Loop diuretics যেমন : furosemide,torsemide এগুলো শুধুমাত্র আমরা resistant hypertension এ ইউজ করবো।
কারণ তখন আমার কাছে অপশন কম।
যেহেতু resistant hypertension মানে অন্য 1st,2nd ,3rd line drug এ কাজ হয়নাই।আমরা অনেকে মনে করি diuretics মানেই anti hypertensive তা ভূল।
thiazide diuretics সচরাচর htn management এ ইউজ হয়।
মনে রাখি:
Loop diuretics গুলো ২৫% পর্যন্ত filtrated Na+ কে excretion করতে পারে।সেজন্য অনেক ব্লাড প্রেশার কমায়,সাথে বডিতে পানি জমেছে এরকম কন্ডিশনে কাজে আসে।
আরো কিছু কন্ডিশন :
Htn crisis with edema
Htn crisis with acute pulmonary edema
Htn crisis with live chirrhosis/heart failure
এগুলোতেও চয়েজ হচ্ছে loop diuretics. মনে রাখতে হবে long standing Htn এ আমরা loop diuretics use করিনা।HTN crisis +edema related disease ছাড়া আর অন্য কোনো ক্ষেত্রে Htn management এ loop diuretics এর তেমন ইউজ নেই.
মূলত:
Cld with ascites
pulmonary edema
generalised edema(nephrotic syndrome)
heart failure
Chronic kidney disease
আবার hyperkalemia তেও এটা ইউজ করি,বডির পটাশিয়াম লোড কমাতে।
এগুলোতে loop diuretics বেশি ইউজ হয়
