এবার আপনি কিছু একটা ভাবলেন, ফ্যানার পাশাপাশি প্রসাবে কী বেশি গন্ধ? রোগীর উত্তর হ্যা।
আপনি একটা Diagnosis এর কথা চিন্তা করলেন এবার – Pneumaturia?
মানে প্রসাবের সাথে Air / Gas যাচ্ছে। এখন Abdomen এর ভেতরে এমন Gas থাকে Intestine এ।
তার মানে এই রোগীর Bladder এর সাথে Intestine এর কোন একটা Part ( Commonly – Colon) এর Fistula Form হয়েছে কোন কারণে, যেখান থেকে Gas টা আসতেছে এবং রোগী বার বার UTI নিয়ে আসতেছে।
আপনি আপনার ধারণাটা Confirm করার জন্য কোন Test টা করতে দিতে পারেন রোগীকে?
Answer Is:- এই Diagnosis confirm করার জন্য সবচেয়ে উপযুক্ত প্রাথমিক test হবে:
CT Scan of Abdomen and Pelvis with Oral and IV Contrast
এটা দেখাতে পারে Fistulous tract,
Bladder wall thickening,
Adjacent bowel inflammation,
Gas within the bladder (pneumaturia) ইত্যাদি।
এটা অনেক বেশি sensitive এবং non-invasive।
Alternatives বা Other helpful tests:
Cystography (Retrograde Cystogram)
Colonoscopy (Sometimes identifies the fistula opening)
Cystoscopy (Bladder ভেতর থেকে দেখতে পারে fistula opening)
কিন্তু প্রথম টেস্ট হিসেবে CT Scan ই হলো Best choice.(আমার মতে)
