Hello I'm Raiyyan Tahsan
Brand Model and Executive

TC(857).(20).Sexology Case No 10. Trauma-related sexual difficulty

Etiology / Must Causes —

A. Trauma-related causes

  • Sexual assault/rape, childhood sexual abuse, marital rape, intimate partner violence
  • Pelvic/genital injury, pelvic fracture, genital surgery, childbirth trauma
  • Medical trauma: painful pelvic exam, ICU/accident trauma, repeated invasive procedures
  • War/torture/disaster-related PTSD
  • Relationship betrayal, coercive sex, fear-based sexual experience

B. Psychological mechanism

  • PTSD: flashback, avoidance, hyperarousal, emotional numbing, dissociation, relationship difficulty—এসব PTSD–তে দেখা যায়।
  • Fear-conditioning: intimacy বা sexual arousal–কে brain “danger signal” হিসেবে নেয়
  • Shame, guilt, self-blame, disgust, low self-worth
  • Depression → libido
  • Anxiety/performance fear → erection/lubrication/orgasm

C. Physical mechanism

  • Pelvic floor muscle spasm → penetration pain/vaginismus
  • Vulvodynia/vestibulodynia, dyspareunia
  • Genital scar, infection, STI, PID, endometriosis
  • Diabetes, thyroid disease, hyperprolactinaemia, hypogonadism
  • Alcohol/substance use
  • Drugs: SSRI, antipsychotic, beta-blocker, opioid, finasteride

Trauma-related sexual difficulty

1. Sexual trauma / abuse

যেমন: rape, sexual assault, childhood sexual abuse, marital rape, forced sex, unwanted touching।

Real example:

2. PTSD — Post-traumatic Stress Disorder

PTSD–

Real example:
Sexual activity

3. Fear and anxiety

Anxiety sympathetic system activate

Male example: performance anxiety → erection
Female example: fear of penetration → vaginal muscle spasm → pain/vaginismus
।

4. Depression

Depression

Real example:
Patient

5. Shame, guilt and self-blame

Real example:
Patient

6. Dissociation

Sexual pleasure

Real example:
Patient

7. Pelvic floor muscle spasm

Vaginal opening

Female example: vaginismus, dyspareunia।
Male example: pelvic pain syndrome, ejaculation pain
।

8. Physical genital or pelvic injury

যেমন: pelvic fracture, genital tear, childbirth tear, episiotomy trauma, surgery scar, accident, rape-related injury।

কীভাবে sexual difficulty করে?
Scar, nerve damage, vascular injury, chronic pain, pelvic instability—

Male: penile injury/pelvic fracture → erectile dysfunction।
Female: vaginal scar/pelvic pain → painful intercourse
।

9. Hormonal and medical causes triggered or worsened after trauma

Trauma–এর পরে sleep disturbance, stress hormone increase, depression, weight change, chronic illness worsen হতে পারে।

Common medical causes:

  • Diabetes → nerve/vascular damage → ED/lubrication problem
  • Thyroid disease → libido change
  • High prolactin → low libido, ED, menstrual problem
  • Low testosterone → male low desire/ED
  • Menopause/lactation → vaginal dryness, painful sex

10. Medicine-induced sexual dysfunction

Common drugs:

  • SSRI: sertraline, paroxetine, fluoxetine
  • SNRI: venlafaxine, duloxetine
  • Antipsychotic: risperidone, haloperidol
  • Beta-blocker: propranolol, atenolol
  • Opioid painkillers
  • Finasteride

Libido

11. Relationship problem / lack of safety

Patient safe feel
Fear and avoidance বাড়ে।

Real example:
Partner

12. Substance use

Short-term disinhibition

TRAUMA = T-R-A-U-M-A

T = Trigger and PTSD
R = Relationship safety problem
A = Anxiety, avoidance, arousal problem
U = Unhealed sexual abuse/injury
M = Muscle spasm, medical disease, medicine side effect
A = Anger, shame, guilt, depression

Summary

Overview / Summary

মূল mechanism হলো PTSD hyperarousal, fear-conditioning, avoidance, dissociation, shame, depression/anxiety, pelvic floor spasm এবং কখনও organic genital/endocrine/vascular disease। Best treatment হলো trauma-focused psychotherapy + sex therapy + pelvic floor physiotherapy + cause-specific medicine।

Actionable Plan / Steps

  1. First visit: safety/IPV/recent assault risk assess
  2. Screen: PCL-5, PHQ-9, GAD-7, FSFI/IIEF-5/PEDT
  3. Basic labs: HbA1c, TSH, prolactin, testosterone in men, pregnancy/STI tests if indicated।
  4. Physical exam: trauma-informed, consent-based pelvic/genital exam।
  5. Start core treatment: trauma-focused CBT/EMDR referral।
  6. Symptom-specific treatment: ED হলে PDE5 inhibitor; nightmares হলে prazosin; PE হলে dapoxetine; dyspareunia হলে pelvic physiotherapy ± lidocaine/estrogen if indicated।
  7. Follow-up: 2–4 weeks for safety/side effects; 6–8 weeks for SSRI/SNRI response; treatment plan adjust।
