(Chorea)
(Cognitive change)
(Behavioral Change)
Huntington’s Disease.
Caudate nucleus
Putamen (Striatum)
- Answer is:- Genetic test korte dibo(PCR) to see gene repeation.
(Symptoms)
(Causes)
(Autosomal Dominant Inheritance)
(Diagnosis)
(Clinical Assessment)
(Cognitive Testing):
(Genetic Testing)
(Genetic Counseling):
(Neuroimaging)**
- MRI (Magnetic Resonance Imaging):
- CT Scan:
(Prevention)
(Genetic Counseling)
(Pre-Konception Testing):
(Lifestyle Modifications & Supportive Care)**
(Symptomatic Treatment)
(Summary)

- Chorea (involuntary movements)
- Psychiatric/Behavioral symptoms (depression, anxiety, irritability, psychosis)
- Cognitive difficulties (attention, executive function)
- Maintain Quality of Life:
- Preserve balance, gait, and fine motor function as long as possible
- Support speech, swallowing, and nutrition
- Manage mood swings, depression, and agitation
- Multidisciplinary Care Team:
- Neurologist/Movement-Disorders Specialist: Oversees pharmacotherapy and monitors progression
- Psychiatrist/Psychologist: Manages mood, behavior, and cognitive therapies

- Speech & Language Therapist: Addresses dysarthria (slurred speech) and dysphagia (swallowing difficulties)
- Nutritionist/Dietitian: Ensures adequate caloric intake and monitors weight (HD patients often require extra calories due to choreic movements)
- Social Worker/Counselor: Provides support for patients and caregivers, links to support groups
2. Pharmacological Management of Chorea
Chorea is one of the most distressing motor features in Huntington’s Disease. When choreic movements interfere with daily activities—or pose a safety risk (e.g., frequent falls)—pharmacotherapy is indicated.
2.1 Tetrabenazine (TBZ)
- Mechanism of Action: Vesicular monoamine transporter 2 (VMAT2) inhibitor. Reduces presynaptic dopamine storage, thereby decreasing involuntary movements.
- Typical Adult Dose:
- Start at 12.5 mg once daily (preferably in the morning).
- Titrate by 12.5 mg increments every week as tolerated, aiming for 50–75 mg per day divided into two or three doses (e.g., 25 mg TID or 25 mg TID with breakfast, lunch, and dinner).
- Maximum: 100 mg/day in divided doses (caution required due to risk of depression, parkinsonism).
- Common Side Effects: Sedation, parkinsonian features (slowness, rigidity), depression, akathisia (restlessness), insomnia.
- Availability/Brands in Bangladesh:
- Generic Tetrabenazine: Available through import from Indian or European manufacturers; sold under names such as “Tetrabenazine TBZ” (check with major pharmacies in Dhaka, Chittagong, Sylhet).
- Cost Estimate: ~ 5,000–8,000 BDT per month (depending on dose and brand). Prices may vary; always confirm current pricing locally.
- Monitoring:
- Baseline and periodic depression screening (PHQ-9 or clinical interview).
- Liver function tests (LFTs) if prolonged use, although TBZ is primarily cleared hepatically.
- Assess for extrapyramidal signs (e.g., bradykinesia).
2.2 Deutetrabenazine (DTBZ)
- Mechanism of Action: Similar VMAT2 inhibition as TBZ, but with deuterium atoms for longer half-life and potentially fewer side effects.
- Typical Adult Dose:
- Start at 6 mg twice daily.
- Titrate by 6 mg per dose (i.e., 6 mg BID → 12 mg BID → 18 mg BID) every 1–2 weeks, targeting 36 mg/day (i.e., 18 mg BID).
- Max: 48 mg/day (i.e., 24 mg BID), though many patients achieve control at 24–36 mg/day.
- Side Effects: Similar to TBZ (drowsiness, nausea, depression), but some patients tolerate better.
- Availability in Bangladesh:
- Deutetrabenazine (brand “Austedo”) is not yet officially registered in Bangladesh (as of early 2025). However, some neurologists import it for select patients under special license (check “BRB” or “Incepta” import records).
- Cost Estimate: Very high (~ 25,000–35,000 BDT per month), often not covered by insurance.
- Monitoring: Same as TBZ (mood, LFTs, motor signs).
2.3 Antipsychotics (Off-Label for Chorea)
When VMAT2 inhibitors are contraindicated (e.g., severe depression) or poorly tolerated, low-dose antipsychotics can be used to reduce chorea by dopamine blockade.
- Risperidone
- Dose for Chorea: Start at 0.25–0.5 mg once daily; can increase to 1–2 mg/day in divided doses.
- Side Effects: Sedation, weight gain, metabolic syndrome (monitor blood sugar/lipids), extrapyramidal side effects (EPS).
