TC(964).(05).Orthopedics Case No 42. Polytrauma / Multiple Trauma

Polytrauma / Multiple Trauma —

1) Definition — Polytrauma

Berlin definition অনুযায়ী polytrauma বলতে সাধারণত AIS ≥3 severity injury in ≥2 body regions + ≥1 physiological risk factor বোঝায়—যেমন SBP ≤90 mmHg, GCS ≤8, base excess ≤−6, INR ≥1.4/APTT ≥40 sec, অথবা বয়স ≥70 বছর।

  2. Fall from height — ছাদ, গাছ, construction site, stairs থেকে পড়ে যাওয়া।
  3. Crush injury — ভবন ধস, machinery, factory/industrial accident।
  4. Assault / violence — মারামারি, stabbing, gunshot, blast injury।
  5. Burn + inhalation injury — আগুন, chemical, electrical burn।
  6. Natural disaster — earthquake, cyclone-related collapse, flood rescue injury।
  7. Sports/occupational trauma — high-speed sports, agriculture machinery injury।

Core mechanism:

  1. Primary injury — direct mechanical damage: fracture, organ rupture, brain bleed, lung contusion।
  2. Hemorrhagic shock — blood loss → low BP → tissue hypoxia → lactic acidosis।
  3. Lethal triad — hypothermia + acidosis + coagulopathy → bleeding
  4. Secondary brain injury — hypotension/hypoxia থাকলে traumatic brain injury খারাপ হয়।
  5. Inflammatory response — SIRS → ARDS, AKI, sepsis, MODS

4) Symptoms / Clinical Features —

Airway / breathing

  • neck swelling, facial trauma, burn inhalation sign
  • unequal chest movement, tracheal deviation — tension pneumothorax suspicion

Circulation / shock

  • BP কম, pulse বেশি, cold clammy skin
  • heavy external bleeding
  • pelvic pain/instability
  • altered mental status from shock
  • urine output কম

Neurological

  • GCS কম, অজ্ঞান, seizure
  • unequal pupil
  • limb weakness/paralysis
  • repeated vomiting, severe headache

Abdomen / pelvis

  • abdominal pain, guarding, distension
  • pelvic pain, perineal bruising
  • blood at urethral meatus, hematuria

Musculoskeletal / soft tissue

  • open fracture, deformity, severe swelling
  • limb numbness, absent pulse
  • compartment syndrome: severe pain, tense limb, pain on passive stretch

A–B–C–D–E: Airway with cervical spine protection, Breathing, Circulation with hemorrhage control, Disability, Exposure/environmental control।

Bedside clinical assessment

  • Mechanism: high-speed RTA, fall > height, crush, blast, penetrating injury
  • Vitals: BP, pulse, RR, SpO₂, temperature
  • GCS + pupil response
  • Full body exposure with log-roll
  • FAST/eFAST if unstable

6) Investigations — specific list

Immediate bedside

  1. Pulse oximetry, ECG, BP monitoring
  2. Capillary glucose
  3. eFAST — intra-abdominal bleed, pericardial tamponade, pneumothorax/hemothorax
  4. Portable X-ray — chest, pelvis, suspected limb fracture
  5. Urinary catheter only if urethral injury excluded

Blood tests

  • CBC: Hb, WBC, platelet
  • Blood group, Rh, cross-match
  • PT/INR, APTT, fibrinogen
  • ABG/VBG: pH, lactate, base deficit
  • Urea, creatinine, electrolytes
  • LFT
  • Serum calcium during massive transfusion
  • CK, urine myoglobin if crush/rhabdomyolysis
  • Pregnancy test in reproductive-age female
  • Troponin/ECG if chest trauma or suspected cardiac contusion

Imaging

  • Unstable patient: eFAST + chest/pelvis X-ray → OR/IR/ICU decision
  • Stable patient: CT head, C-spine, chest, abdomen, pelvis with contrast / trauma pan-CT
  • CT angiography if vascular injury suspected
  • MRI spine later if cord injury suspected and CT insufficient

NICE major trauma guideline focuses on rapid identification and early management of major trauma in prehospital and hospital settings to reduce death and disability.

7) Differential Diagnosis

  1. Isolated head injury
  2. Isolated spinal cord injury
  3. Isolated blunt abdominal trauma
  4. Pelvic fracture with hemorrhage
  5. Myocardial infarction/syncope causing crash
  6. Stroke causing fall/accident
  7. Hypoglycemia/intoxication causing altered consciousness
  8. Sepsis/shock unrelated to trauma
  9. Non-accidental injury/assault

8) Management / Treatment — guideline-based

Shortcut emergency algorithm

Massive bleeding? → Stop bleeding / tourniquet / pelvic binder
A: Airway + C-spine protection
B: Oxygen, chest decompression/chest tube if needed
C: 2 large-bore IV/IO, blood, TXA, hemorrhage control
D: GCS, pupil, glucose, seizure control, CT head/neurosurgery
E: Full exposure + prevent hypothermia
Then: CT/OR/ICU/orthopedic/neurosurgery/rehab

A. Pre-hospital management

  • C-spine protection
  • Catastrophic bleeding control: direct pressure, tourniquet, hemostatic dressing
  • Pelvic binder if pelvic fracture suspected
  • High-flow oxygen if hypoxic/shock
  • IV/IO access if trained team available
  • Rapid transport to trauma-capable hospital
Field triage guidelines use physiologic criteria, anatomic injury, mechanism, and special considerations to identify patients needing trauma center care.

