TC(830).(06).Psychiatry Case No 41. Persistent Depressive Disorder (PDD)

(Definition):

(Etiology/Pathophysiology):

Clinical Features:

  • নিদ্রার সমস্যা (insomnia/hypersomnia)

(Diagnosis):

Investigation:

    • CBC, TFT (থাইরয়েড ডিসঅর্ডার চেক)
    • Vitamin B12, Folate, Electrolyte
Hamilton Depression Rating Scale (HAM-D), PHQ-9

Management / Treatment

১. Pharmacological Treatment:

Drug, Purpose, Dose, Duration, Side Effects, Interactions

1. SSRIs (Selective Serotonin Reuptake Inhibitors)

  • Fluoxetine
  • Capsule 1.Prolert 20 mg AS A Fluoxetine Hydrochloride AS A Phenothiazine related drugs.
    • Dose: 20–40 mg/day
    • Duration: ≥6–12 months (maintenance)
    • Purpose: serotonin level বাড়িয়ে mood improve করে
    • Side Effects: nausea, insomnia, sexual dysfunction, weight change
    • Interaction: MAOIs, warfarin, NSAIDs (bleeding risk)
  • Sertraline
  • Tablet 2.Repose 50 Mg AS A Sertraline Hydrochloride AS A SSRIs & related anti-depressant drugs.
    • Dose: 50–200 mg/day
    • Purpose: mood uplift, anxiety co-morbidity treat
    • Side Effects: diarrhea, insomnia, sexual dysfunction
    • Interaction: MAOIs, pimozide, anticoagulants

2. SNRIs (Serotonin-Noradrenaline Reuptake Inhibitors)

  • Venlafaxine
  • Tablet 3.Venlax 75 mg AS A Venlafaxine Hydrochloride AS A (SNRI).
    • Dose: 75–225 mg/day
    • Purpose: refractory dysthymia, energy & motivation increase
    • Side Effects: nausea, hypertension, sweating
    • Interaction: MAOIs, SSRIs (serotonin syndrome risk)

    • Dose: 25–75 mg/day
    • Purpose: treatment-resistant PDD
    • Side Effects: dry mouth, constipation, sedation, cardiac arrhythmia
    • Interaction: MAOIs, anticholinergics, CNS depressants

4. Adjunct / Augmentation:

  • Bupropion
  • Tablet 5.Depnox-SR 150MG AS A Bupropion Hydrochloride AS A Atypical anti-depressant drugs.

    • Purpose: energy & motivation increase, sexual side effect কম
    • Side Effects: insomnia, seizure risk at high dose
    • Interaction: MAOIs, alcohol, other seizure-threshold lowering drugs

Important: Drug selection largely SSRIs first-line (safe & tolerable), SNRIs if partial response, TCA reserved for refractory cases.

(Overview / Summary):
