TC(873).(20).Sexology Case No 26. Hepatitis B

1. Definition /

WHO বলছে HBV infected blood, semen, vaginal fluid, saliva/body fluid এবং mother-to-child route-এ ছড়াতে পারে।

2. Cause / Etiology —

A. মূল কারণ

WHO ও CDC অনুযায়ী HBV মূলত infected body fluid শরীরে ঢুকলে ছড়ায়।

Core meaning:

2. Unprotected sexual contact

Example:

3. Mother-to-child transmission

Core meaning:

4. Infected blood exposure

  • Unsafe blood transfusion, contaminated blood product, accidental blood exposure risk factor।

Example:

5. Shared needle / syringe

Core meaning:

6. Unsafe medical, dental বা surgical procedure

Example:

7. Needle-stick injury / Occupational exposure

  • Doctor, nurse, dentist, lab technician, cleaner, OT staff—এদের occupational risk আছে।

Core meaning:
Healthcare setting-এ infected needle injury হলে HBV infection হতে পারে, তাই vaccination mandatory।

8. Tattoo, piercing, ear/nose piercing

  • Beauty parlour, barber shop, informal tattoo setup-এ sterilization poor হলে risk বেশি।

Example:

9. Razor, toothbrush, nail cutter sharing

Core meaning:

10. Household close contact

  • Household members should be tested and vaccinated।

Example:

12. Immunosuppression-related HBV reactivation

  • Chemotherapy
  • Rituximab
  • High-dose steroid
  • Biologic therapy
  • Organ transplant immunosuppression
  • HIV infection
  • Uncontrolled stopping of antiviral

Core meaning:

13. High-risk groups

  • HBsAg-positive mother-এর baby
  • HBV-positive patient-এর sexual partner
  • Same household member
  • Healthcare worker
  • Dialysis patient
  • Injection drug user
  • Multiple sexual partner
  • Men who have sex with men
  • Prison population
  • HIV/HCV infected patient
  • Immunosuppressed patient
  • Unvaccinated person living in high-prevalence area

  • coughing/sneezing

Shortcut: Cause

HBV spreads by “B-S-M-N”

  • B = Blood
  • S = Sex
  • M = Mother to baby
  • N = Needle / non-sterile instrument

  • N = Nausea/Vomiting
  • J = Jaundice, Joint pain
  • U = Urine dark
  • D = Dark urine / pale stool

Overview / Summary

Actionable Plan / Steps

  1. Symptom থাকলে শুধু clinical diagnosis নয়—HBsAg, anti-HBc IgM, anti-HBs, ALT/AST, bilirubin, PT/INR করতে হবে।
  2. HBsAg positive হলে HBV DNA, HBeAg, anti-HBe, ultrasound, FibroScan/APRI/FIB-4 করতে হবে।
  3. Confusion, bleeding, severe jaundice, ascites, blood vomiting থাকলে urgent referral।

  • Unprotected sexual contact: infected partner-এর semen/vaginal secretion/body fluid থেকে।
  • Blood exposure: infected blood transfusion, unsafe injection, shared needle/syringe, dialysis exposure।
  • Needle-stick injury: doctor/nurse/lab worker/dentistদের occupational risk।
  • IV drug use: shared syringe সবচেয়ে high-risk route।

5. Clinical Diagnosis & Investigation — Specific

A. Clinical suspicion

History

  • Sexual exposure
  • Injection/drug use
  • Blood transfusion
  • Dental/surgical/tattoo exposure
  • Family history of HBV/HCC
  • Pregnancy status
  • Previous vaccination
  • Occupational needle-stick
  • Alcohol, hepatotoxic drugs, herbal medicine

Physical exam:

  • Jaundice, hepatomegaly, RUQ tenderness
  • Chronic liver disease signs: spider angioma, palmar erythema, ascites, splenomegaly
  • Encephalopathy signs: asterixis, confusion

B. Diagnostic blood tests

Initial HBV serology panel

CDC recommends HBsAg, anti-HBs, total anti-HBc; IgM anti-HBc helps identify acute/recent infection.

TestMeaning
HBsAgCurrent HBV infection—acute or chronic
Anti-HBsImmunity after vaccine or past infection
Total anti-HBcPast or current natural HBV exposure
IgM anti-HBcAcute/recent infection; flare of chronic HBV-তেও positive হতে পারে
HBeAgHigh viral replication/infectivity marker
Anti-HBeLower replication phase, but HBV DNA must be checked
HBV DNA quantitative PCRViral load; treatment decision ও monitoring
HBsAg quantitativeকিছু ক্ষেত্রে disease phase/monitoring-এ helpful

Interpretation shortcut

  • HBsAg positive + IgM anti-HBc positive → Acute HBV likely
  • HBsAg positive >6 months → Chronic HBV
  • HBsAg negative + anti-HBs positive + anti-HBc negative → Vaccine-induced immunity
  • HBsAg negative + anti-HBs positive + anti-HBc positive → Past natural infection/recovery
  • All negative → Susceptible; vaccine needed

C. Liver severity assessment

  • ALT, AST
  • Total/direct bilirubin
  • ALP, GGT
  • Albumin
  • PT/INR
  • CBC with platelet
  • Serum creatinine/eGFR
  • Serum phosphate if tenofovir planned
  • Blood glucose/HbA1c, lipid profile when relevant
  • Pregnancy test if applicable

D. Co-infection screen

  • Anti-HCV
  • HIV Ag/Ab
  • Anti-HDV ± HDV RNA if HBsAg positive
WHO 2024 guideline highlights HDV testing among chronic HBV patients, because HBV-HDV co-infection progresses faster.

