- Bronchial asthma: নির্বাচিত পরিস্থিতিতে হাঁপানির প্রদাহ নিয়ন্ত্রণে।

- Autoimmune diseases: কিছু অটোইমিউন রোগ।
- Selected dermatological diseases: কিছু গুরুতর ত্বকের প্রদাহজনিত রোগ।

কোন উপাদান কী কাজ করে?
Econazole Nitrate 1%
Antifungal agent।
ছত্রাকের কোষঝিল্লির ergosterol synthesis বাধাগ্রস্ত করে fungal growth নিয়ন্ত্রণ করে।
Triamcinolone Acetonide 0.1%
Topical corticosteroid।
ত্বকের প্রদাহ, লালভাব ও চুলকানি কমাতে সাহায্য করে।
কোন রোগে ব্যবহার হয়?
- Eczematous mycoses — ছত্রাকজনিত সংক্রমণের সঙ্গে একজিমা।
- Tinea corporis — শরীরের দাদ।
- Tinea cruris — কুঁচকির দাদ।
- Tinea pedis — পায়ের ছত্রাক সংক্রমণ।
- Inflammatory intertrigo — ত্বকের ভাঁজের প্রদাহ।
- নির্বাচিত অন্যান্য প্রদাহজনিত ত্বকের রোগ।

কিন্তু MedEx-এর সাধারণ product instructions-এ Pevitin cream দিনে সর্বোচ্চ দুইবার ব্যবহারের কথা বলা হয়েছে এবং সাধারণত দুই সপ্তাহের বেশি ব্যবহারের পরামর্শ নেই।
এটি মূলত ত্বকের cream, ear drop নয়।
কানের বাইরের চামড়ায় লাগাতে বলেছেন, নাকি ear canal-এর ভেতরে—ENT চিকিৎসকের কাছ থেকে নিশ্চিত হওয়া প্রয়োজন।

Levofloxacin ব্যাকটেরিয়ার DNA gyrase ও topoisomerase IV enzyme বাধাগ্রস্ত করে।

Definition
Otitis Externa হলো External Auditory Canal-এর ত্বকের প্রদাহ, যা সংক্রামক বা অসংক্রামক কারণে হতে পারে।
Etiology / Pathophysiology
সম্ভাব্য কারণগুলোর মধ্যে রয়েছে:
- Bacterial infection — বিশেষ করে Pseudomonas aeruginosa এবং Staphylococcus aureus।
- Fungal infection — যেমন Aspergillus বা Candida।
- Eczema বা allergic/contact dermatitis।
- কানে পানি জমা, ত্বকের ক্ষতি বা cotton bud ব্যবহারের কারণে irritation।
Clinical Features
সাধারণভাবে দেখা যেতে পারে:
- কানে ব্যথা।
- চুলকানি।
- Ear canal redness বা swelling।
- কান থেকে discharge।
- কান বন্ধ লাগা।
- সাময়িক hearing reduction।
এগুলো এই রোগীর প্রমাণিত উপসর্গ নয়। Otitis externa-এর প্রচলিত clinical features।

Primary Investigation: Otoscopic Examination
এই রোগীর ক্ষেত্রে সবচেয়ে গুরুত্বপূর্ণ পরীক্ষা।
এর মাধ্যমে দেখতে হবে:
- External auditory canal-এ inflammation আছে কি না।
- Canal swelling বা discharge আছে কি না।
- Fungal debris আছে কি না।
- Tympanic membrane intact কি না।
- Middle ear disease-এর প্রমাণ আছে কি না।

Trevox 1.5% একটি ear-drop formulation।
অন্য কিছু Levofloxacin 1.5% ব্র্যান্ড ophthalmic solution হিসেবে তালিকাভুক্ত। এগুলোকে চিকিৎসক বা ফার্মাসিস্টের যাচাই ছাড়া Trevox ear drop-এর সরাসরি বিকল্প বিবেচনা করা উচিত নয়।
৮. Bangladesh Doctors Commonly Used Drugs for Ear Canal Disease
এটি এই রোগীর জন্য নতুন প্রেসক্রিপশন নয়। Clinical diagnosis অনুযায়ী চিকিৎসকেরা যে ধরনের ওষুধ বিবেচনা করেন, তার উদাহরণ।
1. Bacterial Otitis Externa
Topical antibacterial therapy
উদাহরণ: Levofloxacin-containing Trevox ear drop।
Purpose: সংবেদনশীল ব্যাকটেরিয়ার সংক্রমণ নিয়ন্ত্রণ করা।
2. Confirmed Otomycosis
Clotrimazole 1% Ear Drop
বাংলাদেশে MedEx-listed brands:
- Clarizol
- Fungin
- Fungizol
Purpose: কানের ছত্রাকজনিত সংক্রমণের চিকিৎসা।
তবে suspected fungal infection থাকলেই আগের antibiotic-এর সঙ্গে নিজে থেকে antifungal যোগ করা উচিত নয়। আগে otoscopic findings যাচাই করতে হবে।
3. Eczematous Otitis Externa
Topical anti-inflammatory preparations
Purpose: কানের ত্বকের প্রদাহ ও চুলকানি নিয়ন্ত্রণ।
একই সঙ্গে fungal infection থাকলে steroid ব্যবহারে সতর্কতা প্রয়োজন।
4. Severe Ear Pain
Analgesic treatment
উদাহরণ: Paracetamol বা উপযুক্ত ক্ষেত্রে NSAID।
Purpose: ব্যথা নিয়ন্ত্রণ।
Analgesic প্রয়োজন ও নির্বাচন রোগীর ব্যথার মাত্রা, contraindications এবং অন্যান্য medication-এর ওপর নির্ভর করবে।
Guideline note: uncomplicated acute otitis externa-তে topical therapy মূল চিকিৎসা। Oral antibiotic সাধারণত প্রয়োজন হয় না, যদি না সংক্রমণ ear canal-এর বাইরে ছড়ায় বা বিশেষ ঝুঁকি থাকে।

