LAZARUS REFLEX.
Lazarus শব্দটি এসেছে হিব্রু শব্দ Elazar থেকে।যার অর্থ With the help of God.
Gospel of John বলছে যীশু তার একজন সাধু সহচারীকে, যার নাম ছিল Lazarus, ঈশ্বরের সাহায্যে মৃত থেকে জীবিত করেছিলেন।
সূরা আল মায়্যিদাহ তে আল্লাহ তা’লা এর উল্লেখ করেছেন।
চারজনকে একত্রে মৃত অবস্থা থেকে জীবিত করা
ঈসা (আ.) এর একটি মোজেজা ছিল।
মুলত এই ঘটনা থেকে ইন্সপায়ারড হয়ে,মৃত ব্যক্তির হঠাৎ নড়াচড়া/প্রাণ সঞ্চারের যে লক্ষণ দেখা যায় তাকে বলছে
Lazarus Reflex.
Mechanism টা হচ্ছে
মূলত Cardio Respiratory Failure এর পর তৎক্ষণাৎ একজনকে CPR দেওয়া শুরু করা হয়।
মানুষের Cortical Neurons অক্সিজেনের অভাবে বেশিক্ষণ সক্রিয় থাকবে না সিউর।
এখন CPR চলাকালীন যত সময় যাবে functional Neurons এর নিষ্ক্রিয়তা তত বাড়তে থাকবে।এভাবে ৫-১০ মিনিট CPR চলার পর হয়তবা Cardio Respiratory Function regain হইতে পারে।কিন্তু ততক্ষণে Cortical and brainstem death হয়ে গেছে।
এরকম CPR/Ventilation এ থাকা অবস্থায় রোগীর Spinal Cord Neurons যদি অক্সিজেন পেয়ে থাকে,তবে Spinal Shock resolution হইতে পারে।আর তা হলে,
Spinal Cord Neurons will work without Superior Control.
এক্ষেত্রে ১০০% Spinal Cord Neurons এর Hyperexcitability দেখা যাবে।
এ কারনে,
Cortical /Central pain stimuli দিলে
যেমন:
Supra orbital ridges pain stimuli
Trapezeus squeeze
Sternal Rub/pressure
করলে দেখা যাবে মৃত ব্যক্তির কোনো movement response to pain stimuli নাই।
কিন্তু Spinal Nerve stimuli দিলে
যেমন:
Neck Left/Right Rotation করলে
Rapid Flexion of Elbow Joint হবে।অর্থাৎ,
মৃত ব্যক্তি জীবিতদের মত দুই হাত ক্রস করে ভাঁজ করবে।
এটাই Lazarus Reflex.
এখন ওয়ার্ডে রোগীকে মৃত ঘোষণা করার পর যদি কোনো কাকতালে Lazarus Reflex হয়,অথবা
ইন্টেনসিভ কেয়ারে ভেন্টিলেশনে থাকা Brain dead রোগীকে মৃত ঘোষণা করার পর যদি Lazarus Reflex হয় তবে বাংলাদেশের প্রেক্ষিতে উত্তম মধ্যম পাওয়ার সমূহ সম্ভাবনা আছে।

Cortical death চেনার উপায়:
No pulse: Feel for a pulse in the carotid artery (neck).
No heart sounds: Listen for heart sounds using a stethoscope.
No breathing: Observe for any respiratory effort or breath sounds.
Pupil response: Check if the pupils are dilated and fixed (do not react to light).
Unresponsiveness: Assess if the patient shows any response to stimuli.
Flat ECG.
Brainstem death চেনার উপায়
Unresponsiveness to stimuli: pinching or needle pricks, showing no eye opening, verbal response, or limb movement.
Absent pupillary light reflex.
No corneal blink reflex: Gently touching the cornea with a cotton swab should not elicit a blink response.
Absent oculocephalic reflex (doll’s eyes): When the head is turned quickly to one side, the eyes should not move in the opposite direction.
No oculovestibular reflex (cold caloric test): Instilling cold water into the ear canal should not produce eye movement.
No gag or cough reflex: Stimulating with a tongue depressor should not cause a gagging or coughing response.
Apnea test: After confirming the absence of other brainstem reflexes, the patient is temporarily disconnected from the ventilator to observe for spontaneous breathing attempts no breathing will be seen.
