Trop-I বাড়া মানেই MI নয়, MI ছাড়াও আরো ১০ টা কারণে Trop-I বাড়তে পারে.

T=Tachyarrhythmia, Trauma

S= Sub arachnoid Hg, stroke, septic shock

আবারো =Aortic dissection

এসেছেন= ESRD (end stage Renal disease)

তাই ECG finding negative হলে Typical chest pain না থাকলে শুধু Trop-I দেখে MI বলা যাবেনা।

Typical chest pain এর বৈশিষ্ট্য ৩ টি.

1. Squeezing pain radiate to jaw, neck, shoulder

2. হাঁটলে বা exercise করলে ব্যাথা বাড়বে,

3. GTN দিলে ৫ মিনিটের মধ্যে ব্যাথা কমে যাবে,

What is GTN Drug in Chest Pain?

Glyceryl trinitrate, or GTN, is a type of medicine called a nitrate. It’s used to prevent and treat chest pain caused by Angina.

It can help stop chest pain if an angina attack has already started. It can also help to prevent the chest pain starting.

It comes as tablets or as a spray that you put under your tongue. It also comes as patches that you put on your skin.

Nitroglycerin is used for the treatment of angina, Acute Myocardial Infruction, severe hypertension, and acute Coronary Spasms It may be administered Intervenously, as a Sublingual spray, or as a patch applied to skin.

Tablet 1.GTN-SR 2.6mg (AS A Nitroglycerin As a Nitrates: Coronary vasodilators)

Indications of Tablet .GTN-SR 2.6mg (AS A Nitroglycerin As a Nitrates: Coronary vasodilators):-

GTN SR is indicated for the prophylaxis of angina pectoris. The onset of action is not sufficiently rapid for this form to be useful in aborting an acute anginal episode.

Pharmacology of Tablet .GTN-SR 2.6mg (AS A Nitroglycerin As a Nitrates: Coronary vasodilators):-

Nitroglycerin causes relaxation of vascular smooth muscle, producing a vasodilator effect on both peripheral arteries and veins. Dilation of veins promote peripheral pooling of blood and decreases venous return to the heart, thereby reducing left ventricular end-diastolic pressure (preload) and relaxation of arteries reduce systemic vascular resistance and arterial pressure (afterload).

Side Effects of Tablet .GTN-SR 2.6mg (AS A Nitroglycerin As a Nitrates: Coronary vasodilators):-

Headache may occur at the onset of treatment but will usually subside after a few days. If the headache persists dosage should be decreased. Other side effects include tachycardia, postural hypotension and syncope, cyanosis and methaemoglobinaemia.

এই 3 টা বৈশিষ্ট্য থাকলে Typical chest pain mostly due to cardiac cause.

যদি এখান থেকে ২ টা বৈশিষ্ট্য থাকে তাহলে atypical chest pain,

আর যদি এখানের ১ টা থাকে, বা কোনটাই না থাকে, তাহলে pain এর কারণ হচ্ছে non cardiac.
