১১ বছরের বাচ্চা post cervical lymph node swelling (inguinal lymph node ও mild swelling ছিলো) , fever, nausea আর মুখ (face) ফোলা নিয়ে এসেছিল।
CBC তে দেখা গেলো wbc, ESR বেশি । Infection suspect করে antibiotic, antipyretic দিয়ে পেশেন্ট কে বাসায় পাঠায় দেয়া হলো।
কিন্তু পেশেন্ট 2 দিন পরে আবারও হাজির। পেশেন্ট এর পুরা বডি ফুলে গেছে। এরপরে অনুসন্ধান করে পাওয়া গেলো, পেশেন্ট এর গায়ে চুলকানি (recurrent scabies) আছে। হিস্টরি নিয়ে আরো জানা গেলো তার ইউরিন output near to zero।ডায়াগনোসিস এর 90% পথ ই খুলে গেলো এখন , শুধু ছোট্ট একটা history র জন্য ।
টেস্ট করে পাওয়া গেলো Serum creatinine high , RBC present in urine.. high blood pressure…this is a suspected case of Acute Glomerulo-Nephrities following scabies.
