TC(888).(20).Sexology Case No 41. Genophobia

Genophobia — বাংলা Clinical Note

Definition:
Genophobia

1. Etiology / Cause —

A. Psychological causes

  1. Past sexual trauma / abuse / rape
    • পূর্বের unwanted sexual experience, abuse, molestation, marital coercion থাকলে brain sex-কে danger হিসেবে condition করে।
    • Result: intimacy দেখলেই fear response, panic, avoidance.
  2. Painful first intercourse / dyspareunia history
  3. Performance anxiety
  4. Religious/cultural guilt
    • sex নিয়ে excessive shame, guilt, taboo, fear of sin/punishment, misinformation থাকলে anxiety তৈরি হতে পারে।
  5. OCD-related sexual fear
    • STD contamination fear, intrusive sexual thoughts, moral/scrupulosity OCD, fear of harming partner—এসব থেকে sex avoidance হতে পারে।
  6. Body image problem
    • নিজের body নিয়ে shame, scars, obesity, genital appearance concern, body dysmorphic symptoms.
  7. Relationship factor
    • Partner conflict, lack of trust, emotional neglect, domestic violence, coercive relationship.

B. Medical / Gynecological causes

  1. Vaginismus / Genito-pelvic pain-penetration disorder
  2. Vaginal dryness / menopause / hypoestrogenism
    • Estrogen কমে গেলে vaginal wall thin/dry হয় → painful sex → fear.
  3. Vulvodynia / vestibulodynia
    • Vulvar burning pain, touch sensitivity, cotton-swab pain.
  4. STI / genital infection
    • Discharge, ulcer, burning, pelvic pain থাকলে sex fear develop হতে পারে।
  5. Endometriosis / PID / chronic pelvic pain
    • Deep dyspareunia থাকলে avoidance.
  6. Male sexual dysfunction
    • Erectile dysfunction, premature ejaculation, Peyronie’s disease, prostatitis, performance anxiety.

Trigger: sex/intimacy/penetration thought
→ Brain danger signal
→ sympathetic activation: tachycardia, sweating, tremor
→ muscle tension/pelvic floor spasm
→ pain
বা panic
→ patient sex avoid
করে
→ avoidance
করলে temporary relief হয়
→ brain
শেখে “avoid করলেই safe”
→ phobia

1. Past Sexual Trauma / Abuse

Core meaning:
আগের trauma → sex = danger memory → genophobia.

2. Painful Intercourse / Dyspareunia History

    • vaginal dryness
    • infection
    • vulvodynia
    • endometriosis
    • pelvic inflammatory disease
    • pelvic floor spasm
  • Genito-pelvic pain/penetration disorder-এ vaginal penetration difficulty, pain, pelvic floor muscle contraction এবং penetration fear থাকতে পারে।

Core meaning:
Painful sex → pain anticipation → fear → avoidance.

3. Vaginismus / Pelvic Floor Muscle Spasm

Example:
Patient

4. Performance Anxiety

  • Female:

Core meaning:
Fail

5. Religious / Cultural Guilt and Shame

  • Sex নিয়ে অতিরিক্ত taboo, shame, guilt, fear-based upbringing থাকলে genophobia হতে পারে।
  • Patient

Core meaning:
Sex

6. Lack of Sex Education / Misinformation

  • Anatomy, consent, lubrication, arousal, pregnancy, STI—
  • Common false beliefs:
    • first sex always extremely painful
    • sex করলে severe bleeding হবেই
    • condom use করলেও STI হবেই
    • pregnancy instantly হয়ে যাবে
    • penis/vagina damage হবে

7. OCD-related Sexual Fear

  • Examples:
    • STD contamination fear despite low risk
    • repeated HIV/STI testing
    • semen/vaginal fluid নিয়ে excessive disgust
    • religious OCD/scrupulosity: sex নিয়ে excessive guilt

Core meaning:
Obsessive thought → anxiety → reassurance/testing/avoidance → genophobia.

8. PTSD / Trauma-related Anxiety

  • Example:
    • childhood abuse
    • domestic violence
    • physical assault
    • traumatic childbirth
    • painful medical pelvic examination
  • NICE PTSD guideline-এ physical/sexual assault, childhood or domestic abuse, serious accidents ইত্যাদিকে PTSD-related traumatic events হিসেবে উল্লেখ করা হয়েছে।

Core meaning:
Trauma memory active

9. Relationship Problem / Lack of Trust

  • Causes:
    • partner rude/forceful
    • communication problem
    • past betrayal
    • domestic violence
    • fear of rejection
    • pressure for sex

Clinical clue:
Patient

10. Body Image Problem

  • Causes:
    • obesity concern
    • scars/stretch marks
    • breast/genital size concern
    • skin disease
    • body dysmorphic disorder

Core meaning:
Body shame → exposure fear → intimacy avoidance.

