Crush Injury / Crush Syndrome —

1. Definition

Crush syndrome-এর core pathology: muscle necrosis → rhabdomyolysis → myoglobin, potassium, phosphate, lactate release → AKI + hyperkalemia + metabolic acidosis + arrhythmia। INSARAG guideline অনুযায়ী প্রধান acute concerns হলো hypovolaemia, hyperkalemia, metabolic acidosis ও AKI; delayed concerns হলো renal failure, ARDS, coagulopathy ও severe sepsis।

2. Causes /

  • Industrial / construction injury: machine, roller, lift, press machine, crane load
  • Blast / war injury / assault
  • Farm injury: tractor, harvester, heavy agricultural equipment
  • Electrical burn / severe burn: muscle damage + rhabdomyolysis

3. Symptoms / Clinical Features

Local symptoms

  • Swelling, tightness, tense compartment
  • Skin: bruised, pale, mottled, blistered, crushed wound
  • Numbness, tingling, burning sensation, anaesthesia
  • Weakness / paralysis / movement
  • Pulse কম/না পাওয়া, capillary refill slow, limb cold
  • Fracture, open wound, bleeding, deformity

Systemic symptoms

  • Dark reddish-brown urine / tea-coloured urine
  • Urine output
  • Tachycardia, hypotension, dizziness, shock
  • Palpitation, ECG abnormality—hyperkalemia হলে
  • Confusion, restlessness, weakness
  • Nausea, vomiting, severe thirst

INSARAG clinical presentation-এ mottled/blistered/bruised/pale skin, edema, poor perfusion, shock, reddish-brown urine, sensory loss, pain, motor loss ও decreased urine output উল্লেখ আছে।

4. Clinical Diagnosis

Crush injury diagnosis মূলত history + examination + renal/electrolyte monitoring দিয়ে করা হয়।

History

  • urine colour ও urine amount
  • open wound, fracture, bleeding, burn আছে কি?

Examination

  • ABCDE trauma assessment
  • Vitals: BP, pulse, respiratory rate, SpO₂, temperature
  • Limb neurovascular exam: pain, pulse, sensation, movement, capillary refill
  • Compartment syndrome signs:
    • pain out of proportion
    • passive stretch pain
    • tense swollen compartment
    • paresthesia, weakness
    • late sign: pulselessness/paralysis

Rhabdomyolysis clue

AAST consensus অনুযায়ী rhabdomyolysis laboratory finding হিসেবে CK > 5× upper limit of normal বা >1000 IU/L ধরা হয়। Fluid target সাধারণত urine output 1–3 mL/kg/hour, সর্বোচ্চ 300 mL/hour।

5. Investigations

Emergency bedside

  • ECG: peaked T wave, widened QRS, bradycardia → hyperkalemia suspicion
  • Urinary catheter: hourly urine output monitor
  • Pulse oximetry + BP monitoring
  • Pain score + repeated neurovascular charting

Blood tests

  • CBC
  • Blood group, crossmatch
  • Serum electrolytes: K⁺, Na⁺, Ca²⁺, phosphate, magnesium
  • Urea, creatinine, eGFR
  • Creatine kinase / CK serial 6–12 hourly
  • ABG/VBG: metabolic acidosis
  • Serum lactate
  • LFT: AST/ALT may rise due to muscle injury
  • Coagulation profile: PT/INR, aPTT, fibrinogen if DIC suspected
  • CRP, wound culture/blood culture if infection/sepsis suspected

Urine tests

  • Urine R/E
  • Urine dipstick: blood positive but RBC কম → myoglobinuria suspicion
  • Urine myoglobin if available

Imaging

  • X-ray of injured limb/pelvis/spine
  • FAST/eFAST in polytrauma
  • CT trauma series if indicated
  • Doppler US / CT angiography if vascular injury suspected
  • Compartment pressure measurement if diagnosis unclear or patient unconscious

6. Differential Diagnosis

  • Acute compartment syndrome
  • Acute limb ischemia / arterial injury
  • Fracture-dislocation
  • Spinal cord injury / peripheral nerve injury
  • Necrotizing fasciitis
  • Cellulitis with severe swelling
  • DVT
  • Burn/electrical injury-related rhabdomyolysis
  • Drug-induced rhabdomyolysis
  • Heat stroke / seizure-induced rhabdomyolysis

8. Treatment / Management

A. Immediate non-pharmacological management

  • Scene safety
  • ABCDE trauma care.
  • High-flow oxygen if hypoxic/shock.
  • Limb realign + splint.
  • Ring, tight cloth, shoe
  • Open fracture হলে wound ঘষাঘষি/mini-washout না করে saline-soaked sterile dressing + occlusive cover। NICE ও BOAST open fracture-এ early IV antibiotic, saline-soaked dressing, neurovascular documentation ও specialist transfer recommend করে।
  • IV fluid preferably before release/extrication: fluid therapy crush syndrome management-এর cornerstone।
  • Foley catheter দিয়ে urine output monitor.
  • Repeated ECG + K⁺ monitoring.
  • Surgical debridement, fracture fixation, vascular repair, fasciotomy—hospital decision.
  • Dialysis if refractory hyperkalemia, severe acidosis, volume overload, uremic complications, persistent anuria/AKI.

General Quaries of this topics:-

Question No 1. ABCDE trauma assessment kake bole example die bujie bolo?

