TC(817).(06).Psychiatry Case No 28. Depressive neurosis

Etiology / Causes (কারণ)

  1. Biological factors (জৈবিক কারণ):
    • Serotonin, norepinephrine, dopamine-এর অসমতা
    • Genetic predisposition (পরিবারে depression থাকলে ঝুঁকি বেশি)
  2. Psychological factors (মনস্তাত্ত্বিক কারণ):
    • Chronic stress, childhood trauma
    • Personality traits: low self-esteem, perfectionism
  3. Social / Environmental factors (সামাজিক/পরিবেশগত কারণ):
    • Occupational stress
    • Family conflicts, social isolation
    • Financial difficulties

Clinical Features / Symptoms (লক্ষণ)

  • Persistent sadness / melancholy
  • Anxiety, tension
  • Low self-esteem, guilt feelings
  • Fatigue, lethargy
  • Sleep disturbance (insomnia or hypersomnia)
  • Poor concentration
  • Loss of interest in daily activities (anhedonia)
  • Somatic complaints: headache, backache, digestive issues
  • Suicidal ideation (rare, mild compared to major depression)

Clinical Diagnosis & Investigations (ক্লিনিকাল ডায়াগনোসিস ও পরীক্ষা)

  1. Clinical Diagnosis:
    • History & mental status examination
    • DSM-5 criteria for Persistent Depressive Disorder:
      • Depressed mood ≥ 2 years (adult)
      • Two or more of: poor appetite/overeating, sleep disturbance, low energy, low self-esteem, poor concentration, hopelessness
      • No major depressive episode lasting > 2 months
  2. Investigations (Rule out other causes):
    • CBC (anemia, infection check)
    • Thyroid function test (hypothyroidism mimic)
    • Blood glucose / metabolic panel
    • Vitamin B12 / Folate if indicated
    • ECG if cardiovascular risk

Note: Mostly diagnosis is clinical, investigations are for exclusion of secondary causes.

Treatment / Management (চিকিৎসা ও ব্যবস্থাপনা):-

A. Pharmacological treatment (ড্রাগ থেরাপি)

  • লক্ষ্য: Mood stabilization, energy increase, anxiety কমানো, function recovery

1. Selective Serotonin Reuptake Inhibitors (SSRIs)

  • Fluoxetine

2. Serotonin-Norepinephrine Reuptake Inhibitors (SNRIs)

  • Venlafaxine

    • Dose: 25–75 mg/day (start low, titrate)
    • Purpose: Chronic depression, sleep disturbance
    • Side effects: Anticholinergic (dry mouth, constipation), sedation, cardiac arrhythmia
    • Interactions: MAO inhibitors, alcohol, CNS depressants

4. Adjunct / Anxiolytics (প্রয়োজনে)

  • Buspirone (anxiety relief)
  • Tablet 5.Anfree (0.5mg+ 10mg ) AS A (Flupentixol+Melitracen) AS A Combined anxiolytics & anti-depressant drugs.
    • Dose: 15–30 mg/day
    • Side effects: Dizziness, headache
    • Interactions: MAO inhibitors

Important: Antidepressants কার্যকর হতে 2–6 সপ্তাহ সময় লাগে।

B. Non-Pharmacological Treatment

Overview / Summary

Depressive neurosis (Persistent Depressive Disorder) হলো chronic mild-to-moderate depression।
Management pharmacological (SSRIs, SNRIs, TCAs)
এবং non-pharmacological (CBT, lifestyle) উভয়ই প্রয়োজন। Prevention মূলত stress reduction এবং early intervention।

Actionable Plan / Steps

  1. Detailed history + mental status exam
  2. Rule out secondary causes via labs
  3. Start SSRIs (Fluoxetine/Sertraline) 2–6 সপ্তাহ trial
  4. Monitor side effects & response
  5. Add psychotherapy (CBT/IPT)
  6. Educate patient on lifestyle & social support
  7. Long-term follow-up 6–12 months

Purpose / Use Explained (Why Used Specifically)

  • SSRIs (Fluoxetine, Sertraline): First-line therapy; increase serotonin → improve mood & reduce anxiety.
  • SNRIs (Venlafaxine): For resistant depression or when both serotonin & norepinephrine support needed.
  • TCAs (Amitriptyline): Chronic depression with insomnia or pain symptoms; second-line due to side effects.
  • Buspirone: Adjunct for anxiety without sedation or dependency risk.

Key Points

  • Antidepressants take 2–6 weeks to show effect.
  • Start low dose, titrate gradually.
  • Monitor side effects: especially cardiovascular for TCAs, bleeding for SSRIs.
  • Combine with psychotherapy & lifestyle modification for best outcomes.
