এখানে breast cancer treatment-এর জন্য নয়। Fertility practice-এ Letrozole ovulation induction-এর জন্য off-label ব্যবহার করা হয়।
Letrozole সাময়িকভাবে estrogen synthesis কমায় → hypothalamus/pituitary-এর negative feedback কমে → endogenous FSH বাড়ে → ovarian follicle grow করতে সাহায্য করে → ovulation হওয়ার সম্ভাবনা বাড়ে।

5.TVS Folliculometry কেন Day 12?
এটি treatment-এর সবচেয়ে গুরুত্বপূর্ণ monitoring componentগুলোর একটি।
TVS-এ দেখা হবে:

- follicle-এর সংখ্যা ও diameter;
- endometrial thickness/pattern;

Differential Diagnosis / underlying cause
Prescription দেখে underlying cause নিশ্চিত করা যায় না। সম্ভাবনাগুলো হলো:
- Anovulatory/oligo-ovulatory infertility, including PCOS — সবচেয়ে সম্ভাব্য।
- Unexplained infertility with ovarian stimulation.
- Diminished/altered ovarian response requiring stimulation.
- Tubal-factor infertility — আলাদা testing ছাড়া বাদ যায় না।
- Male-factor infertility — semen analysis ছাড়া বাদ যায় না।
- Endometriosis-associated infertility.
- Thyroid, hyperprolactinaemia বা অন্যান্য endocrine ovulatory disorder।
শুধু এই prescription থেকে “PCOS confirmed” লেখা clinically justified হবে না।
Bangladesh-এ এই ধরনের ovulatory infertility-তে কোন drug groups ব্যবহার হয়?
এইগুলো alternative/adjunct; সবগুলো একসঙ্গে দেওয়া হয় না এবং diagnosis/protocol অনুযায়ী নির্বাচন করা হয়।
- Letrozole — PCOS/anovulatory infertility-তে guideline-supported first-line option। Bangladesh brands উপরে দেওয়া হয়েছে।
- Clomiphene citrate — alternative oral ovulation-induction drug। MedEx brands: Fermid, Fertil, Ovajet, Ovuclon, Ovulet, Reomen; 50-mg preparations প্রায় ৳10/tablet।

Investigations / assessment that should accompany treatment
একটি complete infertility assessment-এ clinical context অনুযায়ী semen analysis, pelvic/TVS assessment, ovulation assessment, tubal patency evaluation (HSG/HyCoSy where appropriate), pregnancy exclusion এবং প্রয়োজন অনুযায়ী TSH/prolactin/ovarian-reserve assessment বিবেচনা করা হয়। WHO-এর 2025 infertility guideline diagnosis ও treatment-কে cause-based এবং progressively monitored approach হিসেবে সাজিয়েছে।
FSH চলাকালে severe lower-abdominal pain/distension, rapid weight gain, persistent vomiting, breathlessness বা markedly reduced urine হলে OHSS-এর জন্য জরুরি review দরকার।
