1. Definition
Types:
- Subacute:
- Chronic: >৩ মাস, recurrent discharge/sinus, sequestrum, deformity।
- Hematogenous: blood-borne spread।
- Contiguous/direct inoculation: ulcer, trauma, fracture, operation, implant।
- Diabetic foot osteomyelitis: diabetic foot ulcer থেকে bone infection।
2. Cause / Etiology —
A. Infective causes
- Streptococcus species
- Cellulitis, diabetic foot, soft tissue infection থেকে spread করতে পারে।
- Gram-negative bacilli
- E. coli, Klebsiella, Proteus, Enterobacter।
- UTI, elderly, diabetic, hospital-acquired infection, trauma বা surgery-এর পরে common।
- Pseudomonas aeruginosa
- Puncture wound through shoe, diabetic foot, warm/tropical climate, hospital-acquired infection, chronic wound।
- Anaerobes
- Diabetic foot, foul-smelling ulcer, ischemic/gangrenous tissue, deep polymicrobial wound।
- Mycobacterium tuberculosis
- Spine/Pott disease, chronic back pain, weight loss, low-grade fever; Bangladesh context-এ important differential।
- Fungal osteomyelitis
- Immunosuppressed patient, long-term steroid, transplant, HIV, uncontrolled diabetes।
B. Predisposing factors
- Diabetes mellitus, diabetic foot ulcer।
- Peripheral vascular disease।
- Open fracture, crush injury, penetrating injury।
- Orthopedic implant/prosthesis/internal fixation।
- Recent orthopedic surgery।
- Chronic kidney disease/hemodialysis।
- IV drug use।
- Immunosuppression, malignancy, steroid therapy।
- Sickle cell disease — Salmonella osteomyelitis classically associated।
- Poor wound care, pressure sore।
- Dead bone fragment = sequestrum।
- New reactive bone = involucrum।
4. Symptoms / Clinical Features —
Acute osteomyelitis
- Localized bone pain — deep, constant, movement
- Fever, chills, malaise —
- Swelling, redness, warmth over affected bone।
- Tenderness — palpation
- Restricted movement — adjacent joint movement
- Limping / inability to bear weight — lower limb
- Children: irritability, refusal to walk/use limb, fever।
Chronic osteomyelitis
- দীর্ঘদিনের pain।
- Recurrent swelling।
- Sinus tract দিয়ে pus discharge।
- Non-healing ulcer।
- Low-grade fever বা fever absent।
- Deformity, pathological fracture।
Diabetic foot osteomyelitis
- Chronic deep ulcer, especially >2 weeks।
- Foul-smelling discharge।
- Surrounding cellulitis।
- Foot deformity/Charcot foot।
Vertebral osteomyelitis
- Persistent back pain — commonest।
- Fever may be absent।
- Night pain।
- Neurologic sign: weakness, numbness, bladder/bowel involvement হলে emergency।
- Epidural abscess হলে severe back pain + fever + neuro deficit।
5. Clinically diagnosis — bedside approach
Osteomyelitis clinically suspect
- Bone pain + fever বা raised inflammatory marker।
- Open wound/ulcer over bony prominence।
- Diabetic foot ulcer with exposed/probed bone।
- Recent trauma/open fracture/surgery।
- Persistent back pain + raised ESR/CRP ± bacteremia।
- Chronic sinus discharge from bone।
- X-ray/MRI suggestive changes।
Preferred definitive diagnosis: bone biopsy culture/histopathology।
6. Investigations — specific work-up
A. Blood tests
- Blood culture × 2 sets before antibiotic: especially fever/sepsis/vertebral osteomyelitis।
- RBS/HbA1c: diabetes control।
- Renal function: vancomycin, aminoglycoside, beta-lactam dose adjustment।
- LFT: rifampicin, linezolid, prolonged antibiotic।
- Procalcitonin/lactate: sepsis suspected
B. Imaging
