প্রেসক্রিপশনটি আমি হাতের লেখা + MedEx Bangladesh + বর্তমান allergic rhinitis guideline মিলিয়ে review করেছি। ২ নম্বর ওষুধের brand-name পুরোপুরি পরিষ্কার নয়, তাই সেখানে অনুমানকে আলাদা করে দেখাচ্ছি—ভুল brand তৈরি করে বলছি না।
১. প্রেসক্রিপশনে C/C, Diagnosis ও Investigation
Diagnosis / Working diagnosis:
লেখা আছে “A. Rhinitis” → Allergic Rhinitis। Confidence: 0.98
C/C: আলাদা করে কোনো chief complaint লেখা নেই। অর্থাৎ sneezing, rhinorrhea, nasal blockage, itching ইত্যাদি এই prescription sheet-এ document করা হয়নি। Confidence: 0.96
Investigation: এই প্রেসক্রিপশনে কোনো investigation/test লেখা নেই। Confidence: 0.99
Typical allergic rhinitis সাধারণত history + nasal examination দিয়েই diagnose করা যায়; routine CT/X-ray/lab test দরকার হয় না। Diagnosis uncertain হলে, treatment response না হলে বা allergen-specific therapy বিবেচনা করলে skin-prick test/serum specific IgE উপকারী হতে পারে। Dhingra 8th edition-এ Allergic Rhinitis আলাদা Chapter 30 হিসেবে আছে।

Intranasal synthetic glucocorticoid/corticosteroid।
Use/Purpose:
নাকের mucosal inflammation কমায়—nasal blockage, rhinorrhea, sneezing, inflammatory swelling কমানোর জন্য ব্যবহার করা হয়। Fluticasone glucocorticoid receptor-mediated anti-inflammatory effect তৈরি করে।

গুরুত্বপূর্ণ clinical point: Flonaspray 50 mcg/spray MedEx-এ seasonal/perennial allergic rhinitis-এর জন্য listed, কিন্তু Flonaspray Max 93 mcg/spray specifically chronic rhinosinusitis with/without nasal polyp-এর জন্য listed। তাই এখানে যদি diagnosis শুধু allergic rhinitis হয় এবং CRS/polyp না থাকে, তাহলে 93 mcg “Max” কেন নির্বাচন করা হয়েছে—ENT findings দিয়ে correlate করা উচিত। এটি prescription ভুল প্রমাণ করে না; coexisting rhinosinusitis/polyp বা অন্য clinical rationale থাকতে পারে যা sheet-এ লেখা নেই। Confidence: 0.95

এটি শক্তিশালী systemic anti-inflammatory drug। Severe allergic inflammation/marked nasal obstruction কমানোর জন্য short course হিসেবে দেওয়া হতে পারে। Methylprednisolone immune-inflammatory response suppress করে।
