Fluoroquinolone antibiotic + glucocorticoid.
কেন দেওয়া হয়েছে: bacterial ear infection + associated canal inflammation, swelling, itching ও pain কমাতে।
- Ciprofloxacin → bacterial DNA gyrase inhibit করে।
- Dexamethasone → inflammation ও edema কমায়।
MedEx specifically otitis externa এবং inflammatory ear infections-এ use উল্লেখ করে।
Price: 5 mL ≈ ৳75.
Bangladesh alternatives: Bactin D, Beuflox-D, Cerodex, Ciflodex, Cip-D—same 0.3% + 0.1% composition।
7. Neotison Nasal Spray
Generic: Fluticasone Furoate 27.5 mcg/spray

Pharmacological class: Intranasal corticosteroid.
কেন দেওয়া হয়েছে: allergic rhinitis-এর nasal mucosal inflammation, congestion, sneezing, runny nose কমাতে।
Price: 120 metered sprays ≈ ৳275.
Same-composition Bangladesh brands: Avamist, Avaspray, Fixonase, F-Son, Flutica, Nasogurd, Rhinomist, Snofas ইত্যাদি।
8. X-D3 300,000 IU
Prescription-এ printed X-D3 300000 IU আছে।
Likely generic: Cholecalciferol / Vitamin D3
MedEx Therapeutic Class: Vitamin in bone formation; Vitamin-D preparations
Pharmacological class: Fat-soluble vitamin / calcium-phosphate homeostasis regulator.
কাজ: intestinal calcium ও phosphate absorption বাড়িয়ে bone mineralization এবং vitamin-D deficiency correction-এ সাহায্য করে।
Important: Exact X-D3 300,000 IU brandটি Bangladesh MedEx-এ আমি verify করতে পারিনি, তাই এর Bangladesh price বানিয়ে বলা ঠিক হবে না। MedEx-এ একই generic-এর 200,000 IU/mL injectable/oral ampoule যেমন D3, D-Vine, Defrol, Delight ইত্যাদি প্রায় ৳90–120/ampoule range-এ আছে, কিন্তু এগুলো 300,000 IU-এর direct equivalent নয়।
এই high-dose Vitamin-D regimen repeat করার আগে 25-OH Vitamin D + serum calcium জানা ভালো, বিশেষত prolonged high-dose treatment হলে।
Prescription-এর যে অংশগুলো আমি clinicalভাবে re-check করতাম
প্রথমত, Vori 50 সত্যিই voriconazole কিনা confirm করতাম। যদি uncomplicated otomycosis হয়, debridement + topical antifungal standard; systemic voriconazole অস্বাভাবিকভাবে aggressive হতে পারে।
দ্বিতীয়ত, যদি শুধুই uncomplicated otitis externa হয়, Fixcef systemic antibiotic প্রয়োজনীয় কি না otoscopy ও infection spread দেখে বিচার করা উচিত। AAO-HNS guideline topical preparation-কে initial treatment হিসেবে recommend করে।
তৃতীয়ত, Dextor systemic dexamethasone + Neotison intranasal steroid + Aprodex topical steroid—তিন জায়গা থেকে steroid exposure হচ্ছে। Short course হলে clinician-এর rationale থাকতে পারে, কিন্তু simple allergic rhinitis-এর routine management হিসেবে oral dexamethasone সাধারণত প্রয়োজন হয় না।
Bangladesh-available guideline-consistent options
Otitis externa-তে সাধারণত aural cleaning + topical fluoroquinolone ± steroid, সঙ্গে pain অনুযায়ী paracetamol/ibuprofen ব্যবহৃত হয়। Systemic antibiotic only selected cases। Suspected uncomplicated otomycosis-এ ear toileting/debridement + topical azole such as clotrimazole বেশি standard। Allergic rhinitis-এ intranasal corticosteroid ± second-generation antihistamine, saline irrigation; uncontrolled disease-এ intranasal antihistamine + steroid combination বিবেচ্য।
Overview / Summary
Most likely clinical picture:
External ear infection/inflammation—possibly otomycosis/mixed otitis externa + allergic rhinitis, কিন্তু hearing normal এবং middle-ear function normal। Active middle-ear effusion/hearing loss-এর evidence নেই।
Overall diagnostic confidence: 0.75. প্রধান limitation হলো prescription-এ actual C/C, otoscopy findings এবং formal Dx লেখা নেই।
Actionable Plan
Otoscopy/oto-microscopy finding confirm করুন—canal edema, fungal debris, discharge, TM intact কিনা। যদি fungal material থাকে, ear swab/KOH/culture selected recurrent/refractory case-এ useful। Vori 50 ও Anadol 100-এর medicine strip/packet-এর ছবি দিলে আমি exact drug, dose, price এবং MedEx mapping 100% নির্ভুলভাবে করে দিতে পারব। Vitamin D-এর জন্য 25-OH D ও calcium দেখলে X-D3 indication-ও confirm হবে।
