The Whisper in the Chest”

— Episode 3: Childhood Pneumonia

⸻

Scene 1: The Visual Clues

আমি বাচ্চাটার দিকে তাকালাম।

• Fast breathing

• Nasal flaring

• Lower chest wall indrawing

• Noisy breathing

→ Clue 1: Tachypnea + Chest indrawing = WHO-defined Pneumonia

⸻

Scene 2: Age-Specific Respiratory Rate (WHO Criteria)

Age Tachypnea = Respiratory Rate >

< 2 months 60 breaths/min

2–12 months 50 breaths/min

1–5 years 40 breaths/min

→ আমি বাচ্চাটার শ্বাস গুনলাম—প্রতি মিনিটে 58 বার। বয়স 2 বছর।

→ Tachypnea confirmed

⸻

Scene 3: Auscultation – Sound of Struggle

• Crepitations in right lower zone

• Slightly reduced air entry

→ Consolidation suspected

⸻

Scene 4: Etiology Clues

“কোনও বাচ্চার সাথে খেলা করেছিল যাদের সর্দি-কাশি ছিল?”

“জি, ওর খালার ছেলের সর্দি ছিল।”

→ Contact with viral URI → Possible viral or secondary bacterial pneumonia

No measles, no choking, no travel.

⸻

Common Causes by Age:

Age Likely Organism

<1 month GBS, E. coli

1–3 months RSV, Chlamydia, Strep pneumoniae

4 months – 5 years RSV, Influenza, S. pneumoniae, H. influenzae

>5 years Mycoplasma pneumoniae, S. pneumoniae

⸻

Investigations:

• Clinical diagnosis usually enough

• If severe: CBC, CXR, pulse oximetry, blood culture

→ CXR: Right lower lobe opacity → confirms lobar pneumonia

⸻

Management (based on severity):

Severity Signs Treatment

No pneumonia Only cough, no tachypnea Home care + supportive

Pneumonia (non-severe) Cough + tachypnea Oral amoxicillin 5 days

Severe pneumonia Chest indrawing Admit + IV antibiotics (ampicillin/gentamicin)

Very severe pneumonia Cyanosis, grunting, inability to drink, convulsion Urgent referral + IV ceftriaxone

⸻

Supportive Care:

• Fever: Paracetamol

• Nutrition: Breastfeeding / adequate feeding

• Oxygen if SpO₂ < 90%

• Fluids: Prevent dehydration

⸻

Prevention:

• Vaccines: PCV, Hib, Measles

• Handwashing

• Avoid smoke exposure

• Nutrition and breastfeeding

⸻

Key Clue Summary – Childhood Pneumonia:

Clue Interpretation

Fast breathing Most sensitive early sign

Chest indrawing Sign of severe pneumonia

Grunting, cyanosis Danger signs → hospitalize immediately

Crepitations on auscultation Suggests consolidation
