bilateral renal artery stenosis এ কেনো ACEi
দিবেন না?
Bilateral renal artery stenosis এর ক্ষেত্রে Both the kidneys এর Blood supply কমে যায়। তখন Renin – angiotensin – aldosteron system (RAAS) activate হয়ে Efferent arteriolar vasoconatriction করে GFR maintain করে।
এটা Normal physiological response. কিন্তু ঐ রোগীকে ACE inhibitor দিলে RAAS activation হতে পারবে না। As a result, GFR আরো Deteriorate করে ।
ACEI এর একটা কাজ হল Angiotensin 2 কে inhibit করে efferent arteriole কে constrict হতে দেয় না।
efferent arteriole dilatation এর কারণে GFR কমে BP কমে যায়।
কিন্তু যাদের Renal Artery Stenosis থাকে, তাদের তো এমনিতেই Renal Blood Flow কম, তাই GFR ও কম।
আবার এর সাথে ACEI দিয়ে দিলে GFR আরো কমে গিয়ে Sudden AKI develop করে।
তাই, Bilateral renal artery stenosis এর রোগীকে ACE inhibitor দেওয়া যাবে না ।
