Brand Model and Executive

TC(1011).(05).Orthopedics Case No 89:- Slipped Capital Femoral Epiphysis — SCFE / SUFE

Slipped Capital Femoral Epiphysis — SCFE / SUFE

1. Definition —

Clinically এটি hip/groin/thigh/knee pain, limp, limited internal rotation এবং severe হলে inability to bear weight দিয়ে আসে।

2. Causes / Etiology / Pathophysiology —

A. Mechanical cause

  • Obesity / overweight
বেশি body weight growth plate-এর উপর shear force বাড়ায়; SCFE-এর strongest risk factor obesity।
  • Pubertal growth spurt
  • Femoral retroversion / acetabular retroversion

B. Endocrine / metabolic cause

  • Hypothyroidism — atypical SCFE-এর important cause; non-idiopathic SCFE-এর মধ্যে hypothyroidism common।
  • Growth hormone therapy / GH abnormality
  • Hypogonadism
  • Panhypopituitarism
  • Renal osteodystrophy / chronic kidney disease
  • Vitamin D deficiency / rickets / osteomalacia
  • Down syndrome — endocrine/metabolic association

C. Other risk factors

  • Male sex
  • Family history
  • Previous radiation to hip region
  • Bilateral risk — এক hip-এ হলে contralateral hip-এও হতে পারে; reported bilateral cases around 25%, range wide।

Pathophysiology

3. Symptoms —

Stable SCFE

  • Hip pain / groin pain — activity
  • Limping gait
  • Out-toeing gait — affected leg
  • Pain gradually weeks–months
  • Internal rotation
  • Running/jumping করলে pain বাড়ে।

Unstable SCFE

  • Sudden severe hip/groin/thigh pain
  • Minor trauma/fall-
  • Unable to bear weight even with crutches
  • Affected leg externally rotated
  • Leg short

4. Clinical Diagnosis — clinically

History

  • Age: usually adolescent
  • Pain location: hip/groin/thigh/knee
  • Duration: acute, acute-on-chronic, chronic
  • Weight bearing ability: stable vs unstable classification-
  • Obesity, endocrine disease, renal disease, GH therapy, family history

Physical examination

  • Antalgic limp
  • Out-toeing gait
  • Hip flexion করলে affected leg involuntarily external rotation-এ চলে যায় — Drehmann sign
  • Limited internal rotation — very important sign
  • Limited abduction and flexion
  • Pain at extremes of hip motion

Clinical classification

TypeMeaningClinical importance

5. Investigations — specific investigation

A. First-line imaging

1. X-ray pelvis with both hips

Views:

  • AP pelvis view
  • Frog-leg lateral view — stable SCFE-তে useful
  • Cross-table lateral view — unstable SCFE-তে frog-leg avoid করে pain/slip worsening কমাতে করা হয়।

B. X-ray signs

  • Physeal widening / irregularity
  • Posterior–inferior displacement
  • Southwick angle: slip severity measure

C. MRI

  • X-ray normal কিন্তু clinical suspicion strong
  • Pre-slip / early SCFE suspected
  • Bilateral occult disease

E. Laboratory tests —

Atypical clues:

  • Age <10
  • Very short stature
  • Underweight/weight <50th percentile
  • Bilateral SCFE
  • Recurrent/very severe slip
  • Known renal/endocrine disease।

Suggested labs:

  • TSH, Free T4
  • Serum calcium, phosphate, alkaline phosphatase
  • 25-OH vitamin D
  • PTH
  • Urea/creatinine, electrolytes
  • Fasting glucose/HbA1c if obesity/metabolic syndrome suspected
  • LH/FSH/testosterone/estradiol if delayed puberty/hypogonadism suspected

6. Differential Diagnosis

  • Transient synovitis
  • Septic arthritis
  • Perthes disease
  • Femoral neck fracture
  • Osteomyelitis
  • Juvenile idiopathic arthritis
  • Hip dysplasia
  • Muscle strain/sports injury
  • Knee pathology — but adolescent knee pain + limp হলে hip examine করতে হবে।

7. Complications — রোগের জটিলতা

1. Avascular necrosis — AVN

2. Chondrolysis

3. Femoroacetabular impingement — FAI

4. Early osteoarthritis

5. Slip progression

6. Bilateral SCFE

8. Treatment / Management

A. Immediate management

  • No weight bearing immediately
  • Urgent orthopedic admission/referral
  • Pain control
  • X-ray confirmation
  • Surgery planning।

B. Definitive treatment — Surgery

Stable SCFE

  • In-situ single screw fixation most common/gold standard for mild–moderate stable slips।
  • Forceful closed reduction

Unstable SCFE

  • Urgent stabilization
  • Gentle reduction/open approach may be needed
  • AVN risk বেশি, তাই specialist pediatric orthopedic management দরকার।

Severe deformity

  • Corrective osteotomy / modified Dunn procedure selected cases-এ; complication risk আছে, তাই experienced center দরকার।

Contralateral hip fixation

High-risk হলে consider:

  • Age <10
  • Endocrine disease
  • Severe obesity
  • Bilateral tendency
  • Skeletal immaturity।

  • Endocrine disease control — hypothyroidism, renal osteodystrophy, vitamin D deficiency, delayed puberty থাকলে proper treatment।
  • Known SCFE হলে opposite hip follow-up — contralateral slip risk থাকলে regular clinical + radiologic monitoring।
  • High-risk child: age <10, bilateral symptoms, short stature, endocrine disease থাকলে endocrine workup।

Overview / Summary

SCFE হলো adolescent hip-এর growth plate disorder যেখানে femoral head slip করে। Main presentation: limp + hip/groin/thigh/knee pain + limited internal rotation। Diagnosis-এর best first test হলো bilateral AP pelvis + lateral hip X-ray। Treatment medicine নয়—urgent non-weight bearing + orthopedic surgery, most commonly in-situ screw fixation।

Actionable Plan / Steps

  1. Suspected SCFE হলে patient-কে হাঁটাবেন না — wheelchair/crutch, no weight bearing।
  2. Urgent orthopedic referral/admission।
  3. X-ray: AP pelvis + frog lateral if stable; AP pelvis + cross-table lateral if unstable।
  4. Pain control: paracetamol first; NSAID only if no contraindication।
  5. Confirm stable vs unstable — weight bearing ability
  6. Definitive treatment: in-situ screw fixation / specialist surgery।
  7. Atypical case হলে labs: TSH, FT4, Ca, PO4, ALP, PTH, vitamin D, renal function।
  8. Post-op follow-up: X-ray monitoring until physeal closure; AVN/chondrolysis/FAI signs watch।
