***Patient main history***:-
সম্মানিত রোগী কেরানিগঞ্জ থেকে আমাকে দেখালেন। তিনি আমার পোস্ট,ভিডিও দেখে Voriconazole খাওয়া বাদ দিয়েছেন তাও ২০ দিনের মাথায়। তার মধ্যে যতটুকু সতেচনতা এসেছে সেটাই অনেক।
২৪ বছর বয়স, এখনি দাউদের মহামারিতে তার মুখ, মাথায় চুলের মধ্যে সহ শরীরের সমস্ত জায়গায় এগুলা ছড়িয়ে গেছে।
৫ বছর ধরে ভুগছে এই দাউদের জন্য। দেখিয়েছেন ডাক্তার তবে একজনও তারা MBBS ডাক্তারও না। প্রেস্ক্রিপশন দেখলেই বুঝবেন সে কোন লেভেলের চিকিৎসা দিয়েছে।
এখন আর তর্কে জড়াই না, কমেন্টে গেলে অনেক কিছু দেখি চুপচাপ করে। যে যা খুশি চিকিৎসা দিচ্ছে। চিকিৎসা দেওয়া এত সহজ না রে পাগলা।
মেয়েটাকে Liver Function test দিয়েছি। রিপোর্ট ছাড়া একটাও Oral antifungal দেইনাই আমি।
আমার জীবনে আমি একজন রোগীকে ভোরিকোনাজল লিখেছিলাম, সে ভোরিকোনাজল খেয়েই সুস্থ থাকে, তার Skin scrapping for Fungal Culture & sensitivity Test এ সকল এন্টিফাংগাল রেজিস্ট্যান্ট ছিলো শুধু Voriconazole আর Posaconazole ছাড়া। তাই বাধ্য হয়ে দিয়েছিলাম।
আমাকে যারা চেনেন জানেন, একটা কথা বলি,
” আমি কোন ম্যাজিক দেখানো ডাক্তার না, রাতারাতি ভালো করে দেওয়ার গ্যারান্টি দিতে পারবোনা”- সুস্থ করার মালিক উপরওয়ালা আর আপনি নিজেও। মেনে চললে সুস্থ হবেন আশাকরি, সুস্থ থাকবেন ও আশা করি।

প্রেসক্রিপশনটি আমি হাতের লেখা + MedEx therapeutic class + standard dermatology/infectious-disease guidance মিলিয়ে review করেছি। কিছু জায়গার handwriting অস্পষ্ট, তাই যেগুলো নিশ্চিত নয় সেগুলো আলাদা করে বলছি।
1. C/C — Chief Complaints
প্রেসক্রিপশনে C/C আলাদা করে লেখা নেই। ওষুধের pattern থেকে সম্ভাব্য complaint ছিল:
- চুলকানিযুক্ত recurrent/widespread skin rash
- Superficial fungal/dermatophyte infection—বিশেষত tinea corporis/cruris জাতীয় infection
- Ivermectin থাকার কারণে associated/suspected scabies-ও বিবেচনায় ছিল বলে মনে হয়।
Confidence: 0.70 — কারণ রোগের history বা lesion-এর photograph/clinical description এখানে নেই।
2. Working Clinical Diagnosis
সবচেয়ে সম্ভাব্য Working Diagnosis
Extensive/Recurrent Dermatophytosis (Tinea corporis / Tinea cruris) with significant pruritus.
Itraconazole + luliconazole + ketoconazole preparation + terbinafine-containing cream—এই antifungal-heavy prescription dermatophytosis-এর দিকে খুব strongly ইঙ্গিত করে। অন্যদিকে ivermectin 12 mg ৭ দিন পরপর ধরনের নির্দেশনা scabies treatment pattern-এর সঙ্গে মেলে।

তবে এটিকে confirmed final diagnosis বলা যাবে না। Recurrent/extensive tinea হলে KOH microscopy এবং প্রয়োজন অনুযায়ী fungal culture করা যৌক্তিক; recalcitrant cases-এ culture বিশেষভাবে উপকারী।

আমার reading:
Cap. Itracon SUBA 65 mg — 1+0+1; duration-এর গোল করা অংশটি স্পষ্ট নয়।
Tab. Voricare 50 mg — 1+0+1; duration পুরোপুরি readable নয়।
Tab. Urtizin 25 mg — 0+0+1; সম্ভবত প্রায় ১ মাস।
Tab. Alben-DS — 0+0+1; পাশে “৭ দিন পরপর” জাতীয় নির্দেশনা দেখা যাচ্ছে।
Tab. Alice 12 mg — 0+0+1; “৭ দিন পরপর” লেখা বেশ স্পষ্ট।
Lulider cream + Tetramix cream — সম্ভবত একসঙ্গে/নির্দেশমতো affected area-তে দিনে ২ বার, প্রায় ১ মাস লেখা।
Odessy Shampoo লেখা আছে কিন্তু crossed out। নিচে Ketokon/Ketocon soap লেখা আছে।
Dose-duration-এর অস্পষ্ট অংশগুলো original prescriber/pharmacist-এর কাছ থেকে confirm করা উচিত—বিশেষ করে দুইটি systemic azole একসঙ্গে থাকার কারণে।

এই prescription-এ সম্ভাব্য উদ্দেশ্য: Difficult/refractory fungal infection cover করার চেষ্টা।

