TC(1059).(05).Orthopedics Case No 137:- Kienböck Disease-1

Kienböck Disease — বাংলায় Clinical + Prescription-Oriented Notes

Clinical key point: Kienböck disease-এ tablet/medicine দিয়ে lunate bone-এর avascular necrosis reverse করা যায় না। Medicine mainly pain ও inflammation control করে; definitive treatment হয় stage-based splint/cast, hand therapy, এবং প্রয়োজন হলে hand surgery। AAOS ও StatPearls অনুযায়ী treatment goal হলো pain relief, wrist motion ও grip strength preserve করা।

1. Definition —

2. Causes / Etiology —

  • Trauma / repeated microtrauma
    • পড়ে গিয়ে wrist injury, repeated wrist loading, heavy manual work, push-up type loading—এসব lunate-এর vessels damage করতে পারে।
  • Negative ulnar variance
  • Radius–ulna length mismatch
  • Reduced radial inclination
  • Lunate morphology / shape variation
  • Vascular / systemic association

3. Symptoms —

AAOS-এর common symptoms: wrist pain, swelling, stiffness/limited ROM, decreased grip strength, dorsal central wrist tenderness।

Point-wise details

    • Grip, push-up, lifting, typing, manual work করলে বাড়ে।
  • Wrist swelling
  • Wrist stiffness
  • Grip strength
  • Tenderness over lunate
  • Activity-related intermittent pain
  • Late stage symptoms
    • Increasing pain, grip weakness, limited motion, crepitus, chronic disability, arthritis-like pain।

4. Clinical Diagnosis — clinically কীভাবে diagnose করবেন

History

  • Chronic wrist pain: usually months to years.
  • Previous fall / sprain-like history.
  • Dominant hand involvement.
  • Occupation: manual worker, mechanic, carpenter, athlete, gym/push-up habit.
  • Pain increases with wrist extension/loading.
  • Past history: steroid use, SLE, sickle cell disease, smoking, thrombophilia, renal disease.
  • Rule out inflammatory arthritis, gout, infection, occult fracture.

Examination

  • Inspection: dorsal wrist swelling, reduced motion.
  • Palpation: lunate tenderness — dorsal wrist center, roughly 3rd metacarpal axis proximal to capitate.
  • ROM: wrist flexion-extension reduced; extension often painful.
  • Grip strength: compare with opposite hand.
  • Special assessment:
    • Watson test — scapholunate/scaphoid pathology rule out.
    • TFCC/ulnar-sided pain test if ulnar-sided wrist pain.
    • Neurovascular exam.
    • Compare both wrists.

Clinical suspicion

Kienböck suspect

  • Chronic dorsal central wrist pain +
  • Grip weakness +
  • Lunate tenderness +
  • X-ray normal হলেও symptoms persist করে +
  • MRI-তে lunate signal change থাকে।

5. Investigations — specific tests

1) X-ray wrist — PA, lateral, oblique

  • Early stage-এ X-ray normal হতে পারে।
  • Later findings:
    • Lunate sclerosis
    • Lunate collapse
    • Fragmentation
    • Carpal collapse
    • Secondary radiocarpal/midcarpal arthrosis

2) MRI wrist — best early investigation

  • Early diagnosis-এর জন্য MRI most sensitive।
  • Hallmark: T1-weighted image-এ lunate marrow signal diffusely decreased।
  • STIR/T2 marrow edema, necrosis extent, cartilage condition
  • Contrast MRI perfusion/viability assess করে, revascularization planning-এ helpful।

3) CT scan wrist

  • Surgical planning-এর জন্য useful।
  • Subchondral fracture, coronal fracture, fragmentation, collapse, carpal instability better

4) Blood tests — diagnosis confirm করে না, but differential/risk assessment

  • CBC, ESR, CRP — infection/inflammatory arthritis exclude.
  • RF, anti-CCP — rheumatoid arthritis suspicion.
  • Serum uric acid — gout suspicion.
  • HbA1c, lipid profile — vascular risk.
  • ANA/ENA if SLE suspected.
  • Thrombophilia screen only selected young/bilateral/recurrent AVN cases.

