TC(895).(20).Sexology Case No 48. Chronic Pelvic Pain Syndrome / CPPS

Chronic Pelvic Pain Syndrome / CPPS —

1. Definition

CP/CPPS = NIH Category III prostatitis
Pelvic/perineal/prostatic pain or discomfort ≥৩ মাস, often with LUTS বা sexual symptoms, কিন্তু uropathogenic bacteria absent.

Clinical core meaning:
“Pain আছে, infection নেই, single drug cure নেই—phenotype-based multimodal management লাগে।”

2. Cause / Etiology & Pathophysiology

Common mechanisms:

  1. Pelvic floor muscle overactivity / myofascial trigger point
Pelvic floor muscle tightness → pain–spasm–pain cycle তৈরি করে। EAU pelvic floor overactivity ও trigger point active look করার পরামর্শ দেয়।
  2. Central sensitization / neuropathic pain
  3. Previous infection as trigger, but ongoing infection usually absent
    Acute prostatitis/UTI pain trigger করতে পারে, কিন্তু CPPS stage-এ culture often negative।
  4. Urinary dysfunction
Bladder neck/prostatic smooth muscle tension, urgency, frequency, hesitancy, weak stream pain worsen
  5. Psychological stress, anxiety, depression, pain fear
    Pain imaginary নয়; তবে stress/anxiety pain amplification করে। EAU pain-related fear, anxiety, depression assess করতে বলে।
  6. Sexual dysfunction-related pain
Painful ejaculation, ED, premature ejaculation, libido reduction CPPS-এর সাথে common association।
  7. Overlapping disorders
IBS, bladder pain syndrome/interstitial cystitis, fibromyalgia, chronic fatigue, pudendal neuralgia, constipation, endometriosis/PID in female differential.

1. Pelvic floor muscle spasm / myofascial dysfunction

Example:

2. Neuropathic pain / central sensitization

Example:
Urine report normal, culture negative, ultrasound normal—তবুও রোগীর burning, deep pelvic pain, penile tip discomfort থাকে।

3. Previous infection as trigger

Important clinical point:

4. Abnormal immune response / chronic inflammation

Example:
তাই CPPS শুধু infection/inflammation নয়।

5. Dysfunctional voiding / bladder outlet functional problem

Mechanism:

Example:

6. Intraprostatic ductal reflux

Simple explanation:

7. Anatomical variation / prostatic calculi

Example:
Recurrent prostatitis-like symptoms, recurrent culture-positive infection, অথবা chronic bacterial prostatitis suspicion থাকলে prostatic stone/calcification clinical relevance পেতে পারে।

When suspect

  • Urethral discharge
  • New sexual partner
  • Unprotected sex
  • Dysuria with sterile pyuria
  • Testicular/epididymal pain
  • Partner STI history

9. Psychological stress, anxiety, depression

Example:
Exam stress, work pressure, relationship stress, poor sleep—এসবের সময় pelvic pain flare হতে পারে।

EAU guideline early assessment-এ functional, emotional, behavioural, sexual এবং quality-of-life issue evaluate করতে strongly recommend করে।

10. Sexual dysfunction-related trigger

Painful ejaculation CPPS-এ common।

Mechanism:
Pelvic floor spasm + prostate/seminal vesicle contraction + nerve sensitization → post-ejaculatory pain।

Example:

11. Bowel dysfunction / constipation / IBS overlap

Example:
কোষ্ঠকাঠিন্য বাড়লে perineal pressure, rectal pain, urinary frequency বাড়ে।

12. Lifestyle trigger

Common trigger:

  • Spicy food
  • Caffeine
  • Alcohol
  • Dehydration
  • Poor sleep

Shortcut clinical memory

CPPS cause মনে রাখার shortcut: “P-N-I-U-S-P”

  • P = Pelvic floor spasm
  • N = Neuropathic pain / central sensitization
  • I = Inflammation / immune response
  • U = Urinary dysfunction / reflux
  • S = Stress / sexual dysfunction
  • P = Previous infection trigger

Overview / Summary

সবচেয়ে গুরুত্বপূর্ণ mechanisms হলো pelvic floor muscle spasm, neuropathic/central sensitization, abnormal immune-inflammatory response, previous infection trigger, dysfunctional voiding, prostatic reflux, stress/anxiety, এবং sexual/bowel dysfunction overlap।

