TC(860).(20).Sexology Case No 13. Genito-pelvic pain/penetration disorder

Genito-pelvic pain/penetration disorder —

Cause Explain :-

Genito-pelvic pain/penetration disorder — Causes /

1) Vaginal dryness / lubrication

    • Menopause
    • Breastfeeding
    • Postpartum period
    • Foreplay
    • Anxiety বা fear
    • কিছু antidepressant/psychotropic drug

2) Menopause-related vaginal atrophy

3) Pelvic floor muscle spasm / vaginismus

4) Vulvodynia / vestibulodynia

10) Postpartum or delivery-related scar

11) Previous surgery or radiation

  • Pelvic surgery, vaginal surgery, hysterectomy, cancer radiation-এর পর fibrosis/scarring হতে পারে।

  • Partner pressure, poor communication, lack of emotional safety থাকলে pain cycle worsen হয়।

Core Meaning

  1. Pain source আছে — infection, atrophy, endometriosis, PID, skin disease
  2. Muscle tight হয়ে যায় — pelvic floor spasm/vaginismus
  3. Fear/anxiety pain বাড়ায় — pain–fear–spasm cycle
  4. Vaginal dryness/lubrication কম — menopause, lactation, low arousal

Shortcut

Entry pain

  • Vaginal dryness
  • Candida/BV/STI
  • Vulvodynia
  • Vaginismus
  • Vulvar skin disease
  • Scar / Bartholin cyst

Deep pain

  • Endometriosis
  • PID
  • Ovarian cyst
  • Fibroid
  • Pelvic mass
  • Bladder/bowel pain syndrome

Summary

Symptopms Explain:-

Genito-pelvic pain/penetration disorder — Symptoms /

4) Pelvic floor muscle tight/spasm

  1. Penetration difficulty
  2. Penetration pain
  3. Pain-এর ভয়/anxiety
  4. Pelvic muscle tight/spasm

Deep pain
হলে uterus/ovary/pelvic cause ভাববেন।

3. Clinical Diagnosis — কীভাবে diagnose করবেন

DSM-5-TR clinical criteria

  • Penetration pain নিয়ে marked fear/anxiety।

4. Clinical evaluation — specific history + examination

History

  • Pain location: entry না deep?
  • Onset: first intercourse
  • Duration: ≥
  • Pain character: burning, stabbing, tearing, cramping।
  • Triggers: intercourse, tampon, speculum, urination, cycling, tight clothes।
  • Discharge/itching/odor/ulcer/bleeding
  • Menstrual history: dysmenorrhea, heavy bleeding, infertility।
  • Postpartum, breastfeeding, menopause, pelvic surgery history।
  • STI risk, contraception, condom/lubricant allergy।
  • Sexual trauma/intimate partner violence screening।
  • Anxiety/depression, relationship distress, medication history।

Physical examination

  • Patient-centered, consent-based, step-by-step exam।
  • External vulvar inspection: erythema, fissure, ulcer, scar, atrophy, lichen sclerosus, Bartholin swelling।
  • Cotton swab/Q-tip test: vestibule pain mapping—provoked vestibulodynia identify করতে useful।
  • Single-digit vaginal exam: pelvic floor tenderness/hypertonicity।
  • Speculum exam: discharge, cervicitis, atrophy, lesion—patient tolerate
  • Bimanual exam: cervical motion tenderness, uterine/adnexal tenderness, mass, endometriosis/PID suspicion।
    AAFP dyspareunia guidance
অনুযায়ী safe sexual history, cotton-swab pressure test, single-digit exam এবং bimanual exam cause localization-এ গুরুত্বপূর্ণ।

5. Investigations — specific tests

Basic tests

  • Pregnancy test — reproductive-age female, pelvic pain/bleeding
  • Urine R/E + culture — dysuria, frequency, bladder pain
  • CBC, CRP/ESR — fever, PID, systemic infection suspicion।
  • Blood glucose/HbA1c — recurrent candidiasis হলে diabetes screen।

Vaginal/cervical infection work-up

  • Vaginal pH।
  • Saline wet mount: clue cells, trichomonads, WBC।
  • 10% KOH microscopy: Candida hyphae/pseudohyphae।
  • Whiff test for BV।
  • Candida culture/PCR — recurrent/complicated/non-albicans suspected
  • NAAT for Chlamydia, Gonorrhea, Trichomonas।
    CDC

Imaging / special tests

  • Transvaginal ultrasonography — ovarian cyst, fibroid, adenomyosis, pelvic mass।
  • MRI pelvis — deep endometriosis/adenomyosis complex case।
  • Diagnostic laparoscopy — endometriosis/PID complication suspected but imaging inconclusive।
  • Vulvar biopsy — lichen sclerosus, suspicious lesion, non-healing ulcer, malignancy suspicion।
  • Pap smear/HPV test — screening indication অনুযায়ী; BV/trichomoniasis diagnosis-এর জন্য Pap reliable diagnostic test নয়।

