Growth Plate Injury / Physeal Injury

Growth Plate Injury / Physeal Injury —

Growth plate injury-কে সাধারণত Salter–Harris type I–V দিয়ে classify করা হয়। Type III, IV, V-তে joint surface/growth plate বেশি জড়িত থাকায় complication risk বেশি।

1. Definition —

2. Cause / Etiology —

Common causes

  • Fall on outstretched hand — FOOSH:
  • Sports injury: football, cricket, gymnastics, cycling, jumping, martial arts।
  • Child abuse consideration: খুব ছোট বাচ্চার inconsistent history, multiple fractures, delay in presentation থাকলে non-accidental injury exclude করতে হয়।

Salter–Harris Type

  • Type I: fracture শুধু growth plate দিয়ে যায়।
  • Type II: growth plate + metaphysis; সবচেয়ে common।
  • Type III: growth plate + epiphysis; joint
  • Type IV: metaphysis + growth plate + epiphysis; joint ও growth plate দুটোই জড়িত।
  • Type V: crush/compression injury; X-ray initially normal হতে পারে, কিন্তু growth arrest risk বেশি।

3. Symptoms / Clinical Features —

Main symptoms

  • Movement limitation: wrist/ankle/knee/elbow
  • Unable to bear weight: ankle/knee injury
  • Bruising / ecchymosis: soft tissue injury
  • Severe pain with passive stretch: compartment syndrome-এর warning sign।
  • Numbness, tingling, cold limb: neurovascular compromise-এর sign—urgent referral needed।

4. Clinical Diagnosis — Clinically কীভাবে diagnosis করবেন

History

  • Injury mechanism: fall, twist, direct trauma, road accident।
  • Time since injury।
  • Pain severity and ability to move/use limb।
  • Weight-bearing ability।
  • Previous fracture বা bone disease history।
  • Tetanus immunization status if wound/open fracture।
  • Suspicion of abuse: history inconsistent

Physical examination

  • Inspection: swelling, deformity, wound, bruising।
  • Palpation: maximal tenderness over physis।
  • Range of motion: active/passive ROM।
  • Neurovascular exam: distal pulse, capillary refill, sensation, motor power।
  • Compartment assessment: pain out of proportion, tense swelling, pain on passive stretch।
  • Joint above and below examine

5. Investigations — Specific test

First-line

  • X-ray AP + lateral view: suspected bone/joint area।
  • Oblique view: ankle, elbow, wrist-এর কিছু fracture detect করতে helpful।
  • Include joint above and below: long bone injury
  • Comparison X-ray: small child বা subtle Salter–Harris I সন্দেহে opposite side X-ray occasionally helpful।

Advanced imaging

  • CT scan:
    • Salter–Harris III/IV
    • intra-articular fracture
    • surgical planning
    • displaced fracture detail দেখার জন্য।
  • MRI:
    • X-ray normal but strong clinical suspicion
    • Salter–Harris I/V occult injury
    • cartilage, physis, bone marrow edema, ligament injury দেখতে।
  • Ultrasound: very young children-এ cartilage ossification না হলে sometimes useful; operator dependent।
  • Lab tests: closed simple fracture-এ usually দরকার নেই। Open fracture/surgery/infection suspicion হলে CBC, CRP/ESR, blood group, renal function, etc.

  • Ligament sprain
  • Torus / buckle fracture
  • Greenstick fracture
  • Epiphyseal avulsion fracture
  • Osteomyelitis / septic arthritis — fever, raised inflammatory markers
  • Soft tissue contusion
  • Dislocation / subluxation
  • Non-accidental injury in children

Important complications

  • Limb length discrepancy:
  • Angular deformity: growth plate-
  • Early osteoarthritis: joint-involving fracture malunion হলে future arthritis risk।

8. Management / Treatment — Guideline-based

A. Immediate first aid

  • Injured limb immobilize
  • Ice pack
  • Limb elevate।
  • Severe pain, deformity, numbness, open wound, child unable to walk/use limb হলে emergency orthopaedic assessment।

B. ED / Orthopaedic management

  • Pain control: paracetamol ± ibuprofen; severe pain হলে hospital opioid analgesia।
  • Immobilization: splint/cast/backslab।
  • Closed reduction: displaced fracture হলে sedation/analgesia দিয়ে alignment correct।
  • Surgical fixation: Type III/IV, unstable/displaced fracture, failed reduction, open fracture, neurovascular compromise।
  • Follow-up: distal radial Salter–Harris I/II injury-এর জন্য fracture clinic review within 5 days with repeat X-ray recommended by RCH guideline.
  • Open fracture: antibiotics, tetanus prophylaxis, irrigation/debridement, stabilization; pediatric open fracture management-এর core principles হলো infection prevention, debridement, skeletal stabilization, soft tissue coverage।

9. Pharmacological drugs — Bangladesh brand + dose + purpose

  • Follow-up X-ray miss না করা, কারণ growth arrest late complication হতে পারে।

Overview / Summary

Diagnosis মূলত history, point tenderness over physis, X-ray; X-ray normal হলেও suspicion থাকলে MRI/CT দরকার হতে পারে। Treatment-এর মূল হলো immobilization, reduction, follow-up, এবং Type III/IV/displaced/open fracture হলে orthopaedic surgery। Drug শুধু pain, infection/tetanus prevention, postoperative care-এর জন্য।

Actionable Plan / Steps

  1. Injury হলে limb immobilize + ice + elevate।
  2. Deformity, severe pain, numbness, open wound, walking/using limb impossible হলে emergency orthopaedic referral।
  3. X-ray AP/lateral; joint involvement বা occult injury হলে CT/MRI।
  4. Closed fracture: paracetamol ± ibuprofen short course + cast/splint।
  5. Open fracture: urgent IV antibiotic, tetanus assessment, debridement/stabilization।
  6. Fracture clinic follow-up 5–7 days; growth disturbance risk থাকলে long-term follow-up until growth normal confirmed।
