Brand Model and Executive

TC(1073).(05).Orthopedics Case No 149:- Hill-Sachs Lesion

Hill-Sachs Lesion — বাংলা Clinical Notes + Bangladesh Drug List

1. Definition /

2. Etiology / Cause —

A. Direct mechanism

  • Anterior shoulder dislocation সবচেয়ে common cause।

B. Common clinical causes

  • Fall on outstretched hand /
  • Contact sports injury — football, rugby, wrestling
  • Overhead sports — cricket throwing, volleyball, badminton, baseball-type throwing
  • Road traffic accident / direct trauma
  • Young male athletes বেশি risk group; first dislocation-এর পর recurrence risk বেশি, বিশেষ করে young male/contact sport group-এ।

C. Important note

  1. Shoulder anteriorly dislocate করে।
  2. Humeral head glenoid socket-
  3. Anterior glenoid rim humeral head-এর posterolateral cancellous bone-এ চাপ দেয়।
  4. Compression fracture / depression defect

4. Symptoms / Clinical Features —

Acute stage

  • তীব্র shoulder pain — dislocation moment-এ sudden severe pain
  • Shoulder deformity — shoulder flattened/abnormally prominent
  • Arm move
  • Swelling ও bruising
  • Muscle spasm
  • Weakness

Chronic / recurrent instability stage

  • Clicking / catching sensation
  • Overhead activity বা throwing-এ pain
  • Loss of confidence in shoulder
  • Recurrent subluxation/dislocation

5. Clinical Diagnosis — কীভাবে clinically diagnose করবেন

A. History

  • Recent anterior shoulder dislocation?
  • Previous dislocation
  • Mechanism: fall, sports trauma, RTA, seizure?
  • Numbness/weakness
  • Dominant hand involvement?
  • Sports/occupation: overhead worker, athlete?

B. Inspection

  • Loss of normal deltoid contour
  • Anterior fullness
  • Arm held in slight abduction/external rotation
  • Bruising/swelling

C. Neurovascular examination —

  • Axillary nerve: lateral deltoid “regimental badge area” sensation, deltoid contraction
  • Radial, median, ulnar nerve function
  • Radial pulse, capillary refill

D. Specific clinical tests

  • Apprehension test: abduction + external rotation করলে dislocation fear/pain
  • Relocation test: posterior pressure দিলে apprehension কমে
  • Surprise/release test: posterior pressure suddenly release করলে apprehension আসে
  • Load-and-shift test: anterior instability assess
  • Sulcus sign: generalized laxity/multidirectional instability
  • Rotator cuff testing: especially age >40 years, because traumatic dislocation-এর সাথে cuff tear থাকতে পারে। BESS/BOA pathway older patients-এ cuff integrity assess করতে early imaging consider করতে বলে।

6. Investigations — specific

1. X-ray shoulder — first-line

Views:

  • AP view
  • Scapular Y view
  • Axillary view — dislocation direction confirm
  • Internal rotation AP / Stryker notch view — Hill-Sachs defect দেখতে helpful

  • Dislocation confirm
  • Fracture exclude

2. CT scan / 3D CT — bony defect assessment-এর জন্য best

Indications:

  • Recurrent dislocation
  • Large Hill-Sachs suspected
  • Glenoid bone loss quantification
  • Preoperative planning
  • On-track/off-track calculation

3. MRI shoulder / MR arthrogram

Indications:

  • Bankart lesion / labral tear
  • HAGL lesion
  • Rotator cuff tear
  • Cartilage injury
  • Capsuloligamentous injury

4. Ultrasound shoulder

  • Age >40 years বা cuff weakness থাকলে rotator cuff tear screen করতে useful
  • Dynamic, cheap, accessible

5. EMG/NCS

  • Axillary nerve palsy suspected এবং 6 weeks পরও clinical recovery না থাকলে EMG করা যায়; BESS/BOA pathway no recovery হলে nerve injury centre referral suggest করে।