6. Differential diagnosis

  • Vulvodynia / provoked vestibulodynia
  • Vaginismus / levator ani syndrome
  • Vulvovaginal candidiasis
  • Bacterial vaginosis
  • Trichomoniasis / cervicitis / STI
  • Genital herpes
  • Lichen sclerosus / lichen planus / eczema
  • Genitourinary syndrome of menopause / atrophic vaginitis
  • Endometriosis / adenomyosis
  • PID
  • Fibroid / ovarian cyst / pelvic mass
  • Bartholin cyst/abscess
  • UTI / interstitial cystitis
  • IBS / constipation-related pelvic pain
  • Postpartum scar pain / episiotomy scar
  • Sexual trauma-related pain/anxiety disorder

7. Management / Treatment — guideline-based

Core principle

GPPPD-এর treatment cause-specific + multidisciplinary। Common effective approach: education, vulvar care, lubricant, pelvic floor physiotherapy, graded dilator/desensitization, CBT/sex therapy, and underlying medical cause treatment। MSD/AAFP দুটোই treatment of cause, pelvic floor physical therapy, progressive desensitization, psychological therapy, vaginal estrogen/topical analgesic ইত্যাদি recommend করে।

Non-pharmacological management

  • Pain education: “Pain real, treatable, and not patient’s fault.”
  • Temporary avoidance of forced penetration until pain cycle controlled।
  • Water-based, fragrance-free lubricant during intercourse।
  • Vulvar care: cotton underwear, avoid douching, perfume soap, vaginal deodorant।
  • Pelvic floor physiotherapy: relaxation training, biofeedback, trigger point release, myofascial release।
  • Graded vaginal dilator therapy: smallest dilator → gradual progression, pain-free pace।
  • CBT / psychosexual therapy: fear-avoidance, trauma, relationship distress, performance anxiety।
  • Couple counselling: partner education, non-penetrative intimacy, pressure-free sexual activity।
  • Treat comorbidities: anxiety, depression, IBS, bladder pain syndrome, endometriosis।
A multicenter RCT in provoked vestibulodynia found multimodal physical therapy reduced intercourse pain more effectively than topical lidocaine, supporting pelvic floor physiotherapy as a key first-line option.

8. Pharmacological treatment — cause-specific drug list

  • Painful penetration forcefully continue করবেন না; pain cycle worse হয়।
  • Adequate arousal + water-based lubricant
  • Douching, scented soap, vaginal deodorant, antiseptic wash avoid।
  • Recurrent discharge/itching হলে self-treatment না করে microscopy/pH/NAAT করে treat।
  • STI prevention: condom, partner testing/treatment, multiple partner risk counselling।
  • Menopause/lactation-related dryness হলে early lubricant/moisturizer ± local estrogen।
  • Postpartum perineal pain থাকলে early pelvic floor rehab।
  • Chronic pelvic pain/dysmenorrhea/endometriosis symptoms থাকলে early gynecology referral।
  • Anxiety/trauma/relationship distress থাকলে psychosexual counselling delay করবেন না।
  • Tight underwear, irritant pads, prolonged moisture avoid।

10. Clinical shortcut

Entry pain + burning + Q-tip positive → Vulvodynia/vestibulodynia → pelvic floor physiotherapy + vulvar care ± lidocaine/neuropathic pain drug.

Dryness + menopause/lactation + atrophy → GSM/atrophy → lubricant/moisturizer + vaginal estrogen.

Itching + curdy discharge + pH <4.5 → Candida → fluconazole/clotrimazole.

Fishy odor + pH >4.5 + clue cells → BV → metronidazole.

Frothy discharge/STI risk + NAAT positive → Trichomoniasis → metronidazole + partner treatment.

Deep pain + dysmenorrhea/chronic pelvic pain → Endometriosis/PID/pelvic mass → USG ± laparoscopy/gynecology referral.

Overview / Summary

GPPPD হলো penetration-related pain, fear/anxiety এবং pelvic floor tightening-এর biopsychosocial disorder। Diagnosis purely “symptom label” নয়—প্রথমে infection, atrophy, vulvodynia, pelvic floor dysfunction, endometriosis, PID, dermatologic disease exclude/identify করতে হবে। Best outcome আসে cause-specific drug + pelvic floor physiotherapy + graded desensitization + CBT/psychosexual therapy combined করলে।

Actionable Plan / Steps

  1. Pain map করুন: entry vs deep।
  2. Red flag দেখুন: fever, pregnancy, severe pelvic pain, abnormal bleeding, ulcer/mass, cervical motion tenderness।
  3. Exam: vulvar inspection → Q-tip test → single-digit pelvic floor exam → speculum/bimanual as tolerated।
  4. Tests: pregnancy, urine, vaginal pH/wet mount/KOH, Candida culture if recurrent, STI NAAT, TVUS if deep pain।
  5. Treat cause: Candida/BV/STI/GSM/PID/endometriosis/dermatosis।
  6. Start pelvic floor physiotherapy + lubricant + vulvar care।
  7. Persistent vulvodynia/neuropathic pain হলে specialist-led lidocaine/amitriptyline/gabapentin/duloxetine।
  8. 6–8 weeks follow-up; refractory case → gynecologist + pelvic pain/psychosexual specialist referral।
