Pseudophakia কী?
👉 Definition (Core):
Pseudophakia = চোখে natural lens এর জায়গায় artificial intraocular lens (IOL) বসানো থাকা

👉 Cataract অপারেশনের সময়:
- চোখের ঘোলা natural lens ফেলে দেওয়া হয়
- তার জায়গায় plastic artificial lens (IOL) বসানো হয়
➡️ এই অবস্থাকে বলা হয় Pseudophakia
PCO কী? (Posterior Capsular Opacification)
🔍 Definition (Core)
👉 PCO = Cataract অপারেশনের পর lens এর পেছনের capsule ঘোলা হয়ে যাওয়া
➡️ এটাকে বলা হয়:
“Secondary cataract”
🧠 সহজভাবে বুঝো
👉 Cataract surgery তে:
- সামনের lens remove করা হয়
- কিন্তু posterior capsule রেখে দেওয়া হয় (IOL বসানোর জন্য)
👉 পরে ওই capsule:
- ধীরে ধীরে opaque / fibrosed হয়ে যায়
➡️ ফলে আবার ঝাপসা দেখা শুরু হয়
PCO (Posterior Capsular Opacification) এর Symptoms (বাংলায়)
👉 খুব সহজ ও ক্লিনিক্যালি যেগুলো দেখা যায়—

- চোখে ঝাপসা দেখা (Blurred vision)
👉 ধীরে ধীরে দৃষ্টি কমে যায়
- আলোতে ঝলক / গ্লেয়ার (Glare)
👉 আলো দেখলে চোখে ঝিলমিল লাগে
👉 রাতে বেশি সমস্যা
- ডাবল দেখা / ছায়া দেখা (Monocular diplopia)
👉 এক চোখে দুইটা ইমেজ দেখা
- আলোতে সংবেদনশীলতা (Photophobia)
👉 আলো সহ্য করতে কষ্ট হয়
- ভিশন আবার cataract এর মতো হয়ে যাওয়া
👉 রোগী ভাবে আবার cataract হয়েছে
PCO (Posterior Capsular Opacification) এর Cause (বাংলায়)
🔍 মূল কারণ (Core Cause)
👉 Lens epithelial cell (LEC) বেঁচে থাকা
➡️ Cataract অপারেশনের পর কিছু কোষ থেকে যায়
➡️ এই কোষগুলো পরে বাড়তে থাকে

1. Residual lens epithelial cell proliferation
👉 অপারেশনের পর:
- capsule এ কিছু cell থেকে যায়
- এগুলো divide করে বাড়ে
2. Cell migration
👉 এই কোষগুলো:
- posterior capsule এর উপর চলে যায়
➡️ vision axis ঢেকে ফেলে
3. Fibrosis (দাগ পড়ে যাওয়া)
👉 capsule:
- শক্ত ও ঘোলা হয়ে যায়
➡️ light pass করতে পারে না

👉 cell গুলো:
- গোলাকার cluster তৈরি করে
➡️ “pearls” এর মতো দেখায়

📌 কেন করা হয়:
- retina ঠিক আছে কিনা দেখা
- optic nerve damage আছে কিনা
- macula সমস্যা আছে কিনা
2. Visual Field Test (Perimetry)
👉 চোখের দেখার পরিধি পরীক্ষা
📌 কেন করা হয়:
- glaucoma আছে কিনা
- optic nerve কাজ করছে কিনা
- vision loss pattern বোঝা
