Hello I'm Raiyyan Tahsan
Brand Model and Executive

TC(1064).(05).Orthopedics Case No 142:- Wrist Drop

Wrist Drop — Bangla clinical notes

1) Definition /

A. Compression neuropathy — সবচেয়ে common

  • Tight plaster/cast, tight bandage, tourniquet, handcuff: external compression থেকে nerve palsy।

B. Trauma / injury

  • Humeral shaft fracture—বিশেষ করে spiral groove/shaft fracture। Closed humeral shaft fracture-এ radial nerve palsy common; systematic review-এ primary radial nerve palsy-এর অধিকাংশ spontaneous recover করে।
  • Elbow dislocation, forearm injury, penetrating injury, stab/gunshot wound
  • Iatrogenic injury: surgery, injection, ORIF plating, prolonged operation positioning।

C. Entrapment neuropathy

  • Posterior interosseous nerve palsy—supinator muscle/Arcade of Frohse-এ entrapment; sensory loss থাকে না, mainly finger/thumb extension weakness।
  • Radial tunnel syndrome—pain বেশি, motor palsy কম।
  • Ganglion, lipoma, tumor, bony spur nerve compress

D. Systemic / medical cause

  • Diabetes mellitus—mononeuropathy/entrapment tendency।
  • Vasculitis / mononeuritis multiplex—painful, asymmetric nerve palsy।
  • Lead poisoning—classic wrist drop cause।
  • Alcoholism, vitamin B12 deficiency, hypothyroidism, renal failure

E. Mimic / differential cause

  • C7 radiculopathy—neck pain + triceps/wrist/finger weakness।
  • Brachial plexopathy
  • Stroke/UMN lesion—sudden onset হলে central cause exclude করতে হবে।

3) Pathophysiology /

  • Radial nerve posterior cord of brachial plexus থেকে আসে; root value C5–T1।
  • এটি wrist extensor, finger extensor, thumb extensor muscle supply করে।
  • Injury types:
    • Neurapraxia: compression; nerve intact; recovery
    • Axonotmesis: axon damaged; recovery slow, EMG/NCS monitoring
    • Neurotmesis: nerve cut/rupture; surgical repair

4) Symptoms / লক্ষণ — Bangla point আকারে

Motor symptoms

Sensory symptoms

  • হাতের পেছনের radial side-এ numbness/tingling।
  • First dorsal web space—thumb

Pain symptoms

  • Radial tunnel syndrome-এ lateral forearm/elbow pain prominent।

Functional problem

5) Clinical diagnosis — specifically

A. History

  • Onset: sudden নাকি gradual?
  • Trauma: fall, humerus fracture, elbow injury
  • Compression history: ঘুমের সময় arm চাপা, alcohol/sedative, crutch use, tight cast/bandage।
  • Pain: severe progressive pain হলে nerve compression/vasculitis/compartment concern।
  • Systemic: diabetes, weight loss, fever, rash, joint pain, neuropathy elsewhere, lead exposure, leprosy contact।
  • Neck pain/radiation থাকলে cervical radiculopathy ভাববেন।

B. Motor examination

  • Wrist extension: patient-
  • Thumb extension: thumb up sign।
  • Brachioradialis power: elbow flexion with forearm mid-prone।
  • Triceps power: elbow extension—high radial lesion হলে weak।

C. Sensory examination

  • First dorsal web space—superficial radial nerve।
  • Dorsum of thumb/index/middle radial side।
  • Posterior forearm/arm—lesion level

D. Reflex

  • Brachioradialis reflex—C6/radial nerve।
  • Triceps reflex—C7/radial nerve।

E. Lesion localization clue

  • Axilla lesion: triceps + wrist/finger/thumb extension weak; sensory loss wider।
  • Spiral groove lesion: triceps usually spared; wrist/finger extension weak; dorsal hand numb।
  • Posterior interosseous nerve lesion: finger/thumb extension weak, wrist extension may remain with radial deviation; no sensory loss।
  • Superficial radial neuropathy: sensory symptoms only; true wrist drop
  • C7 radiculopathy: neck pain, triceps reflex affected, pronator/flexor involvement

6) Investigations — specific list

জরুরি/প্রথম-line

  • X-ray humerus/elbow/forearm/wrist: trauma, fracture, dislocation, callus, bony lesion
  • Nerve conduction study + EMG: radial neuropathy confirm, lesion site/severity/localization, axonal loss আছে কি না, recovery monitoring। EMG/NCS radial neuropathy বনাম cervical radiculopathy/brachial plexopathy differentiate করতে খুব useful।
  • Practical timing:
    • Baseline NCS early

Imaging for nerve

  • High-resolution ultrasound: nerve continuity, entrapment, neuroma, swelling, mass।
  • MRI/MR neurography: tumor, deep mass, muscle denervation, nerve discontinuity বা complex entrapment। Imaging exact site, extent, local cause ও surgical planning-এ সাহায্য করে।

Laboratory tests — cause

  • FBS/RBS/HbA1c—diabetes।
  • CBC, ESR, CRP—infection/vasculitis/inflammation।
  • Vitamin B12, folate—nutritional neuropathy।
  • TSH—hypothyroid neuropathy।

  • Brachial plexopathy
  • Stroke / cortical hand palsy
  • Motor neuron disease
  • Tendon rupture—extensor tendon injury
  • Myopathy rarely

8) Complications — Bangla explanation

  • Chronic neuropathic pain: burning, electric shock-like pain, allodynia।
  • Psychosocial disability: writing, work, driving, manual work, daily living affected।

9) Treatment / Management

A. Core management —

  • Compression remove: tight cast/bandage/crutch pressure
  • Wrist cock-up splint: wrist 15–30° extension; finger/thumb support if needed।
  • Physiotherapy/Occupational therapy:
    • passive ROM: wrist, MCP, thumb

    • vasculitis/leprosy/lead poisoning → disease-specific specialist care

B. Surgical referral indications

  • Open fracture / penetrating injury
  • Vascular injury
  • Suspected nerve transection
  • Progressive neurological deficit
  • Severe intractable neurogenic pain
  • Mass lesion/ganglion/tumor compression
Closed humeral shaft fracture-associated radial nerve palsy-তে systematic review-এ >90% recovery without re-intervention reported হয়েছে।
  • Late irreversible palsy: tendon transfer, nerve graft/transfer specialist decision।
