Hello I'm Raiyyan Tahsan

TC(438).(26).Pulmonology Case No 13. COPD

পুরুষ, known case of COPD, Smoker।

-Orthopnea

Vitals

-Pulse: 94 bpm

-BP: 134/86 mmHg

-RR: 10/min ↓

-SpO₂: 95%(with oxygen)

Why?

pH:7.20

pCO2:75 mmHg

HCO:32 mmol/L

-Type 2 Respiratory Failure (Hypercapnic) with CO₂ narcosis

→ Most likely due to oxygen over-correction in COPD

-Peripheral chemoreceptor (in carotid & aortic bodies)

Normally, deoxygenated hemoglobin গুলো more CO₂ ক্যারি করে।

Further rise in blood pCO₂

High pCO₂ → Cerebral vasodilation → raised intracranial pressure →

→ Confusion → drowsiness → unconsciousness → coma → arrest

Maintain SpO₂ between 88–92%

→ This gives enough oxygen without knocking out their hypoxic drive

Chronic Obstructive Pulmonary Disease (COPD) is a progressive lung disease characterized by airflow limitation that is not fully reversible. It includes chronic bronchitis and emphysema. The main goals of treatment are to relieve symptoms, improve quality of life, reduce the risk of exacerbations, and slow disease progression.

1. Lifestyle Modifications

  • Smoking cessation (most important): Strongly recommended in all patients who smoke.
  • Avoidance of environmental pollutants: Reduce exposure to occupational or indoor pollutants.
  • Vaccinations: Influenza, pneumococcal (PCV13 and PPSV23), and COVID-19 vaccines.

2. Pharmacologic Treatment

Bronchodilators

  • Short-acting bronchodilators (SABAs/SAMAs): For symptom relief.
    • e.g., Albuterol (SABA), Ipratropium (SAMA)
  • Long-acting bronchodilators (LABAs/LAMAs): Maintenance therapy.
    • e.g., Salmeterol (LABA), Tiotropium (LAMA)

Inhaled Corticosteroids (ICS)

  • Used in patients with frequent exacerbations and high eosinophil counts.
  • Often combined with LABAs: e.g., Fluticasone/Salmeterol

Triple therapy

  • LABA + LAMA + ICS for severe cases with frequent exacerbations.

Phosphodiesterase-4 Inhibitors

  • Roflumilast: In severe COPD with chronic bronchitis and frequent exacerbations.

Theophylline

  • Rarely used due to side effects; used when other treatments fail.

3. Oxygen Therapy

  • For patients with chronic hypoxemia (PaO₂ < 55 mmHg or SpO₂ < 88%).

4. Pulmonary Rehabilitation

  • Multidisciplinary program including exercise training, education, and behavior change.

5. Surgical Interventions

  • Lung volume reduction surgery
  • Bullectomy
  • Lung transplantation: In selected end-stage cases.

6. Management of Exacerbations

  • Short-acting bronchodilators
  • Systemic corticosteroids (e.g., Prednisone)
  • Antibiotics (if bacterial infection is suspected)
  • Oxygen and ventilation support, if necessary