গুরুত্বপূর্ণ clinical point: Itraconazole এবং voriconazole দুটিই systemic triazole। Routine dermatophytosis-এ একসঙ্গে দুই systemic azole ব্যবহার standard management নয়। Voriconazole-এর usual approved indications-ও মূলত invasive/severe fungal disease। তাই দুটো সত্যিই একই সময়ে খেতে বলা হয়েছে কিনা prescriber-এর সঙ্গে confirm করা প্রয়োজন।

Pharmacological class: First-generation H1 receptor antagonist; piperazine antihistamine
Purpose: চুলকানি/pruritus কমানোর জন্য। Hydroxyzine histamine-mediated itching কমায় এবং sedationও করতে পারে।

প্রেসক্রিপশনে 0+0+1 — ৭ দিন পর পর জাতীয় নির্দেশনা দেখা যাচ্ছে। Classic scabies-এ ivermectin weight-based 200 µg/kg, সাধারণত দ্বিতীয় dose 7–14 দিন পর দেওয়া হয়। তাই এই ওষুধটি prescription-এ scabies suspicion-এর সবচেয়ে শক্তিশালী clue।

যদি রোগটি সত্যিই dermatophytosis/tinea হয়, তাহলে clobetasol-containing steroid-antifungal combination দীর্ঘদিন ব্যবহার করা ভালো practice নয়। Potent steroid fungal inflammation সাময়িক কমিয়ে lesion mask করতে পারে, fungus ছড়িয়ে দিতে পারে এবং tinea incognito তৈরি করতে পারে। Dermatophytosis consensus-এ topical corticosteroids strongly discouraged।

Handwriting-এ এটি লেখা আছে, কিন্তু exact Bangladesh brand/composition MedEx থেকে নিশ্চিত করতে পারিনি। MedEx-এ ketoconazole medicated bars বিভিন্ন brand-এ পাওয়া যায় এবং generic ketoconazole-এর therapeutic class Drugs for subcutaneous and mycoses।
তাই exact unit price না জেনে অনুমান করা ঠিক হবে না।

5. Clinical Review — Prescription-এর প্রধান 3টি weak point
1. Itraconazole + Voriconazole একসঙ্গে
Concern: High; confidence 0.93.
দুটিই systemic triazole antifungal। Routine tinea corporis/cruris-এর guideline-based treatment-এ systemic treatment প্রয়োজন হলে terbinafine বা itraconazole ব্যবহার করা হয়; dual systemic azole therapy routine recommendation নয়। Voriconazole মূলত invasive/severe fungal disease-এর medicine।
2. Tetramix-এ Clobetasol আছে
Concern: High; confidence 0.95.
Potent corticosteroid fungal lesion-এর redness/itching কমালেও infection mask বা spread করতে পারে এবং tinea incognito তৈরি করতে পারে।
3. Diagnosis confirm না করেই multiple antifungals + antiparasitic
Concern: Moderate–high; confidence 0.88.
Recurrent/extensive tinea হলে অন্তত KOH microscopy, এবং chronic/recurrent/recalcitrant disease হলে fungal culture যুক্তিযুক্ত। Ivermectin/Albendazole-এর indication-ও history ও examination দিয়ে পৃথকভাবে establish করা উচিত।
Investigations
এই ধরনের case হলে আমার clinical approach হবে:
- Skin scraping + KOH microscopy from active margin।
- Recurrent/recalcitrant হলে fungal culture/speciation।
- Scabies suspicion থাকলে burrow examination/dermoscopy বা scraping এবং household contact history।
- Prolonged systemic itraconazole/voriconazole ব্যবহার হলে baseline এবং follow-up LFT; itraconazole >1 month ব্যবহারে MedEx-ও liver monitoring-এর কথা উল্লেখ করে।
- Diabetes/immunosuppression/recurrent fungal disease-এর clinical suspicion থাকলে appropriate glucose/HbA1c বা immune evaluation।
Differential Diagnosis
Clinical lesion না দেখে যে differentialগুলো বাদ দিতে হবে: Tinea corporis/cruris, tinea incognito, scabies, candidal intertrigo, pityriasis versicolor, seborrhoeic dermatitis, eczema/contact dermatitis, inverse psoriasis, erythrasma।
Overview / Summary
আমার working final assessment:
Extensive/recurrent superficial dermatophytosis (probably tinea corporis/cruris) with pruritus; concomitant scabies is possible.
Confidence: 0.75.
শুধু prescription দেখে confirmed diagnosis দেওয়া যায় না, তবে antifungal pattern-এর কারণে dermatophytosis খুব likely এবং Alice 12 mg-এর 7-day interval pattern scabies suspicion নির্দেশ করে।
সবচেয়ে বড় concern হচ্ছে Itraconazole + Voriconazole concomitant use এবং clobetasol-containing Tetramix prolonged use।
Actionable Plan
প্রথমে lesion clinically examine করে KOH ± fungal culture দিয়ে diagnosis confirm করব। যদি সাধারণ/extensive dermatophytosis হয়, guideline-concordant single systemic antifungal strategy + appropriate topical antifungal বেশি যৌক্তিক; দুই systemic azole একসঙ্গে নেওয়া হচ্ছে কিনা prescriber-এর সঙ্গে অবশ্যই verify করা উচিত। Scabies confirmed/suspected হলে ivermectin dose body-weight based হওয়া দরকার এবং close contacts/environmental measuresও প্রয়োজন।
