Psychiatry diseases: commonly known + commonly missed disorders
1. Mood disorders
Common: Major depressive disorder, bipolar disorder.
Less commonly known: Bipolar II, cyclothymia, persistent depressive disorder, premenstrual dysphoric disorder.
Depression শুধু sadness নয়—loss of interest, sleep/appetite change, fatigue, guilt, poor concentration, suicidal thought।
Red flag: suicidal ideation, psychotic depression, postpartum severe mood symptoms.
2. Anxiety and fear-related disorders
Common: Generalized anxiety disorder, panic disorder, phobia.
Less commonly known: Agoraphobia, selective mutism, separation anxiety disorder in adults.
Unexplained palpitation, chest tightness, dizziness, fear of dying—panic attack হতে পারে।
3. Obsessive–compulsive and related disorders
Common: OCD.
Less commonly known: Body dysmorphic disorder, hoarding disorder, trichotillomania, excoriation disorder.
4. Trauma and stressor-related disorders
Common: PTSD.
Less commonly known: Complex PTSD, adjustment disorder, prolonged grief disorder.
Trauma-এর পরে flashback, nightmare, avoidance, hyperarousal, emotional numbness। Complex PTSD-তে long-term trauma-এর কারণে emotional dysregulation, shame, relationship difficulty থাকে।
5. Dissociative disorders
Less commonly known but important: Dissociative amnesia, depersonalization/derealization disorder, dissociative identity disorder.
6. Psychotic disorders
Common: Schizophrenia.
Less commonly known: Schizoaffective disorder, brief psychotic disorder, delusional disorder, substance-induced psychosis.
Hallucination, delusion, disorganized speech/behaviour, negative symptoms। NICE psychosis guideline early recognition, antipsychotic treatment, CBT, family intervention, physical health monitoring গুরুত্ব দেয়।
7. Somatic symptom and health anxiety disorders
Less commonly known: Somatic symptom disorder, illness anxiety disorder, conversion/functional neurological symptom disorder.
Repeated medical symptoms থাকলেও investigations repeatedly normal হতে পারে। Patient “fake” নয়—symptom real, কিন্তু brain-body stress processing involved। Conversion disorder-এ weakness, seizure-like episode, sensory loss হতে পারে, but neurological pattern inconsistent।
8. Feeding and eating disorders
Commonly missed: Anorexia nervosa, bulimia nervosa, binge-eating disorder, ARFID.
Weight concern, body image distortion, binge eating, purging, laxative misuse। ARFID-এ body image issue না থাকলেও food avoidance, nutritional deficiency, weight loss থাকে।
9. Personality disorders
Often misunderstood: Borderline, antisocial, narcissistic, avoidant, obsessive-compulsive personality disorder.
Long-standing pattern of emotion, relationship, impulse, identity problem। Borderline personality disorder-এ fear of abandonment, unstable relationship, self-harm, emotional instability common।
10. Neurodevelopmental disorders
Commonly missed in adults: ADHD, autism spectrum disorder, learning disorder, tic disorder.
Adult ADHD শুধু “অমনোযোগ” নয়—poor planning, impulsivity, procrastination, emotional dysregulation, academic/work impairment। Autism adult female patients-এ masked হতে পারে: social exhaustion, sensory sensitivity, rigid routine।
11. Substance use and addictive disorders
Common: Alcohol, opioid, cannabis, stimulant use disorder.
Less commonly known: Sedative/hypnotic misuse, inhalant use disorder, gambling disorder.
Craving, tolerance, withdrawal, failed control, harm despite use। WHO mhGAP guideline mental, neurological and substance-use disorders-এর জন্য non-specialist settings-এ assessment and management framework দেয়।
12. Sleep–wake disorders with psychiatric overlap
Often missed: Insomnia disorder, hypersomnolence, narcolepsy, nightmare disorder, circadian rhythm disorder.
Chronic insomnia-তে CBT-I first-line; sedative dependence avoid করতে হয়।
Clinical shortcut: psychiatric case
- Mood: depression, mania/hypomania
- Anxiety: panic, worry, phobia
- Psychosis: hallucination, delusion
- Risk: suicide, self-harm, violence, neglect
- Substance: alcohol, cannabis, yaba/stimulant, sedative
- Medical causes: thyroid, seizure, delirium, infection, drugs
- Function: sleep, study/work, relationship, self-care
Basic investigations before labeling “psychiatric”
Depending on presentation:
CBC, ESR/CRP if indicated, RBS/HbA1c, electrolytes, calcium, renal function, liver function, TSH, vitamin B12/folate, pregnancy test, urine toxicology, ECG before some antipsychotics/TCAs, neuroimaging/EEG if seizure, focal neurological sign, atypical late-onset psychosis, delirium, head trauma.
Emergency referral immediately
Refer urgently if there is suicidal plan, violent behaviour, severe psychosis, catatonia, delirium, severe withdrawal, postpartum psychosis, refusal to eat/drink, severe self-neglect, or command hallucination.
Overview / Summary
Psychiatric diseases include mood, anxiety, OCD-related, trauma-related, psychotic, somatic symptom, eating, personality, neurodevelopmental, substance-use, sleep-wake and neurocognitive disorders. Many commonly missed conditions are bipolar II, body dysmorphic disorder, hoarding disorder, complex PTSD, dissociative disorder, illness anxiety disorder, conversion disorder, adult ADHD, autism in adults, binge-eating disorder, and borderline personality disorder.
Actionable Plan / Steps
For practical clinical assessment:
First screen mood, anxiety, psychosis, substance use, suicide risk and functioning. Then exclude medical/substance causes. Use ICD-11/DSM-5-TR criteria, assess severity and risk, start psychoeducation and evidence-based first-line treatment, and refer complex/high-risk cases to psychiatry.
