Hello I'm Raiyyan Tahsan
Brand Model and Executive

TC(760).(35).Endocrinology Case 42.

১) Diabetic Neuropathy

2) Post menopausal Syndrome

It might be due to Post menopausal Syndrome.

১) Breast Cancer

2) Endometrial Cancer

HRT এর পরিবর্তে Non hormonal treatment হিসেবে New option Tab. Fezotan 45 mg দেওয়া যেতে পারে,যাতে Breast cancer, Endometrial cancer এর ঝুঁকি নাই বললেই চলে। Once daily for 6- 12 month

Tablet 1.Fezotan 45 mg

Indications:-

Fezotan tablet is indicated for the treatment of moderate to severe vasomotor symptoms due to menopause.

Pharmacology:-

Fezolinetant is a neurokinin 3 (NK3) receptor antagonist that blocks neurokinin B (NKB) binding on the kisspeptin/neurokinin B/dynorphin (KNDy) neuron to modulate neuronal activity in the thermoregulatory center. Fezolinetant has high affinity for the NK3 receptor (Ki value of 19.9 to 22.1 nmol/L), which is more than 450-fold higher than binding affinity to NK1 or NK2 receptors.

Treatment with fezolinetant did not show any clear trends in sex hormones measured (follicle-stimulating hormone, testosterone, estrogen, and dehydroepiandrosterone sulfate) in menopausal women. Transient decrease of luteinizing hormone (LH) levels was observed at peak concentrations of fezolinetant.

Dosage & Administration:-

Take a single 45 mg Fezolinetant tablet orally once daily with or without food. Take Fezolinetant with liquids and swallow whole. Do not cut, crush, or chew tablets. Administer Fezolinetant orally at about the same time each day. If a dose of Fezolinetant is missed or not taken at the usual time, administer the missed dose as soon as possible, unless there is less than 12 hours before the next scheduled dose. Return to the regular schedule the following day.

Pediatric Use: The efficacy and safety of Fezolinetant in individuals less than 18 years of age have not been established.

Geriatric Use: There have not been sufficient numbers of geriatric women involved in clinical trials utilizing Fezolinetant to determine whether those over 65 years of age differ from younger women in their response to Fezolinetant.

Side Effects:-

Common side efects of Fezotan include:

  • stomach (abdominal) pain
  • diarrhea
  • difculty sleeping
  • back pain
  • hot fashes or hot fushes

Stop Fezotan if you have the following signs or symptoms of liver problems:

  • nausea
  • vomiting
  • itching
  • yellowing of the eyes or skin (jaundice)
  • pale feces
  • dark urine
  • pain in the right upper stomach (abdomen)

Burning sensation in a 40+ diabetic woman — important differentials

  1. Diabetic Peripheral Neuropathy (DPN)
  2. Perimenopausal/Postmenopausal vasomotor syndrome

১) Diabetic Neuropathy — Clinical Clues

Typical symptoms

  • Burning feet
  • Tingling / numbness
  • “Pins and needles”
  • Electric shock–like pain
  • Night worsening
  • Glove and stocking distribution

Distribution

সবচেয়ে characteristic:

  • Distal symmetrical pattern
  • Sole, toes, feet → gradually upward

Examination findings

  • Reduced vibration sense
  • Reduced ankle jerk
  • Decreased pinprick sensation
  • Monofilament abnormal

Associated history

  • Long-standing diabetes
  • Poor glycemic control
  • Retinopathy / nephropathy

Treatment

First-line options:

  • Pregabalin
  • Gabapentin
  • Duloxetine (Suggested)
  • Amitriptyline (Tryptin)

Example

Pregabalin 75 mg HS → titrate
বা
Amitriptyline 10–25 mg HS

২) Perimenopausal / Postmenopausal Syndrome

এখানে “burning” usually neuropathic burning না।

  • Hot flush
  • Heat sensation
  • Facial flushing
  • Scalp warmth
  • Upper body warmth
  • Sweating

Typical age

  • Usually >45 years
  • Menstrual irregularity

Associated symptoms

খুব important:

  • Amenorrhea / irregular cycle
  • Hot flush
  • Night sweats
  • Mood change
  • Vaginal dryness
  • Sleep disturbance
  • Reduced libido

Distribution

More commonly:

  • Face
  • Neck
  • Chest
  • Scalp
  • Upper torso

এটা neuropathy pattern না।

Important Correction About HRT

এটা partially true but oversimplified.

Actually:

Endometrial cancer risk ↑

  • Mainly with unopposed estrogen therapy
  • Especially if uterus intact

তাই uterus থাকলে:

  • Estrogen + progesterone combination

Breast cancer risk

  • Long-term combined HRT use এ risk slightly increases
  • Short-term appropriate HRT generally acceptable after risk assessment

Before Starting HRT — Evaluation

আপনার Mammogram + TVS point clinically reasonable।

Common baseline assessment:

  • Breast examination
  • Mammography (age/risk dependent)
  • TVS if abnormal bleeding
  • BP
  • BMI
  • VTE risk
  • Cardiovascular risk

সব patient এ routine TVS mandatory না unless:

  • Abnormal uterine bleeding
  • Endometrial concern

Important Correction About Fezolinetant

Fezolinetant

  • Neurokinin-3 receptor antagonist
  • Used for moderate–severe vasomotor symptoms (hot flush)

Advantage

  • Non-hormonal
  • Does NOT stimulate endometrium like estrogen
  • Breast cancer risk comparatively lower than HRT

But important:

“ঝুঁকি নাই বললেই চলে” — এভাবে বলা ঠিক না।
No drug is completely risk-free.

Liver monitoring may be needed.

Clinically safest approach

If burning is:

Distal feet dominant + numbness

→ Think diabetic neuropathy first

If:

Hot flush + sweating + menstrual irregularity

→ Think perimenopause/menopause
