TC(1051).(05).Orthopedics Case No 129:- Mechanical Low Back Pain

Mechanical Low Back Pain — বাংলায় Clinical Notes

Important: নিচের drug list adult patient-এর জন্য academic/prescription-writing guide। Pregnancy, CKD, PUD/GI bleed, CAD/stroke, uncontrolled HTN, anticoagulant use, liver disease, elderly patient—এসব ক্ষেত্রে dose/choice অবশ্যই individualize করতে হবে।

1) Definition /

Mechanical low back pain

NICE guideline অনুযায়ী low back pain with/without sciatica assessment ও management-এ serious alternative diagnosis exclude করা জরুরি, আর non-specialist setting-এ routine imaging করা উচিত নয়।

2) Causes / কারণগুলো — Bangla pointwise

A. Muscle–ligament strain / sprain

  • sudden exertion, gym injury
  • paraspinal muscle spasm

B. Poor posture ও occupational cause

  • forward head/rounded shoulder posture
  • prolonged driving
  • laptop/mobile use with flexed spine
  • repeated lifting, pushing, pulling

C. Disc-related mechanical pain

  • annular tear বা disc degeneration
  • sitting/flexion-এ pain বাড়তে পারে

D. Facet joint arthropathy

  • extension/standing/walking-এ pain বাড়ে
  • local paraspinal pain
  • older age বা degenerative spine disease-এ common

E. Sacroiliac joint dysfunction

  • buttock/low back pain
  • prolonged standing, stair climbing, pregnancy/postpartum biomechanics-এ হতে পারে

F. Risk factors

  • obesity
  • sedentary lifestyle
  • weak core muscle
  • smoking
  • poor sleep
  • anxiety/depression/fear-avoidance behaviour
  • previous episode of LBP
  • heavy manual work

Lancet review অনুযায়ী low back pain recurrent ও multifactorial condition; biological, psychological ও social factors disability বাড়াতে পারে।

3) Symptoms /

Common symptoms

  • bending, lifting, prolonged sitting/standing-এ pain বাড়ে
  • paraspinal muscle tightness/spasm

  • localized low back pain
  • posture-related pain
  • pain changes with movement
  • no fever, no weight loss, no night sweats
  • bladder/bowel সমস্যা নেই
  • progressive weakness নেই

Red flag symptoms — এগুলো থাকলে urgent evaluation

  • urinary retention/incontinence
  • fecal incontinence
  • saddle anesthesia
  • bilateral sciatica বা rapidly progressive leg weakness
  • fever, chills, IV drug use, immunosuppression
  • unexplained weight loss, previous cancer
  • significant trauma

NICE serious pathology যেমন cancer, infection, trauma, inflammatory disease exclude করতে বলেছে।

4) Clinical Diagnosis / Clinically diagnosis

Mechanical LBP diagnosis mainly clinical।

History taking

  • Onset: sudden/gradual
  • Trigger: lifting, bending, fall, twisting
  • Pain site: lumbar/paraspinal/SI region
  • Pain character: dull/aching/spasm
  • Aggravating factor: movement, flexion, extension, sitting, standing
  • Relieving factor: rest, heat, position change
  • Red flags: fever, weight loss, trauma, cancer history, bladder/bowel symptoms
  • Occupational history
  • Previous episodes
  • Drug history: steroid, anticoagulant
  • Comorbidities: CKD, PUD, CAD, HTN, diabetes

Physical examination

  • Gait, posture, scoliosis/kyphosis
  • Lumbar ROM: flexion, extension, lateral bending
  • Paraspinal tenderness/spasm
  • SI joint provocation tests if suspected
  • Straight Leg Raise test: radicular pain suggestive
  • Femoral stretch test: upper lumbar radiculopathy
  • Full neurological exam:
    • Power: L2–S1 myotomes
    • Sensation: dermatomes
    • Reflex: knee jerk L3–L4, ankle jerk S1
    • Plantar response
    • Perianal sensation/anal tone if cauda equina suspected

Clinical diagnosis suggestive of mechanical LBP

  • Pain related to posture/movement
  • No systemic symptoms
  • No progressive neurological deficit
  • Normal or near-normal neurological examination
  • Red flags absent
  • Pain duration acute <6 weeks, subacute 6–12 weeks, chronic >12 weeks

5) Investigations / Investigation specific plan

Routine investigation

  • No routine X-ray/MRI in uncomplicated acute mechanical LBP. NICE says non-specialist setting-এ low back pain with/without sciatica-তে routine imaging offer করা উচিত নয়; specialist setting-এ imaging only if result management change করবে।

When to do X-ray lumbar spine

  • significant trauma
  • suspected fracture
  • elderly + osteoporosis
  • long-term steroid use
  • deformity
  • persistent pain >6 weeks despite proper management
  • suspected spondylolisthesis/structural abnormality

When to do MRI lumbosacral spine

  • cauda equina symptoms
  • progressive neurological deficit
  • suspected infection/malignancy
  • severe radiculopathy not improving after 6 weeks
  • surgery/injection planning
  • unexplained severe night/rest pain

When to do lab tests

  • CBC, ESR, CRP: infection/inflammatory/malignancy suspicion
  • RFT/serum creatinine: NSAID use before/while, elderly, CKD/HTN/DM
  • LFT: paracetamol risk, liver disease, alcohol use
  • Urine R/M/E: renal colic/UTI differential
  • HLA-B27: inflammatory back pain/spondyloarthritis suspicion
  • Serum calcium, ALP, PSA/other malignancy workup: cancer suspicion

6) Differential Diagnosis /

  • Lumbar disc prolapse with radiculopathy
  • Lumbar spinal stenosis
  • Vertebral fracture
  • Spinal infection: discitis, osteomyelitis, epidural abscess
  • Malignancy/metastasis
  • Ankylosing spondylitis/axial spondyloarthritis
  • Renal colic/pyelonephritis
  • Abdominal aortic aneurysm
  • Hip osteoarthritis
  • Sacroiliitis
  • Gynecological cause: endometriosis, PID
  • Pancreatitis/retroperitoneal pathology

  • Chronic low back pain: ৩ মাসের বেশি ব্যথা চলতে পারে।
  • Recurrent episodes:
  • Sleep disturbance, anxiety/depression: chronic pain mental health impact করে।
  • Work absenteeism: productivity
  • Drug-related complications: NSAID থেকে gastritis/ulcer/renal injury/BP rise; muscle relaxant থেকে sedation/fall risk।
  • Missed serious diagnosis: red flag ignore করলে infection, malignancy, cauda equina delay হতে পারে।

8) Treatment & Management

A. Non-pharmacological management — first-line

Patient education

  • Work/ADL early return encourage করতে হবে।

Heat therapy

  • Acute muscle spasm হলে local heat 15–20 min, দিনে 2–3 বার।

Exercise / physiotherapy

  • Walking
  • McKenzie extension exercise if extension preference
  • Core stabilization
  • Pelvic tilt
  • Hamstring/hip flexor stretching
  • Glute strengthening
  • Posture correction
  • Ergonomic training

Avoid routinely

  • Lumbar belt/corset routine নয়
  • Traction নয়
  • TENS/ultrasound/interferential therapy routine নয়
  • Routine spinal injections
