Hello I'm Raiyyan Tahsan
Brand Model and Executive

TC(982).(05).Orthopedics Case No 60.Lumbar Disc Herniation

Lumbar Disc Herniation — বাংলা Clinical Note

1) Definition /

এতে low back pain, radiculopathy/sciatica, sensory–motor deficit, এমনকি bladder/bowel dysfunction হতে পারে।

2) Causes / Etiology —

A. Degenerative cause — সবচেয়ে common

  • intervertebral degeneration lumbar disc herniation-এর common underlying cause।

B. Mechanical stress

  • repeated bending, prolonged sitting, driving vibration।

C. Lifestyle risk factor

  • Obesity / overweight।
  • Poor posture।
  • Core muscle weakness।
  • Sedentary lifestyle।
  • Smoking — disc nutrition কমিয়ে degeneration বাড়ায়।

D. Occupational factor

  • Construction worker, driver, manual laborer।
  • repeated lifting/twisting job।

E. Trauma

  • Fall, accident, direct injury।
  • sudden axial loading।

F. Genetic predisposition

  • family history থাকলে disc degeneration/herniation risk বাড়তে পারে।

3) Symptoms / Clinical Features —

A. Low back pain

B. Sciatica / Radicular pain

  • pain সাধারণত sharp, shooting, electric shock-like হয়।
  • cough/sneeze/straining করলে pain বাড়তে পারে।

C. Numbness / Tingling

  • burning sensation।
  • dermatomal sensory loss।

D. Muscle weakness

E. Reflex changes

  • L4 root: knee jerk
  • S1 root: ankle jerk

F. Root-wise clinical pattern

Nerve rootPain/Sensory areaMotor weaknessReflex
L4anterior thigh, medial legknee extension weaknessknee jerk ↓
L5lateral leg, dorsum foot, great toegreat toe extension/ankle dorsiflexion weaknessusually normal
S1posterior calf, lateral foot/soleplantar flexion weaknessankle jerk ↓

G. Red flag symptoms — emergency

  • Urinary retention/incontinence।
  • Bowel incontinence।
  • Saddle anesthesia।
  • Bilateral sciatica।
  • Progressive motor weakness।
  • Fever, weight loss, cancer history।
  • Severe night pain।
  • Recent trauma।

NICE guideline বলেছে new/changed low back pain with or without sciatica হলে cancer, infection, trauma, inflammatory disease-এর মতো alternative diagnosis/red flags exclude করতে হবে।

4) Clinical Diagnosis — কিভাবে clinically diagnosis করবেন

A. History

  • Pain onset: sudden/gradual।
  • Dermatomal pattern
  • cough/sneeze/strain-এ pain বাড়ে কিনা।
  • numbness, weakness, bladder/bowel symptoms।
  • trauma, fever, weight loss, malignancy history।
  • previous episode, occupational risk।

B. Physical examination

1. Inspection

  • posture।
  • antalgic gait।
  • scoliosis/listing।
  • muscle spasm।

2. Range of motion

  • lumbar flexion painful/restricted।
  • extension painful কিনা।

3. Neurological examination

  • Power: hip flexion, knee extension, ankle dorsiflexion, great toe extension, plantar flexion।
  • Sensory: L4, L5, S1 dermatome।
  • Reflex: knee jerk, ankle jerk।
  • Gait: heel walking, toe walking।

4. Special tests

  • Straight Leg Raise Test / SLR: 30–70°-এ radicular leg pain হলে positive।
  • Crossed SLR: opposite leg raise করলে affected leg pain—more specific for disc herniation।
  • Femoral nerve stretch test: upper lumbar root L2–L4 suspicion

SLR lumbar disc herniation/radiculopathy screening-এ commonly used; sensitivity বেশি হলেও specificity variable, তাই history + neuro exam + imaging correlation জরুরি।

A. MRI lumbosacral spine — investigation of choice

Indication:

  • severe radicular pain।
  • progressive neurological deficit।
  • foot drop।
  • cauda equina symptoms।
  • epidural injection/surgery plan করলে।

B. X-ray lumbosacral spine

Useful when:

  • trauma।
  • fracture suspicion।
  • spondylolisthesis।
  • deformity।
  • malignancy suspicion।

C. CT scan lumbar spine

Use when:

  • MRI contraindicated।
  • pacemaker/metallic implant।
  • bony canal stenosis better evaluate করতে।

D. CT myelogram

  • uncommon but useful in selected cases।

E. EMG/NCS

Use when:

  • radiculopathy বনাম peripheral neuropathy unclear।
  • symptoms chronic।

G. Recurrence

8) Treatment / Management — Guideline-based

A. General principle

  • Most patients conservative management-এ improve করে।
  • bed rest দীর্ঘদিন নয়।
  • activity maintain করতে হবে।
  • physiotherapy + education + analgesia।
  • red flag থাকলে urgent referral।

NICE self-management ও normal activity continue করার advice দিতে বলেছে।

B. Non-pharmacological treatment

1. Patient education

2. Activity modification

  • heavy lifting বন্ধ।

Overview / Summary

Lumbar disc herniation হলো lumbar disc-এর nucleus pulposus বেরিয়ে nerve root compress/irritate করা, যার ফলে low back pain, sciatica, numbness, weakness হতে পারে। Treatment প্রথমে conservative: education, activity, physiotherapy, short-course analgesic/NSAID ± PPI ± muscle relaxant। Severe refractory sciatica হলে epidural steroid injection, আর cauda equina/progressive weakness/persistent disabling MRI-correlated radiculopathy হলে surgery consideration।

Actionable Plan / Steps

  1. Red flag check করুন: urinary retention, saddle anesthesia, bilateral sciatica, progressive weakness থাকলে urgent MRI/referral।
  2. Clinical localization করুন: L4/L5/S1 dermatome, power, reflex, SLR।
  3. Uncomplicated হলে 2–6 সপ্তাহ conservative care: activity, posture, physio, NSAID short course + PPI if needed।
  4. Neuropathic symptoms severe হলে: guideline caution মাথায় রেখে selected patient-এ pregabalin/gabapentin/amitriptyline specialist judgment অনুযায়ী।
  5. 4–6 সপ্তাহেও disabling radicular pain থাকলে: MRI lumbosacral spine।
  6. MRI-confirmed nerve compression + persistent severe symptoms: epidural steroid or microdiscectomy discussion।