B. Emergency Department / Hospital management

1. Airway with cervical spine protection

  • Suction blood/vomit
  • Jaw thrust, airway adjunct
  • RSI + endotracheal intubation if airway compromised, GCS ≤8, severe facial trauma, inhalation injury
  • Surgical airway if cannot intubate/cannot oxygenate

2. Breathing

  • Oxygen, ventilation
  • Needle decompression/chest tube for tension pneumothorax
  • Chest tube for massive hemothorax
  • Pain control for rib fracture to prevent atelectasis/pneumonia; thoracic trauma guidelines emphasize oxygen/ventilation support and adequate analgesia.

3. Circulation / hemorrhage control

  • Direct pressure/tourniquet
  • 2 large-bore IV/IO
  • Warmed balanced crystalloid only as bridge
  • Massive transfusion protocol: PRBC + plasma + platelet as per hospital protocol
  • Tranexamic acid if active/suspected bleeding and within 3 hours
NICE recommends IV tranexamic acid as soon as possible in major trauma with active/suspected bleeding and not using it after 3 hours unless hyperfibrinolysis evidence exists.

4. Disability / neuro

  • GCS, pupil, limb movement
  • Correct hypoxia, hypotension, hypoglycemia
  • CT head when stable
  • Neurosurgery if EDH/SDH/ICH/mass effect
  • Mannitol/hypertonic saline only if raised ICP/herniation signs and under monitoring

5. Exposure + temperature

  • Fully expose, examine front/back
  • Prevent hypothermia: warm blanket, fluid warmer
  • Log-roll and spine exam
  • Secondary survey after primary survey stabilization

6. Definitive treatment

  • Emergency laparotomy for intra-abdominal bleeding
  • Thoracotomy/chest tube for chest injury
  • Angioembolization for pelvic/solid organ bleeding if available
  • External fixation / damage control orthopedics

Overview / Summary

Polytrauma হলো multiple severe injuries with physiological compromise। ABCDE + bleeding control + early TXA if bleeding within 3 hours + blood-based resuscitation + imaging only when appropriate + definitive surgery/ICU।

Actionable Plan / Steps

  2. Primary survey: A–B–C–D–E।
  5. Active/suspected bleeding + injury <3 h হলে TXA দিন।
  6. Stable হলে CT; unstable হলে eFAST/X-ray + OR/IR decision।
  7. Open fracture হলে early antibiotic + tetanus prophylaxis + debridement।
  8. ICU monitoring: brain, lung, kidney, coagulopathy, infection, DVT prevention।
  9. Early rehab: physiotherapy, nutrition, mental health support।

General Quaries of this topics:-

Question No 1.DVT Kake bole ?

DVT = Deep Vein Thrombosis

  • polytrauma, fracture, surgery
  • pelvic/leg fracture
  • obesity
  • cancer
  • pregnancy/postpartum
  • oral contraceptive pill
  • smoking
  • previous DVT/PE
  • inherited thrombophilia

Diagnosis

  • Clinical Wells score
  • D-dimer — low-risk patient-এ helpful
  • Compression Doppler ultrasound — main investigation
  • PE সন্দেহ হলে CT pulmonary angiography

NICE guideline DVT/PE-এর rapid diagnosis ও anticoagulant treatment-এর জন্য structured assessment recommend করে।

Treatment core meaning

Summary

Question No 2. Calf pain kake bole ?

Calf pain

Calf

Example:

Calf pain

  1. Muscle cramp —
  2. Muscle strain —
  3. Dehydration বা electrolyte imbalance
  4. Achilles tendon problem
  5. Varicose vein
  6. DVT — পায়ের deep vein-এ blood clot
  8. Poor blood circulation

DVT-related calf pain

CDC অনুযায়ী DVT-এর typical লক্ষণ হলো affected leg/arm-এ pain or tenderness, swelling, warmth, redness/discoloration।

  • recent surgery, fracture, polytrauma, long bed rest আছে

Core meaning:

Question No 3. Achilles tendon Kake bole ?

Achilles tendon

Real example

Achilles tendon problem

Achilles tendon rupture হলে

Achilles tendon

Core meaning