11. Depression and Low Self-esteem

  • Patient intimacy enjoy

12. Anxiety Disorder / Panic Disorder

  • Patient

13. STI / Pregnancy Fear

  • Especially:
    • previous STI history
    • unwanted pregnancy history
    • poor contraceptive knowledge
    • partner fidelity concern
    • repeated online searching

Core meaning:
Risk

14. Female Gynecological Causes

Important causes:

  • Vaginitis: itching, discharge, burning
  • Candidiasis: curdy white discharge, itching
  • Bacterial vaginosis: fishy smell discharge
  • STI: ulcer, discharge, pelvic pain
  • Vulvodynia: vulvar burning pain
  • Endometriosis: deep dyspareunia, dysmenorrhea
  • PID: lower abdominal pain, cervical motion tenderness
  • Menopause/GSM: dryness, burning, painful sex
  • Postpartum pain: episiotomy scar, dryness, fear after delivery

Core meaning:
Physical pain/infection/dryness untreated
থাকলে sex fear naturally develop করে।

15. Male Sexual or Urological Causes

  • Erectile dysfunction
  • Premature ejaculation
  • Penile pain
  • Peyronie’s disease
  • Prostatitis
  • Past failed sexual attempt
  • Porn-induced unrealistic expectation
  • Fear of infertility
  • Fear of partner dissatisfaction

Core meaning:
Repeated sexual difficulty → embarrassment → performance anxiety → sex fear.

16. Painful Medical Experience

17. Strict or Fear-based Upbringing

18. Previous Relationship Betrayal or Emotional Trauma

19. Pornography-related Unrealistic Expectation

  • Excessive porn exposure বা unrealistic sexual performance expectation anxiety বাড়াতে পারে।
  • Male: erection/performance pressure
  • Female: body comparison, pain fear, performance shame
  • Result: real intimacy নিয়ে fear.

Common combination:

  • Painful first intercourse + vaginismus + anxiety
  • Sexual trauma + PTSD + relationship fear
  • Religious guilt + lack of sex education + performance anxiety
  • Vaginal dryness/infection + dyspareunia + avoidance
  • ED/premature ejaculation + shame + anxiety

Clinical Shortcut

  2. Past sexual trauma/abuse
  3. Pain, discharge, dryness, ulcer, pelvic pain

Overview / Summary

Actionable Plan / Steps

  1. প্রথমে cause category বের করুন: psychological, trauma, pain/medical, relationship, OCD/anxiety।
  2. Pain/discharge/dryness থাকলে gynecological/urological evaluation করুন।
  3. Trauma থাকলে PTSD screening করুন।
  5. Main treatment cause অনুযায়ী: CBT/sex therapy/pelvic floor therapy/trauma therapy।

3. Clinical Features / Symptoms —

A. Psychological symptoms

  • Excessive reassurance seeking

1. Mental / Emotional Symptoms

  • Patient
    “Sex

2. Panic feeling

4. Disgust বা repulsion

  • Sexual touch, semen/vaginal fluid, genital contact বা intercourse-এর idea disgust তৈরি করতে পারে।
  • OCD/contamination fear

5. Catastrophic thought

  • “STI/HIV হবেই”
  • “Pregnancy হয়ে যাবে”
  • “Partner আমাকে judge করবে”
  • এসব thought fear cycle বাড়ায়।

2. Physical Anxiety Symptoms

Common physical symptoms

  • Palpitation / heart beat
  • Chest tightness
  • Shortness of breath
  • Sweating
  • Tremor /
  • Dry mouth
  • Nausea /
  • Dizziness /
  • Abdominal discomfort
  • Hot flush বা cold feeling
  • Muscle tightness
  • Fatigue after anxiety episode

Clinical meaning:

3. Behavioral Symptoms

1. Avoidance

  • Sex, intimacy, dating, marriage discussion, physical closeness avoid
  • Bedroom situation avoid করা।

2. Excuse

3. Reassurance seeking

4. Avoidance after failed attempt

4. Sexual Symptoms

1. Low libido secondary to fear

  • Fear libido suppress করে।

2. Arousal problem

  • Male: erection maintain

3. Orgasm difficulty

4. Intercourse incomplete

5. Female-specific Symptoms

Important female symptoms

  • Vaginal opening tight
  • Penis/finger/speculum/tampon ঢুকাতে difficulty

  • Gynecological exam tolerate

Typical patient statement:
“

6. Male-specific Symptoms

Common male symptoms

  • Premature ejaculation fear
  • Partner satisfy
  • Condom/STI/pregnancy fear
  • Porn comparison বা penis size anxiety

Clinical meaning:
Male genophobia

7. Trauma-related Symptoms

Trauma-related signs

  • Sexual touch-এ flashback
  • Nightmare
  • Body freeze response
  • Crying during intimacy
  • Partner safe হলেও unsafe feel করা
  • Hypervigilance: সবসময় alert থাকা
  • Startle response
  • Sleep disturbance
  • Irritability
  • Emotional numbness