Concern 2: Pevitin Cream Frequency and Site
প্রেসক্রিপশনের frequency ও MedEx product instructions-এর মধ্যে পার্থক্য রয়েছে।
কানের ভেতরে নাকি বাইরের ত্বকে cream লাগানোর নির্দেশ, সেটিও স্পষ্ট করতে হবে।

Concern 3: Duration of Ear Drop Treatment
Trevox দুই সপ্তাহ দেওয়া হয়েছে।
সাধারণ uncomplicated otitis externa-তে প্রচলিত topical treatment course প্রায় 7–10 দিন। তবে রোগের ধরন ও চিকিৎসকের পরীক্ষার ফল অনুযায়ী সময় ভিন্ন হতে পারে।
Priority: Moderate
দীর্ঘমেয়াদি বা জটিল সংক্রমণের কারণ আছে কি না, তা যাচাই করা যুক্তিযুক্ত।

বাংলা: কানের বাইরের নালির সম্ভাব্য প্রদাহ বা সংক্রমণ, যার সঙ্গে একজিমাজনিত প্রদাহ থাকতে পারে।
Confidence: 0.60
এই confidence রোগ নিশ্চিত হওয়ার সম্ভাবনার পরিসংখ্যান নয়; উপলব্ধ তথ্যের ভিত্তিতে assessment-এর সীমাবদ্ধতা নির্দেশ করে।
প্রেসক্রিপশনে দেওয়া চিকিৎসা suspected bacterial ear disease এবং inflammatory skin disease—দুটোর সঙ্গেই সামঞ্জস্যপূর্ণ।
তবে কানের পর্দা, ear canal এবং clinical signs না দেখে এটিকে confirmed diagnosis লেখা উচিত নয়।
Prescription-এ Dx লিখতে হলে
রোগীর লক্ষণ ও পরীক্ষায় সমর্থন পাওয়া গেলে:

কিন্তু বর্তমানে শুধু এই প্রেসক্রিপশনের ভিত্তিতে দ্বিতীয় diagnosis নিশ্চিত হিসেবে লেখা যুক্তিসংগত নয়।
১১. Actionable Clinical Plan
- প্রথমে Prescribing ENT Specialist-এর সঙ্গে Xalcort strength নিশ্চিত করুন। একই সঙ্গে Pevitin কোথায় এবং কতবার ব্যবহার করতে হবে এবং Trevox কোন কানে দিতে হবে, তা যাচাই করুন।
- Clinical history নিন: কানে ব্যথা, চুলকানি, discharge, hearing loss, symptom duration, পূর্ববর্তী ear drop ব্যবহার, পানি ঢোকার ইতিহাস, diabetes ও allergy history।
- Bilateral otoscopy করুন: External canal, discharge, fungal debris এবং tympanic membrane মূল্যায়ন করুন।
- Diagnosis অনুযায়ী চিকিৎসা পর্যালোচনা করুন: fungal, bacterial ও eczematous disease-এর management এক নয়। 48–72 ঘণ্টায় উন্নতি না হলে আগেই reassessment প্রয়োজন।
- Red flags থাকলে urgent ENT review করুন: তীব্র বা ক্রমবর্ধমান ব্যথা, কান বা মুখে ছড়িয়ে পড়া ফোলা, জ্বরসহ অসুস্থতা, facial weakness বা উল্লেখযোগ্য hearing loss।
কানে cotton bud ব্যবহার, চুলকানো এবং অপ্রয়োজনীয় পানি ঢোকানো এড়িয়ে চলা উচিত।

৩. ENT চিকিৎসক পরীক্ষা করে কী বলেছেন—কানে ফাঙ্গাস, ইনফেকশন, একজিমা, নাকি অন্য কোনো সমস্যা?
এই তিনটি তথ্য এবং otoscopy findings পাওয়া গেলে provisional diagnosis আরও নির্দিষ্টভাবে মূল্যায়ন করা সম্ভব হবে।
