হঠাৎ ডান দিকের তলপেটে ব্যথা
রোগীর নাম: শামীমা আক্তার
বয়স: ২১ বছর
মা সমস্যা কী?: হঠাৎ ডান তলপেটে তীব্র ব্যথা
পুরে ডিটেইলস শুনলাম:
ব্যথা শুরু হয়েছে সকাল ৮টায়, ধীরে ধীরে বেড়েছে
সাথে হালকা জ্বর, বমি বমি ভাব
শেষ মাসিক হয়েছে ৪০ দিন আগে
কোনো previous surgery নেই
জিনিসপত্র নাড়াচাড়া করলে ব্যথা বাড়ে
Examination করলাম সাথে সাথে
Findings পেলাম:
-BP: 100/60 mmHg
-Pulse: 110 bpm
-Abdomen এক্সাম করলাম: right lower quadrant এ পেইন
tenderness:rebound tenderness present
-Per vaginal exam: right adnexal tenderness
-Urine pregnancy test: Positive
Provisional Diagnosis তাহলে কী হতে পারে:
এটা Ruptured ectopic pregnancy
Concept-based Explanation শেখার চেষ্টা করি:
1. এটা আমার কাছে Ectopic কেন মনে হলো?
-Delayed menstruation = missed period
-Sudden onset unilateral lower abdominal pain
-সাথে Positive pregnancy test
-Signs of internal bleeding (hypotension, tachycardia)
-রোগীর প্রেগন্যান্সি সিম্পটমপস ও নাই
2. তাহলে কিভাবে rupture হলো?
• Fertilized ovum fallopian tube-এ implant করলে সেখানে grow করতে পারে না → rupture হয়ে যায় → internal bleeding হয় → pain & shock চলে যেতে পারে
Emergency Management Plan:
Resuscitation করবো as soon as possible :
-IV line with normal saline দিতে পারি
-ব্লাড Group and crossmatching করে রাখবো। 2–4 units blood হাতে রাখবো।
-Urgent USG করতে দিবো (Transvaginal preferred) – no intrauterine pregnancy, adnexal mass, free fluid in pouch of Douglas) এগুলো আসবে রেজাল্ট
•সাথে নিয়মিত ভাইটালস চেক করতে হবে।শকে গেলো কিনদ চেকে রাখবো
Definitive Treatment তাহলে কী?
-Emergency laparotomy/laparoscopy
-Removal of ruptured tube (salpingectomy if severe bleeding)
-Preserve tube if possible for future fertility
Post-op Advice:
Future pregnancy প্লানিং করে একজন obs এন্ড gynae specialist এর সুপারভিশনে থাকবে।
অবশ্যই সাইকোলজিক্যাল সাপোর্ট দিবেন।
তাহলে কী শিখলাম:
Lower abdominal pain in reproductive age female = always rule out ectopic
Positive preg test + abdominal pain + no intrauterine pregnancy = high suspicion
Delayed diagnosis = life-threatening hemorrhage হতে পারে