- Plain X-ray — first test
- Early stage-এ normal হতে পারে।
- Later: lytic lesion, periosteal reaction, sequestrum, cortical destruction।
- MRI with contrast — best imaging for early diagnosis
- Bone marrow edema, abscess, sinus tract, soft tissue extension, epidural abscess detect
- CT scan
- Sequestrum, cortical destruction, surgical planning, CT-guided biopsy।
- Bone scan / tagged WBC scan / PET-CT
- MRI contraindicated, multifocal disease, hardware artifact
C. Microbiology / Histology
- Bone biopsy culture + histopathology
- Gold/reference standard।
- Pus/deep tissue culture
- Surgical debridement sample।
- Superficial swab avoid।
- AFB smear/culture, GeneXpert MTB/RIF
- TB osteomyelitis/Pott disease suspicion
D. Diabetic foot specific
- Probe-to-bone test।
- Plain X-ray।
- ESR/CRP/PCT।
- MRI if diagnosis uncertain।
- Bone sample for culture if possible।
- Vascular assessment: ABI, toe pressure, Doppler।
IWGDF/IDSA diabetic foot guideline osteomyelitis diagnosis, microbiological sampling, antibiotic choice, surgical approach এবং follow-up remission নিয়ে structured recommendation দেয়।
7. Differential diagnosis
- Septic arthritis।
- Cellulitis।
- Deep abscess।
- Charcot neuroarthropathy।
- Bone tumor / Ewing sarcoma।
- Stress fracture।
- Gout/pseudogout।
- TB spine vs pyogenic vertebral osteomyelitis।

- Chronic venous ulcer without osteomyelitis।
- Pathological fracture: infected bone
- Joint stiffness/septic arthritis: adjacent joint involvement।
- Epidural abscess/paralysis: vertebral osteomyelitis-এ spinal cord compression।
- Amputation: diabetic foot/ischemia/necrosis uncontrolled
- Squamous cell carcinoma: দীর্ঘদিনের chronic sinus tract থেকে rare malignant change।
- Recurrence: inadequate debridement, wrong antibiotic, poor vascular supply, uncontrolled diabetes।
9. Treatment management — core principles
Treatment goals
- Source control: abscess drainage, debridement, dead bone removal।
- Culture-directed antibiotic।
- Limb function preserve।
- Diabetes/vascular control।
- Recurrence prevention।
When antibiotic delay
Duration
- Most adult bacterial osteomyelitis:
- Native vertebral osteomyelitis: IDSA recommends total 6 weeks parenteral or highly bioavailable oral therapy for most bacterial cases।
- Diabetic foot osteomyelitis without bone resection/amputation: 6 weeks; minor amputation with positive bone margin culture হলে up to 3 weeks।
- Uncomplicated pediatric acute hematogenous osteomyelitis: PIDS/IDSA suggests 3–4 weeks for uncomplicated S. aureus AHO responding to therapy।
OVIVA trial দেখিয়েছে selected bone/joint infection-এ first 6 weeks oral therapy IV therapy-এর non-inferior হতে পারে, তবে oral switch করতে হলে organism, susceptibility, absorption, adherence, severity এবং source control বিবেচনা জরুরি।

Overview / Summary
Osteomyelitis হলো bone infection; commonest cause Staphylococcus aureus।
Actionable Plan / Steps
- Culture report আসলে narrow করুন: MSSA হলে cefazolin/flucloxacillin; MRSA হলে vancomycin/linezolid option; Gram-negative হলে susceptibility-based ceftriaxone/cefepime/ciprofloxacin/meropenem।
- Abscess, sequestrum, necrotic bone, hardware infection থাকলে orthopedic/vascular surgical source control করুন।
- ESR/CRP, wound healing, pain, fever, renal/LFT/CBC monitoring
- Diabetic foot হলে off-loading, vascular assessment, glucose control এবং foot-care education দিন।
