Menopausal Depression
Cause / Etiology
- Hormonal changes:
- Psychosocial factors:
- Medical comorbidities:
- Hypothyroidism
- Chronic illness (diabetes, cardiovascular disease)
- Previous psychiatric history:
Clinical Features / Symptoms
- Irritability, anxiety
- Physical complaints: headache, joint pain, palpitations
- Rarely suicidal ideation
Clinical Diagnosis & Investigation
Diagnosis
- Clinical diagnosis মূলত history & symptom-based
- Menopausal status যাচাই: hot flushes, amenorrhea >12 months
- Depression screening:
- PHQ-9 (Patient Health Questionnaire-9)
- Hamilton Depression Rating Scale (HAM-D)
Investigations (Rule out secondary causes)
- Basic labs:
- CBC, blood sugar, renal & liver function
- Thyroid function test: TSH, T3, T4 (hypothyroidism mimic
- Hormonal assay (optional): FSH, LH, estradiol
- Vitamin D / B12: deficiency হলে mood affect করে
Management / Treatment

-
- Dose: 20–40 mg/day orally
- Duration: 6–12 months (symptoms improve না হলে adjust)
- Purpose: Serotonin reuptake inhibit → mood improve
- Side effects: nausea, headache, insomnia, sexual dysfunction

- Sertraline
- Tablet 2.Repose 50 Mg AS A Sertraline Hydrochloride AS A SSRIs & related anti-depressant drugs.
- Dose: 50–100 mg/day orally
- Purpose: safer for cardiac patients
- Side effects: diarrhea, insomnia, tremor

- Side effects: nausea, sexual dysfunction, dizziness
- SNRIs (Serotonin–Norepinephrine Reuptake Inhibitors)
- Venlafaxine
- Tablet 4.Venlax 75 mg AS A Venlafaxine Hydrochloride AS A (SNRI).
- Dose: 75–150 mg/day orally
- Purpose: hot flush + depression dual benefit
- Side effects: hypertension, nausea, insomnia
- Interactions: MAOIs, CYP2D6 inhibitors
- Hormone Therapy (HT) / Estrogen Therapy
- Estradiol (oral or transdermal patch)
- Tablet 5.Gynova 2mg AS A Estradiol Female Sex hormones.
- Dose: 0.5–2 mg/day oral or 0.025–0.05 mg patch
- Purpose: hot flush & mood stabilization
- Side effects: breast tenderness, nausea, thromboembolism risk
- Interactions: CYP3A4 inducers → decreased efficacy
- Adjunct / Symptomatic
- Low dose benzodiazepine (e.g., Lorazepam 0.5–1 mg/night)
- Purpose: sleep disturbances & anxiety
- Side effects: sedation, dependence (short-term use only)
- Low dose benzodiazepine (e.g., Lorazepam 0.5–1 mg/night)
Note: Drug choice নির্ভর করে comorbidities, previous response, drug interaction, symptom profile (e.g., hot flush + depression → Venlafaxine better)
2. Non-Pharmacological Management
- Cognitive Behavioral Therapy (CBT)
- Lifestyle modification: regular exercise, yoga, meditation
- Sleep hygiene: avoid caffeine, maintain sleep routine
- Social support: family, friends, support groups
Prevention / Risk Reduction
- Early recognition of mood changes
- Hormone replacement therapy যখন indicated
- Stress management: meditation, counseling
- Regular physical activity
- Adequate nutrition, vitamin D & B12 correction
- Sleep hygiene
- Avoid alcohol, smoking

Notes:
- Choice of drug depends on symptom profile:
- Hot flush prominent → Venlafaxine or Estradiol preferred
- Anxiety/sleep issues → SSRI + short-term Lorazepam
- Cardiac comorbidity → Sertraline safer
- Monitoring: Blood pressure, liver function (especially with SNRIs), mood changes, side effects
- Follow-up: 4–6 weeks for symptom evaluation; dose adjustment if inadequate response