E. Fibrosis/cirrhosis assessment

  • Ultrasound whole abdomen
  • FibroScan / transient elastography
  • APRI / FIB-4
  • Liver biopsy: now rarely needed; unclear casesে specialist decision।

F. HCC surveillance

Chronic HBV with cirrhosis or high-risk status needs ultrasound ± AFP every 6 months. AASLD HCC guidance recommends semiannual surveillance intervals.

6. Differential Diagnosis

  • Hepatitis A, C, D, E
  • Alcoholic hepatitis
  • Drug-induced liver injury—paracetamol, anti-TB drugs, herbal/toxic drugs
  • Autoimmune hepatitis
  • Wilson disease—young patient
  • NAFLD/NASH
  • Acute cholangitis/obstructive jaundice
  • Leptospirosis, dengue hepatitis, malaria-related hepatitis
  • Ischemic hepatitis
  • Hemolytic jaundice

7. Complications — বাংলায় ব্যাখ্যা

1. Acute liver failure

2. Chronic Hepatitis B

3. Cirrhosis

4. Hepatocellular carcinoma—HCC

5. Portal hypertension

Cirrhosis হলে portal pressure বাড়ে → splenomegaly, thrombocytopenia, esophageal varices, ascites।

6. Reactivation

Chemotherapy, steroid, biologics, rituximab, transplant immunosuppression, uncontrolled stopping of antiviral—এসব ক্ষেত্রে dormant HBV আবার active হয়ে severe hepatitis করতে পারে।

7. Extra-hepatic complications

  • Polyarteritis nodosa
  • Membranous nephropathy / glomerulonephritis
  • Serum sickness-like syndrome—fever, rash, joint pain

8. Treatment Management — Guideline-based

A. Acute Hepatitis B

Mild/moderate acute HBV

  • Rest, hydration, nutrition।
  • Alcohol completely avoid।
  • Hepatotoxic drug avoid।
  • ALT/AST, bilirubin, INR monitoring।
  • Severe vomiting, high INR, encephalopathy হলে hospital referral।

Severe acute HBV / acute liver failure

  • Hepatology referral.
  • Tenofovir or entecavir may be used under specialist care.
  • Liver transplant center consultation if acute liver failure.

B. Chronic Hepatitis B — কাকে treatment দেব?

WHO 2024 updated guideline treatment eligibility broaden করেছে: significant fibrosis থাকলে viral load/ALT যাই হোক treatment; HBV DNA >2,000 IU/mL + ALT above ULN; co-infection/family history of liver cancer/cirrhosis/immunosuppression/comorbidity থাকলে treatment; HBV DNA unavailable হলে persistently abnormal ALT-তেও treatment consideration। WHO preferred regimen হিসেবে TDF বা ETV এবং renal/bone risk থাকলে selected ক্ষেত্রে TAF উল্লেখ করেছে।

4. Blood/needle safety

  • Needle/syringe share
  • Tattoo/piercing only sterile setup।
  • Razor/toothbrush/nail cutter share
  • Universal precaution in clinic/lab/dental setting।

5. Infected patient-এর counselling

  • Alcohol avoid.
  • Household/sexual contacts test + vaccinate.
  • Blood donation
  • Antiviral stop করা যাবে না without clinician.
  • Cirrhosis/HCC surveillance follow-up দরকার।

11. Shortcut Clinical Algorithm

HBsAg positive → repeat/confirm + anti-HBc IgM/total + anti-HBs
If acute symptoms + IgM anti-HBc positive → acute HBV → supportive ± severe হলে antiviral/referral
If HBsAg >6 months → chronic HBV → HBV DNA + ALT + HBeAg/anti-HBe + fibrosis assessment
If cirrhosis/significant fibrosis OR HBV DNA >2,000 + ALT high OR co-infection/family HCC/immunosuppression → treat
Preferred drug → TDF or Entecavir
Renal/bone risk → consider TAF or Entecavir
Cirrhosis/high HCC risk → USG liver ± AFP every 6 months

1. Overview / Summary

2. Actionable Plan / Steps

  1. Initial tests: HBsAg, anti-HBs, total anti-HBc, IgM anti-HBc, ALT/AST, bilirubin, PT/INR, albumin, CBC, creatinine/eGFR।
  2. If HBsAg positive: HBV DNA quantitative, HBeAg, anti-HBe, anti-HDV, anti-HCV, HIV test, ultrasound, FibroScan/APRI/FIB-4।
  3. Assess severity: INR, bilirubin, encephalopathy, ascites/variceal bleed থাকলে urgent referral।
  4. Treatment decision: WHO criteria অনুযায়ী fibrosis/cirrhosis, HBV DNA, ALT, co-infection, family HCC, pregnancy, renal/bone risk দেখে।
  5. First-line drug: TDF 300 mg OD অথবা Entecavir 0.5 mg OD; renal/bone riskে TAF 25 mg OD with food consider।
  6. Prevention: contacts test + vaccinate; newborn prophylaxis; safe sex; needle safety; alcohol/hepatotoxic drug avoid।
  7. Monitoring: HBV DNA/ALT 3–6 monthly initially; renal function for tenofovir; HCC surveillance every 6 months in high-risk/cirrhosis।
