Q. Ventilator-associated pneumonia কোন bacteria করে?

→ Pseudomonas aeruginosa

Q. Pseudomonas কি Enterobacteriaceae এর member?

→ না।

Q. কেন?

→ কারণ Pseudomonas:

– Strict aerobe

– Oxidase positive

– Non-fermenters of glucose & others

– Don’t produce nitrites from nitrates

Q. Pseudomonas এর কী কী virulence factor আছে?

– Endotoxin

– Exotoxin: Exotoxin A

– Type III secretion system

– Exo-enzymes: Four → S, U, T, Y

– Enzymes: Protease & Elastase

– Pili

– Pigment

Q. Pseudomonas কী কী pigment produce করে?

– Pyocyanin → Blue pigment (only P. aeruginosa)

– Pyoverdin (Fluorescein) → Yellow-green pigment

– Pyorubrin → Red pigment

– Pyrorubranine → Brownish-black pigment

Q. Type-III secretion system কী?

→ এটা bacterial syringe-er মতো কাজ করে,

→ Exotoxin-কে bacteria থেকে directly adjacent human cell-এ transfer করে

→ তাই antibody exotoxin-কে neutralize করতে পারে না

Q. Pseudomonas কী কী disease করতে পারে?

– UTI

– Pneumonia (specially in cystic fibrosis patients)

– Wound infection in burn patient

– Sepsis & septic shock

– Ecthyma gangrenosum

– Malignant otitis externa (esp. in DM patients)

– Osteomyelitis etc.

Q. Pseudomonas-এর culture করার সময় কী characteristic feature দেখা যায়, যেটা দেখে সহজেই diagnosis করা যায়? সেটা কী?

→ Fruity aroma

Q. Pseudomonas-এর against কী কী antibiotic use করা হয়?

– Piperacillin + Tazobactam, Ticarcillin + Clavulanate

– Aminoglycosides: Gentamicin, Amikacin, Tobramycin

– 3rd Gen Cephalosporin: Ceftazidime, Cefepime

– FQs: Ciprofloxacin

– Colistin (Polymyxin E)
