অর্থাৎ, রোগীর প্রধান সমস্যা ছিল কোমরের নিচের অংশে ব্যথা। পাশে L4–L5 level-ও লেখা আছে বলে মনে হচ্ছে। তবে শুধু এই prescription থেকে disc prolapse, radiculopathy বা অন্য specific diagnosis নিশ্চিত করা যাবে না।

কোমরের pain এবং inflammatory component কমানোর জন্য দেওয়া হয়েছে। Ibuprofen musculoskeletal pain এবং backache-এ ব্যবহৃত হয়।

2. Relentus 2 mg
Generic name: Tizanidine Hydrochloride 2 mg
Therapeutic category:
Skeletal muscle relaxant
Pharmacological class:
Centrally acting α2-adrenergic agonist
Use / Purpose in this patient:
Low back pain-এর সঙ্গে যদি lumbar muscle spasm, tightness বা painful paraspinal contraction থাকে, সেটি কমানোর জন্য। Relentus specifically cervical/lumbar syndrome-associated painful muscle spasm-এর জন্য indicated।

Gabapentinoid; α2δ subunit ligand of voltage-gated calcium channels।
Use / Purpose in this patient:
ডাক্তার সম্ভবত neuropathic/radicular component সন্দেহ করেছেন—যেমন কোমর থেকে পায়ে shooting/burning pain, tingling, numbness ইত্যাদি থাকলে এ ধরনের drug ব্যবহার করা হয়। Gabapen 100 হলো Incepta-এর gabapentin 100 mg।

Evidence point: uncomplicated mechanical low-back pain বা routine sciatica-তে gabapentinoid নিয়মিত ব্যবহারের পক্ষে evidence দুর্বল। NICE বর্তমানে low-back pain এবং sciatica-তে routine gabapentinoid ব্যবহারের বিরুদ্ধে recommendation দেয়।
4. Esoral 20 mg
Generic name: Esomeprazole 20 mg

Use / Purpose in this patient:
এটি কোমরের ব্যথার ওষুধ নয়। Brufen-এর মতো NSAID ব্যবহারে gastric irritation, dyspepsia বা ulcer risk কমাতে gastroprotection হিসেবে ডাক্তার দিয়ে থাকতে পারেন। Esoral 20 হলো Eskayef-এর esomeprazole 20 mg।

5. Tab. Zodef 18 mg — FINAL CORRECTION
Generic name: Deflazacort 18 mg
Brand name: Zodef 18 mg

Therapeutic category: Corticosteroid / Anti-inflammatory drug
Pharmacological class: Systemic glucocorticoid corticosteroid. Deflazacort হলো prednisolone-related corticosteroid, যার প্রধান pharmacological effect anti-inflammatory এবং immunosuppressive।
Low Back Pain-এ বাংলাদেশে সাধারণত কী কী drug ব্যবহার করা হয়?
কারণ অনুযায়ী prescription পরিবর্তিত হয়। Mechanical/nonspecific LBP-তে সাধারণ prescribing groups হলো:
- NSAIDs: Ibuprofen, Naproxen, Diclofenac, Aceclofenac, কখনও selected patient-এ Etoricoxib/Celecoxib। Guideline অনুযায়ী NSAID ব্যবহার করলে lowest effective dose, shortest necessary duration গুরুত্বপূর্ণ।
- Muscle relaxant: acute painful spasm থাকলে short-term Tizanidine-এর মতো drug ব্যবহার করা হয়। ACP acute/subacute LBP-তে medication প্রয়োজন হলে NSAID অথবা skeletal muscle relaxant বিবেচনা করে।
- Gabapentin/Pregabalin: বাংলাদেশে radiating/neuropathic pain-এ prescribing দেখা যায়, কিন্তু routine nonspecific LBP বা sciatica-র জন্য বর্তমান NICE recommendation এগুলো সমর্থন করে না।
- PPI: NSAID-associated gastric risk থাকলে Esomeprazole/Omeprazole/Pantoprazole gastroprotection-এর জন্য ব্যবহার হতে পারে।
- Paracetamol: adjunct হিসেবে কিছু ক্ষেত্রে ব্যবহৃত হয়; NICE low-back pain-এ paracetamol alone recommend করে না।
- Weak opioid, যেমন tramadol: routine treatment নয়; selected acute severe LBP-তে NSAID contraindicated/not tolerated/ineffective হলে বিবেচনা করা যায়। Chronic LBP-তে opioid routine ব্যবহার recommend করা হয় না।

প্রেসক্রিপশনের Investigation অংশে যতটা পরিষ্কার বোঝা যাচ্ছে:
1. MRI of Lumbar/Lumbosacral Spine
- পাশে L4–L5 level লেখা আছে।
- অর্থাৎ কোমরের L4–L5 vertebral/disc level evaluate করার জন্য MRI-এর উল্লেখ আছে বলে মনে হচ্ছে।
ছবিতে প্রথম অংশটা সবুজ clip দিয়ে ঢাকা, তাই MRI নতুন করে করতে বলেছেন নাকি আগের MRI report-এর finding হিসেবে L4–L5 লিখেছেন—এটা ১০০% নিশ্চিত বলা যাচ্ছে না।