- Availability: Widely available as generics (e.g., “Risperdal” generics from Beximco, Square). Cost: ~ 500–800 BDT/month at 1 mg/day.
- Olanzapine
- Dose for Chorea: 2.5 mg once daily; can be titrated to 5–10 mg/day based on response.
- Side Effects: Significant weight gain, sedation, metabolic disturbances.
- Availability: Widely available generics (“OlanZia” by Renata, “Olaxt” by Square). Cost: ~ 900–1,200 BDT/month at 5 mg/day.
- Haloperidol
- Dose for Chorea: 0.5–1 mg once daily to start; can go up to 2–3 mg/day in divided doses.
- Side Effects: High risk of EPS (parkinsonism, akathisia), tardive dyskinesia with long-term use.
- Availability: Generic haloperidol (e.g., “Haldol” generics by popular Bangladeshi pharma). Cost: ~ 300–500 BDT/month at 1 mg/day.
- Risks & Precautions:
- Because antipsychotics can worsen cognition and lead to sedation, most neurologists reserve them for patients who cannot tolerate TBZ/DTBZ or when chorea coexists with psychotic symptoms.
- Regularly monitor weight, fasting blood glucose, and lipid profile if using atypical antipsychotics (risperidone/olanzapine).
2.4 Benzodiazepines (Adjunct for Severe Dyskinesia)
- Examples: Clonazepam 0.5–1 mg once or twice daily (may ease anxiety and reduce chorea intensity).
- Side Effects: Sedation, risk of dependence, cognitive slowing.
- Availability: Readily available generics (“Clonotril,” “Panazol”). Cost: ~ 200 BDT/month at 1 mg/day.
- Use: Typically reserved for short-term relief during chorea exacerbations or if anxiety is prominent.
3. Management of Psychiatric and Cognitive Symptoms
Behavioral and mood symptoms often appear early and may be as disabling as motor signs.
3.1 Antidepressants
- Selective Serotonin Reuptake Inhibitors (SSRIs):
- Sertraline (Zoloft/generic): Start at 25 mg once daily; increase to 50–100 mg/day as needed.
- Availability: Generics by Renata, Opsonin, Incepta. Cost: ~ 600–1,000 BDT/month at 50 mg/day.
- Fluoxetine (Prozac/generic): Start at 10 mg once daily; can increase to 20–40 mg/day.
- Availability: Generics by Eskayef, Square, Beximco. Cost: ~ 500 BDT/month at 20 mg/day.
- Escitalopram (Lexapro/generic): Start at 5 mg once daily; titrate to 10–20 mg/day.
- Availability: Generics by Incepta, Beximco. Cost: ~ 1,200 BDT/month at 10 mg/day.
- Usage Notes: SSRIs help treat depression, anxiety, and obsessive‐compulsive features. They may also modestly improve irritability.
- Sertraline (Zoloft/generic): Start at 25 mg once daily; increase to 50–100 mg/day as needed.
- SNRIs (Serotonin–Norepinephrine Reuptake Inhibitors):
- Venlafaxine (Efexor/generic): Start at 37.5 mg once daily; can increase to 75–150 mg/day.
- Duloxetine (Cymbalta/generic): Start at 30 mg once daily; increase to 60 mg/day.
- Availability/Cost:
- Venlafaxine generics by Renata, Eskayef – ~ 1,000 BDT/month at 75 mg/day.
- Duloxetine generics by Incepta, Beximco – ~ 1,500 BDT/month at 60 mg/day.
- Usage Notes: Reserved for patients with prominent neuropathic pain or when SSRIs are not tolerated.

Note: Cognitive‐enhancing drugs have variable benefit in HD and should be prescribed by a specialist familiar with HD.
4. Supportive & Adjunctive Therapies
- Physiotherapy:
- Focus on stretching, balance exercises, gait training, and strength.
- Aim to prolong ambulation and reduce falls.
- Many tertiary hospitals in Dhaka (e.g., BSMMU, NITOR) have neuro‐rehabilitation units.
- Occupational Therapy:
- Training in using adaptive devices (weighted utensils, hand splints) to facilitate self-feeding, dressing, and writing.
- Home modifications (grab bars in bathrooms, non-slip mats).
- Speech & Language Therapy:
- Techniques to manage dysarthria (slurred speech) and dysphagia (swallowing therapy).
- Diet adjustments (thickened liquids, soft diet) to reduce aspiration risk.
- Nutritional Support:
- High-calorie, high-protein diet: HD patients burn more calories due to involuntary movements.
- Small, frequent meals if swallowing difficulty is present.
- Consider supplementation with vitamins (e.g., multivitamin, vitamin D, calcium) if malnutrition or weight loss is significant.
- Psychosocial Support:
- Family Counseling: HD carries reproductive implications (50% risk for offspring), so preconception genetic counseling is essential.