6. Differential Diagnosis

  • Wrist sprain / ligament injury
  • Scaphoid fracture or scaphoid nonunion
  • Scapholunate ligament injury
  • Ulnar impaction syndrome
  • TFCC injury
  • Lunate intraosseous ganglion
  • Wrist osteoarthritis
  • Rheumatoid arthritis
  • Gout / CPPD
  • Osteoid osteoma of carpal bone
  • Bone contusion after trauma

StatPearls specifically notes ulnar impaction syndrome, lunate intraosseous ganglion, bone contusion, arthritis, osteoid osteoma as important differentials.

7. Complications — রোগের জটিলতা

Kienböck disease untreated/progressive হলে lunate bone collapse করে wrist biomechanics নষ্ট করে। StatPearls অনুযায়ী complications include scapholunate dissociation, secondary radiocarpal/midcarpal degenerative arthrosis, triquetrum malalignment।

সহজ ভাষায় complications

  • Lunate collapse
    • Lunate bone
  • Carpal instability
  • Scapholunate dissociation
  • Chronic wrist pain
    • Long-term pain, কাজের সময় pain flare।
  • Grip weakness
    • Daily activity ও professional work capacity কমে যায়।
  • Wrist stiffness
    • Flexion-extension permanent
  • Secondary wrist arthritis
    • Late stage-এ radiocarpal/midcarpal arthritis হয়, pain ও stiffness স্থায়ী হয়।
  • Functional disability
    • Manual worker, surgeon, typist, athlete—সব ক্ষেত্রে performance affect করতে পারে।

8. Treatment / Management — stage-based

AAOS অনুযায়ী treatment stage, lunate collapse/fragmentation, arthritis, activity level ও patient goal অনুযায়ী করা হয়।

A. Non-pharmacological management

Stage I / very early Stage II

  • Wrist splint বা short-arm cast: usually 6–12 weeks
  • Activity modification:
    • Push-up, heavy lifting, hammering, vibration tool avoid
  • NSAID/analgesic for pain
  • Follow-up X-ray/MRI
  • Physiotherapy after immobilization

AAOS early stage-এ anti-inflammatory medicine ও short period splinting/casting recommend করে।

Stage II–IIIA

  • Joint unloading / joint leveling surgery:
    • Radial shortening osteotomy — especially negative ulnar variance
    • Radial wedge osteotomy
    • Capitate shortening in selected cases
  • Vascularized bone graft / revascularization
  • Core decompression in selected early cases

Stage IIIB–IV / collapse or arthritis

  • Limited intercarpal fusion

12. Overview / Summary

Kienböck disease হলো lunate bone-এর AVN, যার cause usually multifactorial: blood supply variation, trauma/repetitive loading, negative ulnar variance, wrist biomechanics। Main symptoms হলো dorsal central wrist pain, swelling, stiffness, grip weakness, lunate tenderness। Early X-ray normal হতে পারে, তাই persistent clinical suspicion থাকলে MRI wrist best investigation। Treatment stage-based: early stage-এ splint/cast + pain control, advanced stage-এ joint unloading/revascularization/salvage surgery।

13. Actionable Plan / Steps

  1. Suspected case: PA/lateral wrist X-ray + immobilization + paracetamol/topical NSAID.
  2. Persistent pain or normal X-ray but high suspicion: MRI wrist.
  3. Confirmed Kienböck: Lichtman stage determine
  4. Stage I–early II: splint/cast 6–12 weeks + activity restriction + analgesia.
  5. Stage II–III: hand surgeon referral for joint leveling/revascularization planning.
  6. Stage IV arthritis: salvage procedure discussion — PRC, partial/total fusion.
  7. Drug safety: avoid double NSAID; assess renal function, BP, ulcer history, anticoagulant/steroid use.
  8. Follow-up: pain, grip strength, ROM, X-ray/MRI progression monitor.