Actionable Plan / Steps

  1. Cause ভাবার সময় আগে infection/cancer/stone/STI exclude করুন।
  2. Culture negative হলে repeated antibiotic avoid করুন।
  3. Sitting pain + ejaculation pain হলে pelvic floor myalgia খুঁজুন।
  4. Burning/normal report/persistent pain হলে neuropathic sensitization consider করুন।
  5. LUTS থাকলে voiding dysfunction phenotype ধরুন।
  6. Constipation, IBS, stress, sleep disturbance, sexual dysfunction—সব assess করুন।

3. Symptoms — point

একটি multinational CP/CPPS cohort-এ perineal pain সবচেয়ে common ছিল: perineal pain/discomfort 63%, testicular pain 58%, pubic pain 42%, penile pain 32%, painful ejaculation 45%, voiding pain 43%।

Pain symptoms

  • Testicular/scrotal pain
  • Suprapubic / lower abdominal pain
  • Penile tip বা urethral burning
  • Rectal/groin/back pain
  • Pain severity fluctuating—flare ও remission থাকে

  • Fatigue, poor sleep
  • Anxiety, depression, irritability, fear of cancer/infection

Pain symptoms + urinary symptoms + sexual symptoms + psychological/quality-of-life symptoms। NIH-CPSI score-ও CPPS symptoms evaluate করে মূলত pain, urinary symptoms, and quality-of-life impact—এই ৩ domain দিয়ে।

Common pain sites

1. Perineal pain

2. Testicular / scrotal pain

Example:

3. Penile tip / urethral burning pain

4. Suprapubic / lower abdominal pain

5. Rectal / anal / deep pelvic pain

6. Low back / groin pain

CPPS pain

Common pattern:

  • Dull aching pain
  • Burning pain
  • Pressure-like pain
  • Heaviness feeling
  • Stinging urethral pain
  • Deep pelvic discomfort
  • Stress, constipation, ejaculation, spicy food, caffeine, long bike ride

Common urinary symptoms

1. Dysuria /

2. Frequency /

Example:

4. Nocturia /

6. Weak stream /

7. Incomplete emptying sensation

Clinical meaning:

4. Sexual symptoms / যৌন-সম্পর্কিত symptoms

CPPS-এ sexual symptoms খুব common এবং রোগীর quality of life অনেক কমিয়ে দেয়।

Common sexual symptoms

1. Painful ejaculation
বীর্যপাতের সময় বা পরে pelvic/perineal/testicular pain হতে পারে।

2. Erectile dysfunction
Pain anxiety, stress, sympathetic overactivity এবং fear of pain-এর কারণে erection problem হতে পারে।

3. Premature ejaculation

4. Reduced libido

5. Post-ejaculatory flare

Common bowel symptoms

  • Constipation
  • Rectal pressure
  • Pain during defecation
  • IBS-like bloating/cramp

Clinical meaning:

6. Pelvic floor symptoms

Suggestive symptoms

  • Perineal heaviness
  • Rectal pressure
  • Ejaculation-এর পরে pain
  • Urine/stool করার সময় strain
  • DRE করলে pelvic floor muscle tenderness

7. Psychological / quality-of-life symptoms

Common features

  • Anxiety
  • Depression
  • Irritability
  • Poor sleep
  • Fear of cancer/STI/infection
  • Repeated doctor visit
  • Work productivity
  • Relationship problem
  • Sexual avoidance

NIH-CPSI score CPPS-এর quality-of-life impact আলাদা domain হিসেবে measure করে।

Flare-up হতে পারে।

Trigger examples

  • Long sitting
  • Bike/cycling
  • Constipation
  • Stress
  • Poor sleep
  • Spicy food
  • Caffeine
  • Alcohol
  • Dehydration
  • Ejaculation
  • Heavy lower-body exercise

  • Pain ≥
  • Urine culture negative
  • Fever নেই
  • Symptoms
  • Sitting/stress/ejaculation trigger করে
  • Pelvic floor tenderness

  • Fever/chills
  • Acute severe dysuria
  • Urine culture positive
  • Urethral discharge
  • STI risk
  • Acute tender swollen prostate
  • Systemic illness
  • Recurrent documented UTI

10. Red flag symptoms — থাকলে urgent evaluation

  • Fever, chills, toxic look
  • Acute urinary retention
  • Blood in urine
  • Severe sudden testicular pain
  • Testicular swelling/mass
  • Weight loss
  • Night sweat
  • Hard/nodular prostate on DRE
  • New neurological weakness, saddle anaesthesia
  • Age >50 with new severe urinary symptoms
  • Persistent urethral discharge or genital ulcer