ABCDE Trauma Assessment

ABCDE মানে: Airway, Breathing, Circulation, Disability, Exposure। এটি critically ill বা injured patient-এর immediate assessment and treatment-এর জন্য widely used systematic approach।

A = Airway with cervical spine protection

  • Patient

  • Jaw thrust maneuver
  • Oropharyngeal/nasopharyngeal airway

Example:

B = Breathing

  • Respiratory rate
  • Breath sound
  • Chest wound, rib fracture, flail chest, tension pneumothorax

  • Oxygen
  • Chest auscultation
  • Tension pneumothorax হলে urgent needle decompression/chest drain
  • Severe breathing failure হলে ventilation support

Example:
SpO₂ 82%, respiratory rate 34/min, left side breath sound কম।

C = Circulation with haemorrhage control

  • Pulse fast/weak?
  • BP কম?
  • Skin ঠান্ডা, ঘামছে, pale?
  • Capillary refill >2 sec?
  • External bleeding

  • Two large-bore IV cannula
  • IV fluid/blood transfusion as indicated
  • Pelvic binder if pelvic fracture suspected
  • Crush syndrome হলে early IV normal saline
  • ECG monitor

Example:
Leg crush injury patient-এর thigh ফুলে গেছে, pulse 130/min, BP 80/50, skin cold clammy। এখানে Circulation failure/shock—bleeding control + IV access + fluid/blood + urgent surgery consult লাগবে।

D = Disability

  • Conscious level: AVPU
    • A = Alert
    • V = Voice-এ response
    • P = Pain-এ response
    • U = Unresponsive
  • GCS score
  • Pupil size/reactivity
  • Limb movement/sensation
  • Blood glucose

  • Low glucose হলে glucose correct
  • Seizure হলে management
  • Head injury suspicion হলে CT brain
  • Spinal injury suspicion হলে immobilization

Example:

E = Exposure / Environment control

  • Back, buttock, axilla, perineum
  • Open fracture
  • Compartment swelling
  • Crush mark
  • Body temperature

  • Clothes cut করে full examination
  • Warm blanket
  • Warm IV fluid if possible
  • Wound cover
  • Recheck ABCD repeatedly

Example:

Full Example

A — Airway

Meaning: airway currently patent, but blood aspiration risk
Action: suction ready, cervical spine protection।

B — Breathing

RR 30/min, SpO₂ 88%, chest pain।
Meaning: chest injury/pneumothorax/contusion
Action: oxygen, chest auscultation, chest X-ray/eFAST।

C — Circulation

Pulse 125/min, BP 90/60, right thigh swollen, urine dark।
Meaning: shock + rhabdomyolysis/crush syndrome risk।
Action: 2 IV line, normal saline, blood tests: K⁺, creatinine, CK, ECG, catheter।

D — Disability

GCS 15, pupil equal, limb sensation
Action: neurovascular monitoring, compartment syndrome assess।

E — Exposure

Full body check করলে calf tense swelling, skin blister, open wound পাওয়া গেল।
Meaning: severe crush injury + infection/tetanus risk।
Action: sterile dressing, antibiotics, tetanus prophylaxis, orthopedic surgery consult।

A — Airway:
C — Circulation: bleeding/shock

Overview / Summary

Question No 2. Rhabdomyolysis clue kake bole bujie dau?

Rhabdomyolysis clue

AAST consensus অনুযায়ী rhabdomyolysis diagnosis-এ সাধারণত CK > 5 times upper limit of normal বা >1000 IU/L গুরুত্বপূর্ণ laboratory clue।

Important Rhabdomyolysis Clues

1. History clue

  • Crush injury / heavy object
  • severe exercise/heat stroke
  • seizure
  • electrical burn
  • drug/alcohol intoxication

2. Symptom clue

  • Muscle pain / severe body pain
  • Muscle swelling
  • Muscle weakness
  • Limb tight/tense feeling
  • Dark urine / tea-coloured urine / cola-coloured urine
  • Urine
  • Weakness, nausea, vomiting, dizziness

3. Urine clue

সবচেয়ে important bedside clue:

  • Urine tea/cola colour

4. Blood test clue

  • CK / CPK very high
  • Serum potassium high
  • Creatinine/urea
  • Phosphate
  • Calcium initially
  • Metabolic acidosis

Real example

  • CK = 12,000 IU/L
  • K⁺ = 6.2 mmol/L
  • Creatinine
  • ECG-তে peaked T wave

Meaning:

  • IV normal saline
  • ECG monitoring
  • potassium emergency management
  • urine output monitoring
  • renal function monitoring
  • possible dialysis if refractory hyperkalemia/AKI

Shortcut মনে রাখুন: “3 Dark + 3 High”

3 Dark

  • Dark urine
  • Dark history: crush/prolonged compression
  • Dark danger: kidney failure risk

3 High

  • High CK
  • High potassium
  • High creatinine

Overview / Summary

Actionable Plan / Steps

  1. Crush injury হলে urine colour ও urine output দেখুন।
  2. Dark urine বা severe muscle swelling/pain থাকলে rhabdomyolysis suspect করুন।
  3. Test করুন: CK/CPK, serum K⁺, creatinine, urea, phosphate, calcium, ABG, urine R/E, ECG।
  4. CK high বা K⁺ high হলে IV fluid + ECG monitoring শুরু করুন।
  5. Hyperkalemia ECG change থাকলে emergency treatment লাগবে।
