Ganglion Cyst — বাংলা Clinical Note

1. Definition — Ganglion cyst কী?

Ganglion cyst হলো joint capsule বা tendon sheath-এর পাশে তৈরি হওয়া benign, fluid-filled, jelly-like cystic swelling।

2. Cause / Etiology / Pathophysiology —

  • Mucinous degeneration: connective tissue-এর degeneration হয়ে thick jelly-like mucin জমে cyst হয়।
  • Synovial fluid leakage / one-way valve mechanism: joint

3. Symptoms —

  • Reduced range of motion: wrist/finger movement

4. Clinical Diagnosis — Specific clinical approach

History

  • Lump
  • Size
  • Tingling, numbness, weakness
  • Previous aspiration/surgery history?
  • Fever/redness/severe tenderness থাকলে infection/abscess ভাবতে হবে।

Examination

  • Inspection: location—dorsal wrist, volar wrist, flexor tendon sheath, DIP mucous cyst।
  • Neurovascular exam: radial/ulnar pulse, capillary refill, sensation, grip power।
  • Red flag: pulsatile swelling হলে radial artery aneurysm/vascular lesion exclude করতে হবে।

5. Investigations — Specific test list

  • No investigation required in typical case: classic wrist ganglion clinically diagnose
  • X-ray wrist/hand: cyst দেখা যায় না, কিন্তু arthritis, bone tumor, intraosseous lesion exclude করতে useful। AAOS ও StatPearls—দুই source-ই X-ray mainly rule-out purpose-এ বলে।
  • Ultrasound ± Doppler: cystic vs solid lesion, vascular malformation/aneurysm exclude, aspiration guide করতে useful। StatPearls ultrasound-কে cyst vs vascular malformation differentiate করতে useful বলেছে।
  • MRI: occult/deep ganglion, atypical lesion, solid tumor suspicion, pre-operative planning—এই ক্ষেত্রে। StatPearls অনুযায়ী routine MRI usually indicated নয়, unless solid tumor concern থাকে।
  • Aspiration finding: thick, clear, gelatinous fluid।
  • Histopathology: excision করলে confirmatory; atypical/solid lesion হলে malignancy/GCTTS rule out।

6. Differential Diagnosis

  • Giant cell tumor of tendon sheath
  • Lipoma
  • Epidermoid inclusion cyst
  • Tenosynovitis
  • Carpal boss
  • Radial artery aneurysm / vascular malformation
  • Abscess
  • Rheumatoid nodule
  • Gouty tophus
  • Mucous cyst with DIP osteoarthritis
  • Bone tumor / intraosseous cystic lesion
  • Synovial sarcoma—rare but atypical solid/deep/progressive mass হলে consider

7. Complications — রোগের জটিলতা

  • Pain ও functional limitation: বড় cyst joint movement ও daily work disturb করতে পারে।
  • Nerve compression: tingling, numbness, weakness
  • Reduced grip strength: especially wrist ganglion-
  • Surgical complication: scar, stiffness, infection, neuroma, tendon/nerve/vascular injury, recurrence।
  • Permanent disability rare: AAOS বলছে ganglion cyst rarely permanent disability করে।

8. Treatment / Management — Guideline-based practical approach

A. Asymptomatic ganglion

  • Observation + reassurance
  • No drug required.

B. Painful but mild

  • Activity modification
  • Wrist splint/brace 1–3 weeks
  • Short course analgesic/NSAID if pain
  • Physiotherapy/ROM exercise pain

AAOS initial treatment nonsurgical বলে; observation, immobilization, aspiration—এই options দেয়।

C. Painful, large, functional limitation

D. Recurrent / persistent / severe symptoms

  • Surgical excision / ganglionectomy
  • Cyst + pedicle/root remove
  • Scientific Reports 2025 study-তে surgical excision success 92.5%, recurrence 7.5%; aspiration + steroid injection success 64.3%, recurrence 35.7% দেখা গেছে।

Overview / Summary

Diagnosis mostly clinical—visible lump, transillumination, neurovascular exam; ultrasound/MRI/X-ray selected cases।

Actionable Plan / Steps

  1. Asymptomatic small cyst: reassure + observe.
  2. Painful cyst: splint + activity modification + paracetamol/topical diclofenac.
  3. Inflammatory pain: short-course NSAID after risk assessment; avoid combining two NSAIDs.
  4. GI-risk patient: NSAID দিলে PPI consider.
  5. Numbness/weakness/functional limitation: ultrasound ± orthopedic referral.
  6. Recurrent or large symptomatic cyst: aspiration or surgical excision discussion.
  7. Red flags: rapid growth, hard fixed mass, pulsatile swelling, fever/redness, severe night pain—urgent evaluation.