“P-U-S-Q”

  • P = Pain: perineum, testis, penis tip, suprapubic, rectal pain
  • U = Urinary: burning, frequency, urgency, weak stream
  • S = Sexual: painful ejaculation, ED, libido
  • Q = Quality of life: anxiety, poor sleep, depression, work/relationship problem

Overview / Summary

CPPS-এর প্রধান symptom হলো ৩ মাসের বেশি pelvic/perineal/prostatic pain, সাথে urinary symptoms, sexual dysfunction, pelvic floor tightness, bowel overlap এবং quality-of-life impairment থাকতে পারে।

Actionable Plan / Steps

  1. Symptoms লিখে NIH-CPSI score করুন।
  2. Pain site identify করুন: perineum, testis, penile tip, suprapubic, rectal।
  3. Urinary symptoms থাকলে urine R/E, culture, PVR, STI NAAT consider করুন।
  4. Painful ejaculation বা sitting pain থাকলে pelvic floor myalgia assess করুন।
  5. Fever/hematuria/retention/testicular swelling থাকলে urgent urology evaluation করুন।
  6. Symptoms flare diary রাখুন: sitting, sex, stress, food, constipation, caffeine trigger note করুন।

4. Clinical Diagnosis

CPPS হলো diagnosis of exclusion। অর্থাৎ infection, cancer, stone, stricture, STI, bladder pain syndrome, epididymo-orchitis, testicular torsion, hernia, anorectal disease, neurological pathology exclude করতে হয়। EAU history, LUTS, sexual function, psychosocial factors এবং validated symptom score ব্যবহার করতে বলে।

Bedside clinical assessment

  • Detailed pain history: site, duration, severity, trigger, ejaculation/urination relation
  • LUTS history: IPSS if urinary symptoms prominent
  • Sexual history: STI risk, discharge, painful ejaculation, ED
  • Bowel history: constipation, IBS, rectal pain
  • Drug history: ketamine, cyclophosphamide, NSAID use, antibiotics history
  • Psychological screen: anxiety, depression, pain fear
  • Examination:
    • Abdomen
    • External genitalia
    • Testis/epididymis
    • Hernia or varicocele check
    • DRE: prostate tenderness, enlargement, nodules
    • Pelvic floor tenderness/trigger points

5. Investigations — specifically

Basic tests

  1. Urine R/E + microscopy
  2. Urine culture & sensitivity
  3. STI NAAT: Chlamydia, Gonorrhoea if urethral discharge, dysuria, risky sexual exposure
  4. CBC, CRP/ESR only if fever/systemic infection suspicion
  5. Serum creatinine before NSAID/long drug use or renal concern
  6. Blood glucose/HbA1c if recurrent infection, neuropathic symptom, diabetes risk
  7. PSA only if age/risk/abnormal DRE; acute infection/inflammation can falsely elevate PSA

Prostatitis-specific tests

  • 2-glass test: pre-massage urine + post-prostatic massage urine
  • 4-glass Meares–Stamey test: gold standard but cumbersome; 2-glass test is more practical.

Symptom score

  • NIH-CPSI: pain, urinary symptoms, quality of life assessment; useful for baseline and follow-up.

Imaging / procedure — not routine

  • USG KUB + PVR: LUTS, retention, recurrent UTI, stone suspicion
  • Scrotal USG: testicular mass, torsion concern, epididymal pain
  • Cystoscopy: hematuria, bladder pain syndrome suspicion, malignancy red flag
  • MRI/CT pelvis: malignancy, abscess, neurological/spinal or complex pelvic disease suspicion

6. Differential Diagnosis

  • Chronic bacterial prostatitis
  • Acute prostatitis
  • UTI / urethritis / STI
  • BPH / bladder outlet obstruction
  • Bladder pain syndrome / interstitial cystitis
  • Urolithiasis
  • Epididymitis, orchitis, testicular torsion
  • Prostate cancer, bladder cancer
  • Pudendal neuralgia
  • IBS, chronic constipation, anal fissure, haemorrhoid, proctalgia
  • Hernia, musculoskeletal low back/hip pathology
  • Female patient হলে: PID, endometriosis, adenomyosis, ovarian cyst/torsion, ectopic pregnancy

7. Complications

  • Chronic pain sensitization
  • Sexual dysfunction: painful ejaculation, ED, avoidance of intercourse
  • Anxiety, depression, sleep disturbance
  • Work productivity ও relationship impairment
  • Unnecessary repeated antibiotics → resistance, adverse effects
  • Recurrent hospital/doctor visits, fear of cancer/infection

8. Management / Treatment — guideline-based

EAU recommends multimodal and phenotype-directed treatment; monotherapy often fails. Pelvic floor physiotherapy, psychological support, symptom-targeted drugs, acupuncture/TENS/neuromodulation in selected cases can be used.