Red flag:
Intimacy-

8. OCD-related Symptoms

Clues

Core meaning:
Fear

9. Relationship Symptoms

Common signs

  • Partner-এর প্রতি emotional distance
  • Touch avoid করা
  • Irritability or anger when partner initiates intimacy
  • Misunderstanding: partner
  • Marital conflict
  • Trust issue
  • Consent/pressure থাকলে fear আরও বাড়ে

Clinical clue:
Patient

10. Symptoms Severity অনুযায়ী Classification

Mild

  • Sex নিয়ে nervousness
  • কিছু avoidance আছে
  • Relationship functioning

Moderate

  • Intercourse avoid
  • Panic/anxiety repeated
  • Relationship conflict
  • Pain anticipation prominent

  6. Severe symptoms, abuse, suicidal thought, flashback বা relationship violence থাকলে urgent mental health referral দিন।

4. Clinical Diagnosis — কীভাবে diagnosis করবেন

Genophobia diagnosis মূলত clinical history-based। কোনো single blood test নেই।

Diagnostic approach

A. Key history

  1. Fear
    • Intercourse?
    • Penetration?
    • Touch?
    • Nudity?
    • STD fear?
    • Pregnancy fear?
    • Trauma reminder?
  2. Duration
    • ≥6 months হলে specific phobia criteria support করে।
  3. Severity
    • Relationship, marriage, sexual life, mental health impair
  4. Avoidance
    • Patient কি sex/intimacy avoid করছে?
  5. Pain history
    • Female: superficial/deep dyspareunia, vaginal dryness, discharge, burning
    • Male: ED, penile pain, prostatitis symptoms
  6. Trauma history
    • Sexual abuse, forced sex, domestic violence
  7. Psychiatric screening
    • PTSD, OCD, depression, GAD, panic disorder

5. Investigations — specific

A. Psychiatric assessment tools

6. Differential Diagnosis

ConditionKey clue
Specific phobiaSex/intimacy-specific fear, avoidance, ≥6 months
PTSDTrauma history, flashback, nightmare, hyperarousal
OCDIntrusive sexual/contamination thoughts + compulsions
Panic disorderUnexpected panic attacks, not only sex-triggered
Social anxiety/body dysmorphic disorderBody shame, fear of judgment
DepressionLow libido + anhedonia + low mood
Vaginismus/GPPPDPenetration fear + pelvic floor spasm/pain
Dyspareunia due to infectionDischarge, burning, pelvic pain
Menopause/GSMVaginal dryness, painful sex
Erectile dysfunction/performance anxietyMale erection failure linked to fear
Intimate partner violenceFear due to coercion/abuse

7. Complications — রোগের জটিলতা

  • Relationship breakdown
  • Marital conflict
  • Infertility concern due to avoidance
  • Depression/anxiety worsening
  • Panic attacks
  • Low self-esteem
  • Sexual dysfunction
  • Partner avoidance/misunderstanding
  • Trauma symptoms chronic
  • Female ক্ষেত্রে untreated vaginismus/pain disorder chronic pelvic pain তৈরি করতে পারে

8. Treatment / Management — Guideline-based

Core principle

Genophobia-এর main treatment হলো psychotherapy. Medicine primary treatment নয়, unless comorbid anxiety, PTSD, panic disorder, depression, OCD, severe anticipatory anxiety, or pain disorder থাকে। Specific phobia-তে exposure-based CBT best-supported treatment; GPPPD/vaginismus-এ pelvic floor physiotherapy + CBT/sex therapy recommended।

A. Non-pharmacological treatment

1. Psychoeducation

  • Sex নিয়ে misinformation correct করা
  • Consent, safety, anatomy, normal sexual response explain করা
  • “Fear response” normal physiological response—

2. CBT — Cognitive Behavioral Therapy

Goal: catastrophic thought

Example:
“Sex

Overview / Summary

Main treatment হলো CBT + graded exposure + sex therapy + pelvic floor physiotherapy।

Actionable Plan / Steps

  1. Fear trigger identify করুন: sex, penetration, pain, STD fear, trauma, body image, performance anxiety.
  2. Pain/discharge/ulcer/dryness থাকলে gynecology/urology exam করুন।
  3. Psychiatric screening: GAD-7, PHQ-9, PCL-5, Y-BOCS.
  4. First-line: CBT + graded exposure + couple/sex therapy.
  5. Female penetration pain হলে pelvic floor physiotherapy + dilator training.
  6. PTSD থাকলে trauma-focused CBT/EMDR.
  7. Medicine only if indicated: SSRI/SNRI for comorbid anxiety/PTSD/OCD/depression; avoid long-term benzodiazepine.
  8. Follow-up: every 2–4 weeks initially; suicidal ideation, abuse, severe depression থাকলে urgent referral.