7. Differential Diagnosis

  • Anterior shoulder dislocation without Hill-Sachs
  • Bankart lesion / bony Bankart lesion
  • Rotator cuff tear
  • Greater tuberosity fracture
  • Proximal humerus fracture
  • AC joint injury
  • Labral SLAP tear
  • Posterior dislocation with reverse Hill-Sachs
  • Cervical radiculopathy / brachial plexus injury

8. Complications — বাংলায় ব্যাখ্যা

1. Recurrent shoulder instability

2. Bankart lesion association

3. Glenoid bone loss

4. Axillary nerve injury

5. Rotator cuff tear

6. Chronic pain and stiffness

Prolonged immobilization, inadequate rehab, capsular stiffness থেকে painful stiff shoulder/frozen shoulder হতে পারে।

7. Osteoarthritis / dislocation arthropathy

8. Surgical complications

BESS/BOA reported Latarjet complication rate relatively high, তাই routine primary procedure নয়।

9. Treatment / Management — Guideline-based

A. Acute dislocation stage

  1. Neurovascular exam before reduction
  2. X-ray before reduction — fracture/dislocation confirm
  3. Closed reduction — trained clinician, analgesia/sedation/local anesthesia সহ
  4. Post-reduction X-ray
  5. Sling immobilization — pain control পর্যন্ত short period; prolonged immobilization recurrence কমাতে clearly beneficial নয়। BESS/BOA summary বলছে >1 week immobilization recurrence কমাতে beneficial নয়।
  6. Early supervised physiotherapy

B. Non-operative management —

Suitable if:

  • First-time dislocation
  • Small, non-engaging/on-track Hill-Sachs lesion
  • No significant glenoid bone loss
  • Low-demand patient
  • No recurrent instability

Plan:

  • Sling briefly
  • Pain control
  • Physiotherapy: rotator cuff, deltoid, scapular stabilizer strengthening
  • Proprioception training
  • Avoid abduction + external rotation early phase
  • Follow-up for instability

C. Surgical management —

1. Arthroscopic Bankart repair

Indication:

  • Bankart lesion with recurrent anterior instability
  • No major bony defect
  • Young athlete/high-demand patient

2. Bankart repair + Remplissage

Indication:

  • Off-track/engaging Hill-Sachs lesion
  • Little or subcritical glenoid bone loss
  • Recurrent instability

3. Latarjet procedure

Indication:

  • Significant glenoid bone loss
  • Failed soft tissue stabilization
  • Recurrent anterior instability with bone loss
  • Some engaging Hill-Sachs/bipolar bone loss cases

BESS/BOA pathway অনুযায়ী recurrent instability with around 20% glenoid bone loss হলে open Latarjet consider করা হয়; engaging Hill-Sachs lesion-এ Latarjet বা remplissage + Bankart acceptable options হলেও high-quality RCT evidence limited।

4. Bone grafting / humeral head reconstruction

Indication:

  • Very large humeral head defect
  • Persistent off-track lesion despite glenoid procedure
  • Complex bipolar bone loss

5. Arthroplasty

Rare; elderly patient with massive humeral head defect, arthritis, irreparable damage.

  • Bankart lesion/glenoid bone loss থাকলে appropriate stabilization surgery

D. General prevention

  • Fall prevention in elderly
  • Seizure control
  • Avoid high-risk shoulder positions until rehab complete
  • Muscle strengthening and posture correction

Overview / Summary

Actionable Plan / Steps

  1. Acute case: neurovascular exam → X-ray AP/Y/axillary → reduction → post-reduction X-ray.
  2. Pain control: Paracetamol ± short NSAID; high-risk patient হলে PPI add.
  3. Severe pain: short-course tramadol only if needed.
  4. Spasm: tolperisone short course if clinically significant spasm.
  5. Recurrent instability: MRI/MR arthrogram + 3D CT for Bankart, glenoid bone loss, Hill-Sachs size, on-track/off-track.
  6. Small/on-track: rehab-based conservative care.
  7. Off-track/engaging or recurrent: orthopedic shoulder specialist referral for Bankart repair ± remplissage / Latarjet decision.
  8. Prevention: structured physiotherapy, avoid risky positions, return-to-sport criteria follow.