A. Non-pharmacological management

  1. Patient education
    • CPPS usually cancer/infection
    • Reassurance + clear plan anxiety
  2. Pelvic floor physiotherapy
    • Pelvic floor relaxation, trigger point release, breathing, stretching।
    • Kegel exercise avoid if pelvic floor tightness
  3. Lifestyle
    • Long sitting কমানো; soft cushion বা donut cushion।
    • Cycling temporarily avoid বা split saddle।
    • Warm sitz bath 15–20 min/day।
    • Regular walking/light aerobic exercise।
    • Constipation treat করা।
  4. Diet trigger control
    • Caffeine, alcohol, spicy food, acidic drink, carbonated drink reduce if symptoms flare করে।
  5. Psychosexual care
    • Pain-related anxiety, ED, premature ejaculation, relationship distress থাকলে counselling।

9. Pharmacological Treatment — generic, dose, duration, side effect, interaction, BD brand

EAU treatment-naïve, <1 year PPPS-এ quinolone/tetracycline trial উল্লেখ করে, কিন্তু modern practice-এ culture/STI evidence ছাড়া repeat antibiotic avoid করা বেশি safe।

CPPS Drug Selection Shortcut

  • Urinary obstruction: Tamsulosin / Alfuzosin
  • Pain flare: Ibuprofen / Naproxen / Celecoxib short course
  • Burning neuropathic pain + poor sleep: Amitriptyline
  • Neuropathic shooting/burning pain: Gabapentin or selected case Pregabalin

  • Pelvic floor spasm: Baclofen, but specialist-guided
  • STI/culture positive: Doxycycline or Levofloxacin according to organism/sensitivity

Overview / Summary

CPPS drug treatment একক antibiotic-based treatment নয়। Best approach হলো phenotype-based: LUTS থাকলে alpha-blocker, pain flare হলে short NSAID, neuropathic pain হলে amitriptyline/gabapentinoid, urgency-frequency হলে solifenacin, ED/LUTS হলে tadalafil, pelvic floor spasm হলে selected muscle relaxant। Antibiotic শুধু STI, UTI, culture-positive prostatitis বা strong infection evidence থাকলে।

Actionable Plan / Steps

  1. Urine R/E + culture, STI NAAT risk
  2. NIH-CPSI + IPSS score করে baseline রাখুন।
  3. Culture negative হলে repeat antibiotic avoid করুন।
  6. Persistent pain হলে pelvic floor physiotherapy + urologist/pain specialist referral দিন।

Overview / Summary

CPPS হলো chronic pelvic/prostatic pain syndrome, যেখানে usually infection থাকে না। Diagnosis হলো exclusion-based: urine culture, STI testing, DRE, pelvic floor assessment, NIH-CPSI score এবং red flag evaluation জরুরি। Best management হলো UPOINT phenotype-based multimodal treatment—pelvic floor physiotherapy, lifestyle, pain control, alpha-blocker for LUTS, neuropathic agent for burning/central pain, antimuscarinic for urgency, tadalafil for ED/LUTS, antibiotic only if infection proven or strongly suspected.

Actionable Plan / Steps

  1. Baseline: NIH-CPSI + IPSS score
  2. Test: Urine R/E, urine C/S, STI NAAT if risk/discharge; PVR/USG if LUTS.
  3. Exclude red flags: fever, hematuria, retention, cancer suspicion, torsion.
  4. Start non-drug: pelvic floor relaxation PT + sitz bath + sitting/cycling modification + constipation control.
  5. Phenotype drug:
    • LUTS → Tamsulosin/Alfuzosin
    • Pain flare → short NSAID
    • Burning/neuropathic pain/sleep → Amitriptyline or Pregabalin
    • Urgency/frequency → Solifenacin if PVR safe
    • ED + LUTS → Tadalafil
    • Culture positive/STI → appropriate antibiotic
  6. Review after 4–6 weeks: NIH-CPSI improvement ≥6 points হলে continue/step-down; no response হলে urologist + pelvic pain specialist referral.