- Support Groups:
- Huntington’s Disease Association of Bangladesh (HDBA) – informal gatherings in Dhaka and Chittagong.
- Online communities (e.g., Facebook groups) where families share coping strategies.
- Respite Care: Periodic day-care services for HD patients (e.g., some NGOs in Dhaka offer short-term home care).

6. Monitoring & Follow-Up
- Regular Neurology Visits (Every 3–6 Months):
- Adjust chorea medication dosing based on efficacy and side effects.
- Monitor for new or worsening psychiatric symptoms.
- Perform routine examinations (motor, cognitive, psychiatric).
- Periodic Laboratory Tests:
- For VMAT2 Inhibitors (TBZ/DTBZ): Baseline LFTs, periodic depression screening.
- For Mood Stabilizers:
- Valproate: LFTs every 3–6 months, platelet count annually.
- Carbamazepine: CBC every 6 months, serum sodium.
- Lithium: Serum lithium, creatinine, TSH every 3 months.
- Side Effect Surveillance:
- Watch for sedation, parkinsonian features, weight gain, metabolic syndrome (especially if on antipsychotics).
- Screen for suicidality or worsening depression at each visit (use validated scales like PHQ-9).
- Cognitive/Psychiatric Assessment (Annually):
- Formal neuropsychological testing if cognitive decline accelerates.
- Adjust antidepressants or antipsychotics accordingly.
- Rehabilitation Review (Every 6 Months):
- Physiotherapist and occupational therapist to re-evaluate gait, balance, ADLs.
- Speech therapist to reassess swallowing safety and communication needs.
7. Special Considerations in Bangladesh
- Access to Specialist Care:
- Dhaka has the major tertiary centers (BSMMU, NINS, Square Hospitals, United Hospital) where neurologists experienced in movement disorders treat HD. Outside Dhaka, specialists may be scarce, so coordination with a Dhaka‐based neurologist via telemedicine is often necessary.
- Genetic Testing:
- HTT genetic testing is available at some private diagnostic labs (e.g., Medinova Genetics Lab, Islami Bank Diagnostic Center). Cost: ~ 25,000–30,000 BDT for CAG repeat analysis.
- Financial Assistance & Support:
- Some NGOs (e.g., Anjuman Mofidul Islam) occasionally offer subsidies for expensive medications (e.g., TBZ) or help link patients to drug assistance programs in India.
- Private initiatives like “BD Neurology Support” sometimes collect donations to help indigent HD families cover medication costs.
- Cultural & Social Aspects:
- HD carries a 50% inheritance risk for offspring. Pre-marriage and pre-conception genetic counseling is crucial to inform family planning.
- Social stigma around psychiatric symptoms and movement disorders can lead families to hide the disease—emphasizing community education is important.
- Emerging Therapies (Not Yet Widely Available):
- Experimental Gene Silencing (Antisense Oligonucleotides): Ongoing global clinical trials (e.g., Ionis/Roche’s Tominersen) are not yet available in Bangladesh. Families interested may enroll in multinational registries (e.g., Enroll-HD) for future trial notification.
- Stem Cell Therapies: In preclinical stages—no approved indications yet.
8. Summary of Key Points
- First‐Line for Chorea: Tetrabenazine (generic) is the mainstay; Deutetrabenazine is an alternative if available.
- Alternatives for Chorea: Low‐dose antipsychotics (risperidone, olanzapine, haloperidol) when VMAT2 inhibitors are not tolerated or contraindicated.
- Psychiatric Management: SSRIs (sertraline, fluoxetine, escitalopram) for depression/anxiety; mood stabilizers (valproate, carbamazepine, lithium) for mood lability; antipsychotics for psychosis.
- Supportive Care: Physiotherapy, occupational therapy, speech therapy, nutritional support, and psychosocial interventions are essential to maintain function and quality of life.
- Local Availability: Most medications (VMAT2 inhibitors, antipsychotics, SSRIs, mood stabilizers) are available as generics in Bangladesh. High‐cost drugs (Deutetrabenazine, gene therapies) are largely inaccessible or prohibitively expensive.
- Monitoring: Regular clinic visits (every 3–6 months) for side-effect surveillance, dose adjustments, and multidisciplinary assessments. Periodic lab tests are required for mood stabilizers and VMAT2 inhibitors.
- Genetic Counseling & Testing: Available in Dhaka via private labs; critical for family planning.
Final Notes
- Always consult a qualified neurologist before starting, changing, or stopping any medication.
- Individual tolerance to TBZ or antipsychotics varies; dose titration should be gradual, with close monitoring for depression and parkinsonism.
- Encourage families to join support groups in Dhaka or online to share resources, coping strategies, and caregiving tips.
This treatment summary provides a framework for Huntington’s Disease management in the Bangladeshi context. Specific drug brands, costs, and availability may change over time—always verify details with your local neurologist and pharmacy.
