C/C — Chief Complaints / প্রধান অভিযোগ
- Unable to sit independently yet — এখনো নিজে নিজে বসতে পারে না।
- H/O GDD — আগে থেকেই Global Developmental Delay (GDD) আছে।

- Family history: “Sib death” এর পাশে (+) চিহ্ন আছে। “Consanguinity/Abortion” লাইনের নিচে (−) চিহ্ন আছে।
- Birth-history-এর PNA/LBW/NV/NB অংশে কিছু tick/mark আছে, কিন্তু কোনটি positive/negative তা ছবিতে যথেষ্ট পরিষ্কার নয়—এটি নিশ্চিত করে বলা নিরাপদ হবে না।
- Tone ↑ — muscle tone increased.
- Jerks ↑↑ — deep tendon reflexes/brisk jerks markedly increased.
- Plantar ও gait-এর নির্দিষ্ট finding লেখা নেই।
- Development Therapy ও Speech Therapy-তে tick দেওয়া আছে।
- Behavior modification therapy-তে স্পষ্ট tick নেই।
Increased tone + brisk jerks একটি upper motor neuron/spastic pattern-এর সঙ্গে compatible, যদিও প্রেসক্রিপশনে CP-এর specific motor subtype আলাদা করে লেখা হয়নি। Confidence: 0.94
Investigation
এখানে নতুন কোনো blood test/EEG/MRI order লেখা দেখা যাচ্ছে না। একটি CT scan of brain-এর result documented আছে:

- Generalized cortical atrophy — cerebral cortex-এর generalized volume loss/atrophy।
- Multifocal encephalomalacia — মস্তিষ্কের একাধিক জায়গায় পুরোনো injury-এর কারণে tissue softening/loss।
- Hypoplastic CC — Corpus callosum স্বাভাবিকের তুলনায় underdeveloped/hypoplastic।
এই structural abnormalities CP ও significant developmental impairment-এর clinical picture-এর সঙ্গে সামঞ্জস্যপূর্ণ হতে পারে, তবে CT alone দিয়ে functional severity নির্ধারণ করা যায় না। Confidence: 0.98
