TC(1012).(05).Orthopedics Case No 90:- Blount Disease / Tibia Vara

Blount Disease / Tibia Vara — বাংলা Clinical Note

1. Definition —

2. Cause / Etiology —

  • Mechanical overload
  • Obesity / overweight
  • Early walking
Infantile Blount disease-এ early walking একটি recognized risk factor।
  • Genetic susceptibility
  • Large stature / heavy build
  • Physiological bowing

3. Symptoms / Clinical Features —

A. Infantile Blount Disease

  • Bow legs /
  • Usually bilateral
  • Internal tibial torsion
  • Lateral thrust gait
  • Pain
    Infantile form-এ pain rare; deformity প্রধান complaint।

B. Adolescent Blount Disease

  • Medial knee pain
  • Obesity-associated gait problem
  • Limb-length discrepancy

4. Clinical Diagnosis — clinically

Clinical suspicion

5. Investigations — specific test

A. X-ray — most important

Standing long-leg AP radiograph: hip-to-ankle view

X-ray findings

  • Medial metaphyseal beaking
  • Medial physis widening / irregularity
  • Medial metaphyseal sloping
  • Varus alignment
  • Tibiofemoral angle abnormality
  • Langenskiöld stage I–VI classification

B. Metaphyseal-Diaphyseal Angle / Levine-Drennan angle

  • <10° → usually physiological bowing; spontaneous correction likely
  • 11–16° → close follow-up needed
  • >16° → high risk of progression / Blount disease likely

C. MRI

  • child age >4 years কিন্তু X-ray equivocal
  • cartilage/physis damage assess করতে হয়
  • severe/neglected case
  • physeal bar suspected

D. Lab tests — disease confirm

Blount disease-এর diagnosis সাধারণত X-ray-based।

  • Serum calcium
  • Serum phosphate
  • Alkaline phosphatase
  • 25-OH Vitamin D
  • PTH, যদি rickets/metabolic bone disease suspect হয়
  • CBC/ESR/CRP, যদি infection/osteomyelitis suspect হয়

6. Differential Diagnosis

  • Physiological genu varum
  • Nutritional rickets / vitamin D deficiency rickets
  • Renal osteodystrophy
  • Skeletal dysplasia
  • Metaphyseal chondrodysplasia
  • Proximal tibial physeal injury
  • Osteomyelitis
  • Ollier disease
  • Post-traumatic growth arrest

Rickets-এ সাধারণত diffuse metaphyseal widening/cupping/fraying থাকে; Blount disease-এ asymmetrical medial beaking ও sharp angular deformity বেশি দেখা যায়।

7. Complications — রোগের জটিলতা

  • Progressive deformity
  • Abnormal gait
  • Knee pain
    বিশেষ করে adolescent Blount disease-এ medial knee pain common।
  • Limb-length discrepancy
  • Joint degeneration / early osteoarthritis
  • Surgical complications
wound infection, malalignment, compartment syndrome, peroneal nerve injury, hardware migration, premature physeal closure, rebound growth ইত্যাদি হতে পারে।

8. Treatment / Management

A. Non-pharmacological management — main treatment

1. Observation

2. Brace / KAFO

  • Best for: age <3–4 years, Langenskiöld stage I–II, non-obese child
  • Brace type: Knee-Ankle-Foot Orthosis / KAFO

3. Surgery

Surgery indicated when:

  • age ≥3 years with progressive disease
  • Langenskiöld stage III or more
  • brace failure
  • lateral thrust present
  • adolescent Blount disease
  • severe deformity

Common surgery:

  • Guided growth / hemiepiphysiodesis
  • Proximal tibial osteotomy
  • Gradual correction with Ilizarov/Taylor Spatial Frame
  • Physeal bar management
  • Medial tibial plateau elevation in severe neglected cases

Treatment choice depends on age, severity, skeletal maturity, deformity plane, and remaining growth.

  • ২ বছরের পর bow leg থাকলে “wait and see” না করে orthopedics consult
  • vitamin D deficiency/rickets থাকলে early diagnosis and treatment
  • শিশুর gait abnormality, one-sided bowing, knee thrust হলে দ্রুত X-ray
  • family education: physiological bowing সাধারণত improve করে; কিন্তু progressive/asymmetric bowing dangerous sign
  • surgery হলে infection prevention: sterile technique, appropriate antibiotic prophylaxis, wound care, follow-up

Overview / Summary

Blount disease হলো pediatric tibial growth plate disorder, যেখানে proximal medial tibia-এর growth inhibit হয়ে progressive bow leg হয়। Cause multifactorial: obesity, early walking, mechanical overload, genetic susceptibility।

Actionable Plan / Steps

  1. শিশুর বয়স, weight, walking age, unilateral/bilateral deformity note করুন।
  2. Standing hip-to-ankle AP X-ray
  3. MDA/Levine-Drennan angle measure
  4. Rickets exclude করতে Ca, phosphate, ALP, 25-OH vitamin D করুন।
  5. Age <3–4 years + stage I/II হলে KAFO brace consider করুন।
  6. Age ≥3 years, stage III+, lateral thrust, adolescent disease, brace failure হলে pediatric orthopedic surgery referral করুন।
  7. Pain থাকলে paracetamol/ibuprofen short course; surgery হলে antibiotic/pain plan surgeon অনুযায়ী।
