ছবিটা দেখে যতটা পরিষ্কারভাবে পড়া যাচ্ছে, prescription-টির মূল বিষয় হলো Impetigo + Severe phimosis। রোগীর বয়স লেখা আছে 9 months, weight 7.5 kg।
C/C / Diagnosis
1. Impetigo — ত্বকের superficial bacterial infection, সাধারণত Staphylococcus aureus বা Streptococcus pyogenes দিয়ে হয়। এজন্যই oral + topical antibiotic দেওয়া হয়েছে। IDSA guideline-এ mupirocin topical therapy impetigo-তে স্বীকৃত treatment।
2. Severe phimosis — foreskin খুব tight/non-retractile। তবে ৯ মাস বয়সে foreskin না খোলা অনেক শিশুর ক্ষেত্রে physiological হতে পারে; symptoms/scarring/recurrent infection থাকলে আলাদা management লাগে। EAU 2026 guideline symptomatic pathological phimosis-এ topical corticosteroidকে first-line option বলে এবং refractory/suitable cases-এ surgery বিবেচনা করে।

Local wound care — সহজভাবে
প্রথমে Viodin 10% solution ২–৩ চা-চামচ হালকা কুসুম গরম পানিতে মিশাবেন। পরিষ্কার তুলা বা sterile gauze সেই পানিতে ভিজিয়ে আক্রান্ত জায়গা আলতোভাবে পরিষ্কার করবেন। এরপর জায়গাটি শুকিয়ে নিয়ে Trego 2% ointment পাতলা করে লাগাবেন।
Viodin শুধু external use-এর জন্য। চোখ, মুখ বা mucosal surface-এ দেবেন না। আর “২–৩ চামচ কতটুকু পানিতে”—এই dilution ratio prescription-এ স্পষ্ট না থাকলে, বিশেষ করে ৯ মাসের শিশুর ক্ষেত্রে, prescribing doctor/pharmacist-এর কাছ থেকে পানির পরিমাণটা confirm করে নেওয়া ভালো।
Investigation :-
Prescription-এ নিচের coagulation tests দেখা যাচ্ছে:
BT — Bleeding Time
CT — Clotting Time
PT — Prothrombin Time
APTT — Activated Partial Thromboplastin Time
এগুলো bleeding/clotting status assess করার জন্য করা হয়; phimosis-এর জন্য কোনো procedure/circumcision পরিকল্পনা থাকলে pre-procedure evaluation-এর অংশ হিসেবে doctor চাইতে পারেন।

১. Viodin মেশানো কুসুম গরম পানি দিয়ে জায়গা পরিষ্কার করুন।
২. জায়গা শুকিয়ে Trego ointment লাগান।
৩. Fluclox 1.5 mL × 4 times/day × 7 days prescription অনুযায়ী complete করুন।
৪. Deslor ও Xinc-B prescription অনুযায়ী দিন।
৫. Fluclox-এর bottle 125 mg/5 mL নাকি অন্য strength—dose দেওয়ার আগে অবশ্যই মিলিয়ে নিন।
৬. Deslor-এর dose এবং Viodin-এর exact dilution ratio infant-এর জন্য prescriber-এর সঙ্গে confirm করুন।
