Tarsal Tunnel Syndrome —

1) Definition /

2) Etiology / Causes —

A. Mechanical / Local compression causes

  • Flat foot / fallen arch / over-pronation
  • Ganglion cyst / lipoma / schwannoma / mass lesion
  • Varicose vein / engorged vein
  • Tenosynovitis / tendon swelling
  • Bone spur / tarsal coalition / deformity

B. Systemic / medical causes

  • Diabetes mellitus — peripheral nerve vulnerable হয়; swelling ও neuropathy coexist করতে পারে।
  • Rheumatoid arthritis / inflammatory arthritis — synovitis ও tendon sheath swelling nerve compress করতে পারে।
  • Hypothyroidism, obesity, pregnancy, chronic edema — fluid retention ও tissue swelling হয়।
  • Chronic renal failure / dialysis, systemic sclerosis — neuropathy/soft-tissue change risk
Systematic review-এ idiopathic, post-traumatic, lipoma, cyst, ganglion, schwannoma, varicose plantar veins, anatomic anomaly ও systemic inflammatory conditions reported causes হিসেবে উল্লেখ আছে।

3) Symptoms —

  • Tingling / pins and needles —
  • Electric shock-like pain — medial ankle
  • Local tenderness — medial malleolus-এর posterior-inferior side-এ tenderness।
  • Advanced case: intrinsic foot muscle weakness, toe clawing tendency, muscle wasting, gait disturbance।
Johns Hopkins TTS symptoms হিসেবে shooting pain, numbness, tingling ও burning sensation উল্লেখ করেছে।

4) Clinically Diagnosis — কীভাবে clinically ধরবেন

A. History

  • Pain location: medial ankle → plantar foot/heel/toes
  • Neuropathic character: burning, tingling, electric shock, numbness
  • Aggravating factor: prolonged standing/walking/running
  • Risk factor: diabetes, arthritis, trauma, flat foot, pregnancy, hypothyroidism, swelling
  • Rule out: low back pain/radiculopathy, plantar fasciitis, Morton’s neuroma

B. Examination — specific clinical tests

1. Tinel’s sign over tarsal tunnel

2. Dorsiflexion–Eversion Test

  • Ankle maximum dorsiflexion + eversion, toes extension; 5–10 seconds
  • Pain/tingling/numbness reproduce হলে positive।

3. Triple Compression Stress Test — TCST

  • Supine position; ankle full plantar flexion, foot inversion, posterior tibial nerve-এর উপর 30 sec digital pressure।
  • Toe/ball of foot/heel-এ numbness/paresthesia/pain হলে positive। PubMed abstract অনুযায়ী testটি posterior tibial nerve entrapment provoke করার জন্য designed।

4. Sensory–motor assessment

  • Plantar sensation: medial plantar, lateral plantar, calcaneal nerve distribution
  • Two-point discrimination
  • Intrinsic foot muscle weakness/atrophy
  • Gait, arch, hindfoot valgus/varus, over-pronation
  • Pulses, edema, mass, varicose vein

5) Investigations — specific workup

A. Confirmatory / supportive tests

  • Nerve conduction study + EMG
  • Ultrasound ankle/tarsal tunnel
Ganglion cyst, tenosynovitis, varicose vein, nerve swelling, dynamic compression
  • MRI ankle/foot
    Space-occupying lesion, ganglion, tumor, tendon sheath pathology, bony/tarsal coalition, post-traumatic scar tissue—suspected হলে best। Johns Hopkins imaging হিসেবে X-ray/CT/MRI উল্লেখ করেছে।
  • X-ray ankle/foot
Fracture, bone spur, deformity, coalition, arthritis, calcaneal spur rule out।

B. Cause-based laboratory tests

  • FBS/RBS + HbA1c — diabetes/peripheral neuropathy
  • TSH — hypothyroidism
  • ESR/CRP, RF, anti-CCP — rheumatoid/inflammatory arthritis
  • Serum B12 — neuropathic symptoms/deficiency risk
  • Renal function, LFT — drug selection before NSAID/gabapentinoid/duloxetine

6) Differential Diagnosis

  • Plantar fasciitis — heel pain, first-step morning pain; usually paresthesia কম।
  • S1 radiculopathy / lumbar disc disease — back pain, radiating leg pain, reflex/sensory root pattern।
  • Diabetic peripheral neuropathy — bilateral stocking pattern।
  • Morton’s neuroma — forefoot/toe web-space pain।
  • Baxter’s nerve entrapment — medial heel pain।
  • Stress fracture / arthritis / tendinopathy — localized bony or tendon pain।
EFORT review TTS differential হিসেবে plantar fasciitis, lumbosacral radiculopathy, rheumatologic disease, metatarsal stress fracture ও Morton’s neuroma উল্লেখ করেছে।

  • Work limitation: standing/walking-based

8) Treatment Management

A. Non-pharmacological management —

  • Activity modification: দীর্ঘক্ষণ দাঁড়ানো, running, hard surface walking কমানো।
  • Footwear correction: soft sole, wide toe box, arch support।
  • Orthosis: flat foot/over-pronation থাকলে medial arch support/custom orthotic।
  • RICE: acute swelling থাকলে rest, ice, compression, elevation।
  • Physiotherapy: tibial nerve gliding, calf stretch, ankle mobility, intrinsic foot strengthening।
  • Immobilization: severe acute flare/tenosynovitis হলে short-term ankle brace/splint।
  • Cause treatment: diabetes control, RA control, hypothyroidism correction, mass/cyst management।
Conservative management সাধারণত activity modification, pain relief drugs, rehabilitation এবং corticosteroid injection অন্তর্ভুক্ত করে।

B. Surgical management —

  • 3–6 মাস adequate conservative treatment ব্যর্থ
  • Progressive neurological deficit
  • Clear space-occupying lesion: ganglion, tumor, varicose vein, bony lesion
  • Severe NCS abnormality বা persistent disabling pain
Surgical tarsal tunnel release-এ flexor retinaculum decompress করা হয়; EFORT review-এ surgery success rate 44–96% reported।

9) Bangladesh doctors commonly use যে drug groups

  • Do not combine sedating neuropathic agents casually

Suspected local inflammatory compression

  • US-guided corticosteroid injection by trained clinician
  • Consider MRI/US before injection if mass suspected

Refractory/progressive case

  • Orthopedic foot-ankle / neurology referral
  • NCS/EMG + MRI/US
  • Surgical decompression if conservative failure or clear lesion

Overview / Summary

Tarsal Tunnel Syndrome হলো posterior tibial nerve compression neuropathy। Core symptoms হলো plantar burning pain, tingling, numbness, standing/walking-এ worsening।

Actionable Plan / Steps

  1. Confirm neuropathic pattern: medial ankle থেকে sole/toe burning/tingling আছে কি না।
  2. Examine: Tinel’s sign, dorsiflexion–eversion test, TCST, sensory map, foot arch/pronation, mass/varicosity।
  3. Initial management 2–6 weeks: footwear/orthotic + activity modification + physio + paracetamol/topical NSAID ± short NSAID.
  4. Neuropathic pain prominent হলে: pregabalin/gabapentin/amitriptyline/duloxetine থেকে patient profile অনুযায়ী one agent start।
  5. Cause search: diabetes, RA, hypothyroidism, trauma, cyst/mass; HbA1c, TSH, ESR/CRP/RF/anti-CCP as indicated।
  6. Persistent/progressive symptoms: NCS/EMG + US/MRI; steroid injection or surgical referral based on cause।
